Williams, MD, of ColumbiaUniversity.The 2-hour <strong>for</strong>um will begin withtwo reports on technologies <strong>for</strong> thetreatment of mitral regurgitation.The first is a presentation of earlyfindings regarding the use of a novelleaflet plication clip (LCP) <strong>for</strong> the repairof mitral valve prolapse, whichwill be presented by Eric N. Feins,MD, of Boston Children’s Hospitaland Harvard Medical School.The LCP, which was designed totreat a prolapsed leaflet segment –mimicking a segmental leaflet resection– was successful <strong>for</strong> treating mitralregurgitation secondary to mitralvalve prolapse in an animal model.In previous ex-vivo studies, the devicewas shown to successfully eliminateP2 segment prolapse andmitral regurgitation in fresh swinehearts.Dr. Feins will present data that suggestthe device has the potential toimprove the effectiveness of beatingheartvalve surgery, and may enhancethe ease and reproducibility of leafletplication in patients undergoingopen-heart minimally invasive valvesurgery.The second paper is a report ofearly results from the TransapicalPlacement of Artificial ChordaeTendinae (TACT) Trial – an industry-sponsored,multicenter, prospective,non-randomized study toevaluate a new technology <strong>for</strong> offpump,beating-heart correction ofmitral regurgitation.The technology involves placementof an artificial chorda tendina(ACT) using the Neochord chordaldelivery system (Neochord Inc.Minneapolis).Giovanni Speziali, MD, of the Universityof Pittsburgh Medical Center,will present findings in the first 30patients, 26 of whom were treatedsuccessfully. Although the overalldurability of mitral regurgitation repairwas only 65% at 30 days amongthose who experienced acute proceduresuccess, refinements of surgicaltechnique throughout the study dramaticallyimproved the results at 30days in the latter half of the studypopulation, according to Dr. Speziali.Among the other emerging technologiesand techniques that will bediscussed are: A novel surgical technique <strong>for</strong> per<strong>for</strong>mingsympathectomy by embryonicnatural orifice transumbilicalendoscopic surgery using a nasal gastroscope<strong>for</strong> the treatment of patientswith hyperhidrosis. WeishengChen, MD, of Fuzhou General Hospital,will present 1-year results in 35patients showing that 97% and 72%of those with palmar and axillary hyperhidrosis,respectively, were treatedsuccessfully. The use of thoracic aortic endograftsto facilitate the resection oftumors infiltrating the aorta.Stephane Collaud, MD, of TorontoGeneral Hospital will present datafrom five patients suggesting thatthis innovative approach is safe andeffective. After a median of 30months, all five patients – who hadeither non–small cell lung carcinomasor sarcomas infiltrating the aorta– were alive and disease-free, andnone suffered complications fromthe endograft. A laser-assisted zero ischemia coronaryanastomotic connector, whichserves as a simplified alternative <strong>for</strong>hand-sutured anastomosis to facilitateminimal access and closed chest coronaryartery bypass grafting (CABG).David Stecher, MD, of UniversityMedical Center, Utrecht, will presentfindings from a porcine model showingthat the connector enabled fast,simple and reliable anastomosis construction,and that it has potential <strong>for</strong>minimally invasive or endoscopicCABG.“The technological field is changingrapidly,” Dr. Rao said, notingthat the <strong>for</strong>um will provide attendeeswith an improved awareness ofthese and other emerging technologiesthat they may be inclined toemploy in their own practice, andwill provide a glimpse of the futurelandscape in cardiothoracic technology.Dr. Rao disclosed that he has receivedcompensation <strong>for</strong> serving asan advisory board member or consultant<strong>for</strong> Medtronic, CorMatrixCardiovascular, Terumo Inc., andHeartWare Inc. Ferguson, MD,of the University of Chicago, presenteda modified scoring system topredict major cardiovascular eventsafter lung resection, which was developedusing the Society of <strong>Thoracic</strong>Surgeons General <strong>Thoracic</strong>Database. “We analyzed outcomes<strong>for</strong> the period 2003 to 2011 <strong>for</strong> patientshaving elective major