(4) In case of original pass book being lost, or mutilated or damaged, a duplicate pass book maybe issued on payment of a fee of rupees ten in case of issue of first duplicate pass book and rupeestwenty in case of any subsequent issue, on a simple application on plain paper. The issue of duplicatepass book(s) shall be noted by the deposit office in its office records including the ledger folio bearingparticulars of the account.(5) In case the lost passbook is found after issue of a duplicate passbook, it shall not be treated asvalid for any purpose and shall be surrendered immediately to the deposit office who shall destroy thesame immediately in presence of the depositor.11. Transfer of account from one deposit office to another: - A depositor mayapply on FORM G, enclosing the pass book thereto, for transfer of his account fromone deposit office to another.Provided that where the deposit is rupees one lakh or above, a transfer fee of rupees five perlakh of deposit for the transfer and rupees ten per lakh of deposit for the second and subsequenttransfer shall be payable.
