We are currently a candidate schoolfor the International Baccalaureate Primary Years Programme (IB PYP). For further information aboutthe IB and its programmes, please visit its website at http://www.ibo.org. At this time, we are acceptingapplications for rising kindergarten through 5 th grade students.Our application period runs from Monday, April 9, 2012 through Monday, April 30, 2012. We acceptapplications in our <strong>of</strong>fice from 8:00 a.m. until 4:00 p.m. daily. You must reside in Richmond County toattend <strong>Lake</strong> <strong>Forest</strong> <strong>Hills</strong> and have an application processed. Separate application packets must becompleted for each student. Siblings will be considered on a case- to-case basis. Incomplete or lateapplications will not be accepted.1. Please be careful to complete and sign the entire application. A contract is included in thispacket and must be signed by the parent/guardian <strong>of</strong> all applicants.2. Please provide all requested items including the original birth certificate, and copies <strong>of</strong> thereport card from the first semester <strong>of</strong> the 2011-2012 school year and any standardizedtest scores available, including the Criterion Referenced Competency Test (CRCT) resultsfrom Spring 2011 (for students currently in 4 th and 5 th grades ). <strong>Student</strong>s entering 4 th and5 th grades must have a score <strong>of</strong> 800 or above on the Math and Reading CRCT for 2012.3. In order to apply, you must have an 80 or higher in all core subjects (Math, Science, SocialStudies, English/Language Arts) and Conduct.4. <strong>Student</strong>s entering Kindergarten, 1 st , 2 nd or 3 rd grade will be asked to complete an on-siteassessment on either Tuesday, May 8 or Wednesday, May 9, 2012 from 4:00 to 5:00 pm.Kindergarten students will complete a readiness assessment. First, second and third gradestudents will complete a writing sample and a math assessment. You will receive a letterwith further information by mail. If you have not received your letter by Friday, May 4,2012, please call the school at (706) 737-7317.5. Include two self-addressed stamped #10 (4” x 9.5”) envelopes.6. A recommendation form is included in this application packet which should be completedby your child’s current teacher <strong>of</strong> English-Language Arts or Math. Include the signed andsealed copy <strong>of</strong> the recommendation form.7. If you have a change <strong>of</strong> address or phone number, it is your responsibility to notify theschool in writing. You should address the notice to the attention <strong>of</strong> the IB Coordinator orthe Principal.8. If you are not selected this year, you must submit a new application for the next schoolyear. We do not maintain a waiting list. Applicants will be notified by mail by Monday, May21, 2012.9.
