ConceptTo develop a low cost, consistent end of life care programme,available to all care homes. It will support the development ofnominated staff in residential and/or nursing homes tobecome End of Life Care Home Representatives (champions)implementing organisational change utilising the Route toSuccess for care homes.Allow areas supporting the programme to include localpolicies, guidance & literature to promote somestandardisation across settings.
