Patients of all ages are served. The diagnosis of Diabetes may beprimary or a comorbidity.Frequent Procedures/Services/Functions• Patient Education• Blood Glucose Monitoring• Nursing assessment• Multidisciplinary referralHours of Operation• Saturday through Wednesday,Clinic - 7 a.m. - 6 p.m. Saturday - Tuesday, 7 am - 5 p.m. WednesdayClasses - 3 p.m. - 5 p.m. Sunday, Monday, TuesdayCRITERIA FOR ADMISSION TO SERVICE• Outpatients can be referred by Physicians, Nursing Units, Dietitians, Other Clinics such asCardiology, EMS, CVT, Radiology• Inpatients are assessed per individual findings. Teaching Plans for education by Nursingstaff are made or teaching is done by the Diabetes Nurse Educator. Patients are given followupappointments in the Diabetes Education Clinic after discharge from hospital.STAFFING PLANNumber of Staff• Diabetes Nurse Educator 2• Translator 1Method of Adjusting to Staffing VariancesEach educator has regularly schedules Clinics. One Educator works in Physician Diabetes Clinics(3), Sunday morning one Educator has a Clinic in Family Medicine, One Educator does theDiabetes Self-management Classes. Arabic Diabetes Programs for staff are offeredevery July/August by one of the Diabetes Educators. Nursing Focus education is offered for allstaff in November, sessions given by Diabetes Educators. The Diabetes Educators keep updatedthrough the Internet, Diabetes Educator Magazines, Physician information. Gyn Clinic regarding GestationalDiabetes and Pregnant Diabetics.