lung resection,applying the ThRCRI Riskscoring system. We found thatThRCRI successfully stratified patientsat risk <strong>for</strong> major postoperativecardiovascular complications,”Dr. Ferguson said.The ThRCRI Risk score was basedon weighted values <strong>for</strong> serum creatinine,coronary artery disease, cerebrovasculardisease, and extent oflung resection, and was stratified intofour risk categories: 0 (A), 1-1.5 (B),2-2.5 (C), and greater than 2.5 (D).The major cardiovascular complicationsfollowed included myocardialinfarction, ARDS, ventricular arrhythmiarequiring treatment, andall-cause death.The four scoring categoriesdemonstrated significantly differentMark K. Ferguson, MD, presented ascoring system <strong>for</strong> major lung resection.incremental risks of major cardiovascularcomplications, ranging fromaround 3% <strong>for</strong> category A, nearly 6%<strong>for</strong> category B, to over 11% <strong>for</strong> categoriesC and D. This expected riskcalculated in the study group wasfound to be similar to the actual observedrisk, indicating that ThRCRIaccurately predicted specific riskrather than just relative risk, said Dr.Ferguson.This finding could have importantclinical relevance. “Using the ThRCRIscoring system may help in identifyingpatients who may benefit fromadditional preoperative evaluationand from closer perioperative monitoring,”Dr. Ferguson concluded.Pradheep Krishnamohan, MD, andhis colleagues from the Mayo Clinic,per<strong>for</strong>med a retrospective record reviewof around 500 patients who underwentlaparoscopic myotomybetween April 1998 and June 2011.Patients’ follow-up was per<strong>for</strong>medusing a standardized dysphagia questionnaire.Laparoscopic myotomy with partialfundoplication was per<strong>for</strong>med inover 95% of the patients, the rest receivingmyotomy alone. There wasno operative mortality. Complicationswere relatively low.Analysis of all patients that returnedthe questionnaires revealedthat around 70% reported heartburn,more than one-third had regurgitation,and around 68% took antacids,H2 blockers or PPIs within the past30 days of the survey.“Our study shows that laparoscopicmyotomy with partial fundoplicationis a safe operation with extremely effectiveoutcomes in about one-thirdof patients, with those patients whoPradheep Krishnamohan, MD, studiedpatients who had laparoscopic myotomy.were older than 65 years tending tohave better outcomes,” Dr. Krishnamohanconcluded.Outcomes <strong>for</strong> sub-lobar resection(SLR) were equivalent to those <strong>for</strong>lobectomy <strong>for</strong> treatment of earlystage non–small cell lung cancer(NSCLC), according to a large,prospective study presented by NasserK. MINNEAPOLIS CONVENTION CENTER • MINNEAPOLIS, MN • MAY 4 – 8, 2013 AATS 93 RD ANNUAL MEETING 17Congenital Simultaneous Session Focuses on TOFMonday’s Congenital Heart<strong>Surgery</strong> Simultaneous Sessionaddressed attempts toimprove Tetralogy of Fallot repairand on using left ventricle assist device(LVAD) technology to treatchildren with a single functionalventricle, as well as studies to limitcomplications in treating other importantcongenital heart conditions.Despite hopes that the use of amore limited right ventriculotomywould lead to a lowered incidenceof ventricular dilatation and dysfunctionin patients undergoingtransannular repair of Tetrology ofFallot (TOF), there were no benefitsseen <strong>for</strong> using this procedure comparedwith the standard operation,said Cheul Lee, MD, of the SejongGeneral Hospital.Because long-term complicationsof conventional transannular repairwith a large right ventriculotomy(RV-tomy) in patients with TOF arecommon, many centers adopted theuse of transatrial and transpulmonaryapproach with a limited (lessthan 1 cm) transannular RV-tomy inhopes of getting better results, despitea lack of evidence of long-termresults.Dr. Lee and his colleagues per<strong>for</strong>meda study in more than 113 patientsto test whether the use of amore limited RV-tomy would, in fact,result in less right ventricular (RV) dilatationand dysfunction, comparedwith conventional RV-tomy.Between June 2002 and April 2012,patients who had transannular repairContinued from previous pagelung cancer–specific (LSC) survivalin more than 500 patients with clinicalstage 1A NSCLC who had eitherlobectomy or SLR resection inthe International Early Lung CancerAction Program (I-ELCAP)from 1993 to 2011. They evaluateddifferences in the demographic andclinical characteristics between thelobectomy and SLR groups and determinedthe LCS survival asymptoteusing Kaplan-Meier.