PRESENTATION/FORMATCONTENT<strong>EXHIBIT</strong> A<strong>VENDOR</strong> <strong>EVIDENCE</strong> <strong>OF</strong> <strong>RESPONSIBILITY</strong>Exhibit A – Vendor Evidence of Responsibility shall be submitted on 8 1/2" x 11"paper size, using a simple method of fastening. Proposals should be typed andshould not include any unnecessarily elaborate or promotional material. Lengthynarrative is discouraged; presentations should be brief and concise. Theproposal should not exceed 30 pages in length, excluding appendices, if any andor product catalog(s). The form, content and sequence of the proposal shouldfollow the outline presented below.Transmittal Letter/IntroductionThe letter of transmittal shall be addressed to the Contracts &Procurement Manager and must, at a minimum, contain the following:identification of the offering contractor(s), including name, addressand telephone number of each contractor;a statement to the effect that the proposal shall remain valid for aperiod of not less than ninety (90) days from the due date forproposals;a statement to the effect that work or service(s) requested forOCHS’ Fleet of Vehicles will commence no later than July 1, 2013.identification of any information contained in the proposal which thePotential Contractor deems to be, and establishes as, confidential orproprietary and wishes to be withheld from disclosure to othersunder the state Public Records Act (a blanket statement that allcontents of the proposal are confidential or proprietary will not behonored by OCHS); and,signature of a person authorized to bind the offering company tothe terms of the proposal.Qualifications, Related Experience and ReferencesOverview: This section should establish the ability of the PotentialContractor (and its subcontractors, if any) to satisfactorilyperform the required work by reasons of: demonstratedcompetence in the services to be provided; the nature andrelevance of similar work currently being performed orrecently completed; competitive advantages over othercompanies in the same industry; strength and stability as a1 OCHS RFP 13-2005 VEHICLE FLEET MAINTENANCE & REPAIR SERVICES


usiness concern; and supportive client references.Information should be furnished for both the PotentialContractor and any subcontractors included in the offer.Furnish background information about your company, including date offounding, legal form (sole proprietorship, partnership, corporation/stateof incorporation), number and location of offices, principal lines ofbusiness, number of employees, days/hours of operation and otherpertinent data. Disclose any conditions (e.g., bankruptcy or otherfinancial problems, pending litigation, planned office closures, impendingmerger) that may affect the Potential Contractor's ability to performcontractually. Certify that the company is not debarred, suspended orotherwise declared ineligible to contract by any federal, state or localpublic agency.Certify that your company is properly licensed to do business in the Stateof California and you comply with all applicable state and local laws.Describe your company’s most noteworthy qualifications for providing therequired services to OCHS. Specifically, highlight those qualifications thatdistinguish you from your competitors.Describe the range of activities performed by the location from which thework will be done mobile on-site at OCHS vs. off-site services (e.g.,discuss distribution center for supplies, customer service support forordering/invoicing/returns, perform in-depth account analysis andreviews, limited or full-services shop, # of vehicles in mobile fleet, # ofmechanics in mobile fleet, # of off-site repair locations includingaddresses, negotiations for volume discounts with manufacturers, etc.)Discuss this office's experience in furnishing Vehicle Fleet Maintenance &Repair Services particularly with similar programs, corporations, local,state or federal governments, etc…, implementing cost savingstechniques, and aggressive negotiating with manufacturers for volumepurchase(s). Provide a list of business clients--especially private nonprofitorganizations, and federal and state categorical aid programs--to whichyour company has recently provided similar services. Include companynames, beginning/ ending dates of contracts, and names, titles andtelephone numbers of individuals that OCHS can contact as references foryour company.Beyond your ability to provide Vehicle Fleet Maintenance & RepairServices, what additional value-added service(s) and or products can youprovide for a fee or thru Non-Federal Match? Include a brief description of theirqualifications, professional certifications, job functions and officelocation(s). Optional: Furnish brief resumes for all key personnel;include these as an appendix, not in the body of the proposal.If more than two people will be assigned to OCHS’ account(s),include a simple organization chart which clearly delineatescommunication/reporting relationships among members of theservice team.Service AgreementComplete Exhibit C - OCHS’ Service Agreement including Statement ofCompliance form. By signing you are agreeing to the terms andconditions as set forth in this RFP and Service Agreement.Exceptions/DeviationsAppendicesState any exceptions to or deviations from the requirements of this RFP,including the Service Agreement presented in Exhibit C. If you wish topresent alternative approaches to meet OCHS' work requirements, theseshould be thoroughly explained.Additional Information: Include any additional information you deemessential to a proper evaluation of your proposal and which isnot solicited in any of the preceding sections.
