You canbe referred to the palliative care team at anypoint of your illness.MDT co-ordinatorThe clerical member of the team who coordinatesthe team and their meetings, recordsthe discussions and helps to track patients alongtheir care pathway.The multi-disciplinary team (MDT) meets everyMonday to discuss individual cases. All relevantinvestigations such as scans, results of biopsiesand blood tests are discussed and the teamdecide on what treatment to recommend. Theresults of your case will be communicated withyou either in clinic or via a telephone call fromthe nurse specialist. You may be required toattend other hospitals for investigations ortreatment.Please ask questions about your investigations,diagnosis and treatment plan, it is importantthat you understand and are involved in thedecision making process.The team work together with you and yourGP to give you and your family any help andsupport you may need.