“We identified patients in whomthe cancer presented as a solidnodule (with the greater majorityof tumors less than or equal to 2cm as determined by CT screening)who were treated with eitherlobectomy or SLR and found thatthere was no difference betweenthe two groups in median age [63years], sex, presence of hypertension,COPD, cardiac disease, orCheul Lee, MD discussed the longtermresults of a limited RV-tomy.other major comorbidities,” saidDr. Altorki.Overall, there was no significantdifference found in the 14-yearLCS survival rates <strong>for</strong> SLR andlobectomy, with both slightly over90%. For those with tumors lessthan or equal to 2 cm, the LCSsurvival was also in the low 90%range <strong>for</strong> both procedures, a nonsignificantdifference.“In addition, regression analysisshowed there was still no significantdifference in survival ofthose who had lobectomy andSLR,” he said.The session, which also includedpapers on topics as diverse aspulmonary hypertension and postsurgicalfollow-up, was moderatedby Harvey I. Pass, MD, of theNew York University MedicalCenter, and David R. Jones, MD,of the University of VirginiaHealth System.MARTIN ALLREDof TOF at a mean age of 2.8 yearsunderwent magnetic resonance imaging(MRI) <strong>for</strong> evaluation of pulmonaryregurgitation at a mean ageof around 19 years.Patients were divided into limitedRV-tomy group (about one-third) andconventional RV-tomy group (abouttwo-thirds).An equal number of patientsfrom each group were matched <strong>for</strong>comparison using propensity scorematching with covariates of sex,age at repair, and history of priorshunt.There was no difference in thesepropensity score–matched comparisons,even though there was a longerinterval between repair and MRI examinationin conventional RV-tomygroup.“Our study couldn’t demonstrateany long-term benefits of using limitedRV-tomy, compared with conventionalRV-tomy in patients, at least interms of RV size and function,” heconcluded, stating that further studieswere necessary to determine whetherthere was a beneficial role of usinglimited RV-tomy <strong>for</strong> patients who undergotransannular TOF repair.In another examination of theproblems encountered after repairof TOF, Vladimiro Vida, MD, of theUniversity of Padua, and his colleaguesdid a study to examinewhether balloon dilation to preservethe pulmonary valve could be usedto prevent progressive pulmonaryvalve regurgitation leading to rightventricular failure after repair.Since 2008, more than 69 patientswith TOF were enrolled. Therewere 34 patients assigned to pulmonaryvalve preservation using acombination of early trans-atrial repairand intra-operative balloon dilationof the pulmonary valve annulus(Group 1).The remaining 35 patients underwentan early trans-atrial repair withtransannular patch repair and pulmonaryvalve (PV) cusp reconstruction(Group 2), Dr. Vida stated.The degree of pulmonary valve regurgitationwas none/mild in the majorityof the Group 1 patients, whichwas statistically better than in theGroup 2 patients. Follow-up was unequalbetween the 2 groups.“Pulmonary valve preservation usingintra-operative balloon dilationyielded better pulmonary valve functionand leads to improved right ventricularfunction in the mid term,compared to patients who underwenta classical trans-annular patch,” Dr.Vida concluded.Despite a rapidly increasing and favorableexperience with pediatric-specificventricular assist devices (VAD) tobridge children with severe heart failureto transplant or recovery, there isvery little data concerning the use ofVADs in children with functional singleventricle, according to Samuel Weinstein,MD, of the MontefioreMedical School, and his colleagues.Dr. Weinstein presented their largeprospective database study of childrenwith a single functional ventriclesupported with the Berlin HeartPediatric EXCOR VAD, comparedwith the results of those supportedwith biventricular circulation.The researchers reviewed the EX-COR Investigational Device Exemption(IDE) study database of over200 patients implanted between May2007 and December 2010. Around10% of the patients had a singlefunctional ventricle, with the majorityof these having a diagnosis of hypoplasticleft heart syndrome(HLHS).The difference in support days <strong>for</strong>single ventricle patients and the biventriclepatients was non-significant.Transplantation occurred in around42% of the 26 single ventricle patients,compared with 73% of the biventriclepatients who were transplanted orweaned.