Welcome to <strong>Homewood</strong>Mission<strong>Homewood</strong> at Martinsburg is one of several retirement communities owned and operated by<strong>Homewood</strong> <strong>Retirement</strong> <strong>Center</strong>s, a private, not-for-profit corporation with over 80 years of strongoperating performance. <strong>Homewood</strong>’s mission is to establish and operate retirement centers and toprovide compassionate, professional, and caring service in order to enhance the quality of life forresidents, clients and co-workers. <strong>Homewood</strong> is affiliated with the United Church of Christ.<strong>Healthcare</strong> <strong>Center</strong> Services and Accommodations<strong>Homewood</strong> at Martinsburg provides skilled nursing rehabilitation services, long-term nursing care,and Alzheimer’s Dementia Care (for both Personal Care and Nursing Care levels) specifically gearedto the aging population. Age, race, creed, color, national origin, sex, or handicap will not beconsidered when determining eligibility for admission. Applicants are admitted to residency in thearea where their needs can be met most effectively. However, staff reserves the right to transferresidents to other room locations if their needs indicate it is in the best interest of the resident aswell as <strong>Homewood</strong>. The accommodations are primarily single rooms with a few double roomsavailable.Licensure<strong>Homewood</strong> at Martinsburg is licensed by the Pennsylvania Department of Health, Division ofLong Term Care as a Nursing Care Facility. <strong>Homewood</strong> at Martinsburg is also licensed by theDepartment of Public Welfare to provide Personal Care Services and is a charter member of theAmerican Association of Homes for the Aging. <strong>Homewood</strong> at Martinsburg is also a member ofthe Pennsylvania Association of Non-Profit Homes for the Aging.


Levels of Care at<strong>Homewood</strong>’s <strong>Healthcare</strong> <strong>Center</strong>REHABILITATION PROGRAMOften times following a major surgery or medicalemergency, returning home can be quite challenging.You may be faced with decreased mobility, unexpectedmedical needs, and inability to complete daily tasks suchas cooking and laundry.To help you make the transition from hospital to homeless challenging, the Tenley Rehabilitation Unit, locatedon the campus of <strong>Homewood</strong> at Martinsburg, is a specialarea designated exclusively for short-term rehabilitation.Whether you have had a joint replacement, stroke, injury,etc., our rehabilitation unit offers rehabilitative care thatwill get you back home!Rehabilitation TeamAt <strong>Homewood</strong> at Martinsburg, resident care comesfirst. Our physical, occupational, and speech therapistsstrive to deliver some of the finest resident care andclinical expertise in the industry.Our experienced team of therapists from FunctionalPathways TM designs a customized rehabilitationprogram for each individual. The commitment andattention of our staff to each person’s success helpsmake each rehabilitation program a positive experienceas residents regain the skills and abilities they need toachieve the independence they desire. OurRehabilitation Department consists of a caring andcreative team of physical, occupational, and speechtherapists, physical therapist assistants, occupationaltherapist assistants, rehabilitation technicians, as well asa per diem therapist who is available most weekends.


Physical TherapyOur Physical Therapy program improves the quality of lifeby promoting resident independence. Each therapyprogram is customized according to the residents’ needsand may include the following:• Evaluation and treatment for deficits in functionalmobility (transfers, gait), wheelchair mobility,coordination deficits, and deficits with sitting, walking,and standing balance• Upper and lower body strengthening, range of motion,and endurance training• Treatments for seating, standing, and positioning• Establishing restorative nursing or wellness programsfor carry-over therapy goals• Caregiver/Resident training and family education• Home exercise programs• Recommendation for necessary equipment to beindependentOccupational TherapyOur Occupational Therapy program focuses on increasing functional ability with daily activities such asdressing, bathing, and eating. Occupational therapists encourage our residents to perform their ownself-care and home management tasks with customized dressing, bathing, eating, and energy conservationtechniques. Our program includes:• Evaluation and treatment for deficits in Activities of Daily Living (ADLs)• Upper extremity/hand contracture management and splinting• Education in energy conservationtechniques• Treatments for sitting and positioning,standing and wheelchair mobility• Implementing use of adaptiveequipment and instructs in useof equipment• Restorative nursing programs forcarry-over therapy goals• Caregiver/Resident Training• Home Management Training• Self-Feeding Programs• Home Assessment


