P R E O P E R A T I V EP L A N N I N GPreoperative templating is essential to the precisereconstruction of the hip joint (Figure 5). Suggestedpreoperative X-rays include an A-P of the pelvis andhips, a 14" x 17" A-P view of the affected hip andfemur, and a lateral view of the affected hip.The acetabular component may be templated usingthe contralateral normal hip, if available, ortemplated directly on the affected hip. The acetabularcomponent should congruently fit the subchondralbone and the medial aspect of the acetabulum asindicated by the teardrop. Note the center of rotationof the acetabular component through the template forsubsequent reference.Figure 5A C E T A B U L A R E X P O S U R E A N D R E A M I N GFigure 6Complete exposure of the acetabulum is necessary toensure a satisfactory surgical result. Resect theacetabular labrum circumferentially in order todefine the landmarks of the bony acetabulum. Cleansoft tissue or osteophytes from the acetabular fovea inorder to define the limits of the medial wall.Surrounding soft tissues must be protected by retractionduring the reaming process to avoid injury to criticalstructures.Restoration of normal anatomy is an important principleof reconstructive hip surgery. The acetabulummust be medialized to restore the normal center ofthe hip rotation as determined by preoperativetemplating.Ream the acetabulum concentrically in order toensure an excellent fit between the acetabulum andacetabular component (Figure 6).AcetabularReamer DomeAcetabular Reamer Handle5