“The EXCOR device can provide abridge to transplant <strong>for</strong> children withsingle ventricle physiology, albeit lesssuccessfully than in children withbiventricular circulation. The ability to persuade isimportant, rather than just using thepower of your position, he said.Dr. Schaff then listed four keycharacteristics of leadership. You must live and adhere to thevalues of your group. Leaders should be expert in somedomain of the group that they lead. The leader must have a vision andplan. The leader must consider the interestsof colleagues first.“You need to have their back, andnot because leadership is a popularitycontest. Your colleagues will be persuadedbecause they know youwatch out <strong>for</strong> them. They will goalong with your decisions, even withmisgivings if they trust that you considertheir interests first.” And finally,the leader should be a role model,not a mentor. Mentoring involvesmultiple individuals and has becomesynonymous with counselor orguide.Role models, however, are peoplethat individuals compare themselvesto and who occupy the professionaland social roles to which the individualaspires, he said.<strong>Thoracic</strong> surgery is recoveringfrom a minor crisis in confidence becauseof a diminishing applicantpool, and needs role models becauseof this, he said. “Role models are thesurgeons who by dedication, skill andcharacter, drew you to our specialty,”said Dr. Schaff. “Don’t settle <strong>for</strong> beinga mentor, but rather, be a rolemodel.”The second important aspect ofthe AATS that Dr. Schaff addressedwas scholarship, the knowledge thatresults from study and research.“It is my strong belief that the bestresearch is done by those who achieveclinical excellence, because a clinicianknows the important questions to askand has insights into the best avenues<strong>for</strong> investigation. Likewise, the bestclinicians not only have busy practicesbut have thought about and writtenabout their experiences, so that otherswill use their methods and avoid thepitfalls they have observed,” he said.Dr. Schaff emphasized that researchin thoracic surgery is verybroad; it may involve molecular biology,integrated physiology, device development,or outcomes research. Allof these domains are important toour specialty, but what are the benefitsto the trainee? The first benefit to the residentmight be something as simple andpractical as career advancement. A second benefit of research is thesatisfaction of discovery and recognitionof the work through presentationand publication. And thirdly, in some instances thelaboratory experience will be a directlead into a career in investigation as asurgeon/scientist. Often, the techniques learned andthe concepts mastered while doingresearch can be applied later to problemsthat are wholly unrelated to theoriginal project.He gave an example from his ownlife, where in the research laboratory,he learned an important implicationfrom extensive research on the effectof ventricular volume on diastolicfunction. “I remember with crystalclarity the day the cardiologist whorefers many HOCM patients <strong>for</strong>surgery showed me the echocardiogramof a patient with apical diseaseand said, ‘too bad we don’t have aprocedure <strong>for</strong> diastolic dysfunction…this patient’s heart is too stiff.’ Atthat time I immediately recalled thediastolic dysfunction in reperfusedhearts and thought that the problemin this patient was not muscle stiffnessper se, but reduced ventricularvolume. And this extrapolation to anoperation <strong>for</strong> diastolic dysfunctionwas an unexpected benefit of research,20 years after my laboratoryexperience.“As I look out to you in the audience,I see present and future leadersin thoracic surgery.
