15Chapter 3CarersAs a carer of someone with a learning disability you may need some supportyourself, so that you can carry on supporting them. This support might be a shortbreak from caring, financial help, or a chance to do something for you.A carer’s h<strong>and</strong>book is also available, providing more information on carers’ rights<strong>and</strong> the services for carers. A carersassessment is simply an opportunity for you totell health <strong>and</strong> social care services about whatwould make caring easier for you.16CarersYou have the right to have a carers assessment,even if the person you care for has decided not to have a community careassessment. Carers also have the right to choose not to have a carers assessment.<strong>Kirklees</strong> Carers GatewayThe Carers Gateway is there to provide practical <strong>and</strong> emotional support to unpaidcarers in <strong>Kirklees</strong>.They offer training on moving <strong>and</strong> h<strong>and</strong>ling, looking after yourself, support <strong>and</strong>guidance for carers who are interested in work, training, voluntary work orpersonal development (the ACE project). Carers can use the resource centre at theCarer’s Gateway, including Internet access, useful books <strong>and</strong> literature <strong>and</strong> audioinformation.The Carers Gateway can also help carers underst<strong>and</strong> the benefits <strong>and</strong> allowancesavailable to them, including the Carer’s Allowance.They also run a Carers Emergency Card scheme. This is an alert card that tellspeople you are a carer, should you become ill or have an accident. This meansthe person you care for may go to staysomewhere else for a short time, or a supportercomes to your own house to care for them.Following their care assessment, the person youcare for will have been given a short breakallowance. You <strong>and</strong> the person you care for havea choice of the services you can buy using thisallowance to provide a short break.This could be during the day, where the personyou care for goes to day opportunities. 21Transition (moving from child to adult)For information about other services for young adults with a disability pleasecontact Gateway to Care.Moving from children's health services to adult health servicesIf you go to a special school, your health needs have probably been looked afterby a paediatrician (a specialist children's doctor), nurses <strong>and</strong> therapists attachedto the school. But at some time between the age of 16 <strong>and</strong> 19 your medical carewill transfer to adult health services. In adult services your health needs will bemet through services like your GP <strong>and</strong> community teams.<strong>Careers</strong> guidance at the Connexions centrePersonal Advisers are available to provide information, advice, guidance <strong>and</strong>support to help you into employment, education <strong>and</strong> training.See p. 8 (Chapter 1 - Getting support) for more information.AmbitionsAmbitions is for young people with learning disabilities. Look confident.5 Have emergency money with you. Keep it in a separate placeto your other money so if you lose your bag or wallet you stillhave money. This is in case your bus does not turn up orto use in a phone box.6 Carry a written note with your information on -your name, address, doctor's information <strong>and</strong>any medication you are taking <strong>and</strong> a contactperson. It can be difficult to remember thingsin an emergency, if anything happens.7 If you feel worried or in danger, tell someone.You can go into a shop or pub, or go to the policestation. Shout or make a lot of noise to let peopleknow you are worried.8 At home, always close windows <strong>and</strong> lock your doorwhen you leave.9 Only let people into your house who you know, or whoyou are expecting to visit you. Ask to see the person'sbadge. If you are still not sure, say 'no' to them.10 Think road safety. Cross at pelican <strong>and</strong> zebracrossings. Do not wear personal stereos whencrossing the road because you cannot hear the traffic.4 Keep your things safe - keep your money <strong>and</strong> mobilephone in a bag where no-one can see them.