Speech TherapyOur Speech Language Pathologist provides ourresidents with treatment to increase abilities withswallowing, communication, and cognition. Withtherapists’ intervention, residents’ swallowingneeds are met through diet modification andfeeding techniques, while cognitive/languageneeds are met through compensatory strategiesfor increased safety and independence. Programhighlights include:• Dysphagia evaluation and treatment forindividuals with swallowing difficulties• Working closely with our Nursing and DietaryDepartments to prevent weight loss• Diet modifications• Speech/Language assessment and treatment• Addressing cognitive deficits and providingstrategies for improvementAccommodations of the Tenley Rehabilitation UnitThe Tenley Rehabilitation Unit was named in honor of Glen and Betty Tenley in recognition of theirgenerous support of <strong>Homewood</strong>. As you receive therapy on your quest to return back home,<strong>Homewood</strong> wants to make your stay here as comfortable and relaxing as possible with all the amenitiesof home. The Rehab Clinic consists of weights, exercisetables, parallel bars, steps, and Nu-step machines to increase your strength, flexibility, balance, and gait.


Accommodations of the Tenley Rehabilitation Unit (continued)Opportunity for more advanced equipment use is found in the Wellness <strong>Center</strong>’s exercise room, whichincludes multiple-position bikes, treadmills, HUR pneumatic resistive equipment, and ellipticalmachines. <strong>Homewood</strong> also provides the advantages of aquatics with a multi-depth pool ideal forweight-bearing limitations and more progressive resistive exercise. But if your day requires moresolitude, we can tailor your treatment day to bedside or another quiet area of <strong>Homewood</strong>. We hopethat you find our flexible staff dedicated to exceeding your expectations while accommodating yourdesire to return home.NURSING CARECaring, well-trained professionals have been the foundation of <strong>Homewood</strong> at Martinsburg’s <strong>Healthcare</strong><strong>Center</strong> since 1974. Our skilled and intermediate nursing is Medicare-certified and has consistentlyreceived a 5-Star Overall Rating from the <strong>Center</strong>s for Medicare and Medicaid Services (CMS). The<strong>Healthcare</strong> <strong>Center</strong> provides 24-hour nursing care on a long-term basis for those residents who needcontinued care. Our nursing staff provides assistance with any resident needs, whether it is bathing,dressing, grooming, medication management, or taking care of feeding tubes, IVs, etc. Individualizedtreatment plans can include physical, occupational or speech therapy, which are available on site. Weoffer therapeutic recreation and feature a state-of-the-art Alzheimer’s and Special Care Unit offeringboth personal care and nursing care services. ALZHEIMER’S DISEASE AND SPECIAL CARENursing Care: A specialized unit for the treatment of individuals with Alzheimer’s disease andrelated dementias is available at <strong>Homewood</strong> at Martinsburg. It includes a structured environment andprogramming, with specially-trained staff to meet the social, physical, and emotional needs of thoseafflicted with Alzheimer’s Disease or a related dementia.Our Special Care Unit also provides resident access tothe Jean Glass Lingenfelter Sensory Lounge. Thisstate-of-the-art room offers stimulation for all five sensesto assist in reducing behaviors and stimulating moods.This sensory lounge is used for our residents withAlzheimer’s Dementia, depression, and anxiety byexposing them to a soothing, stimulating environment.It ENCHANTS, ENABLES, ENRICHES, and ENGAGESour residents.We invite you to come and see the Jean Glass LingenfelterSensory Lounge as well as our Special Care Dementia Unittoday—where we offer both skilled nursing care andpersonal care services. Allow <strong>Homewood</strong> to becomethe caregiver for your loved one so you can become thespouse, son, daughter, or grandchild again.Personal Care: A specialized unit for those individuals with early stages of dementia who are doingwell physically, but who mentally are not safe to be at home alone any longer. This level of care can bedifficult to find in our area, making the care offered at <strong>Homewood</strong> at Martinsburg unique in centralPennsylvania. Our specially-trained staff provides unique programming, meals, and schedules for thosewith memory and cognitive impairment.The Alzheimer’s and Related Dementia (ARD) Unit offers the following:Safety and Security: The unit is secured and is onlyaccessible by punching a key-code to enter or leave the unit.There are two fully-enclosed outdoor courtyards for residentsto enjoy during pleasant weather. Hallways are kept clearand free of clutter so that residents are able to wander safely.Structured Daily Routine: Keeping the daily routinestable is a vital part of eliminating mood and behavior issues.With <strong>Homewood</strong>’s structured activities and meal schedules,the day moves quickly, and residents are never bored.


Activities at <strong>Homewood</strong><strong>Homewood</strong> at Martinsburg offers a truly unique and diverse Activities Department. This program isdevoted to meeting the individual needs and interests of our residents through special social interaction,intellectual stimulation, physical well-being, spiritual support and emotional encouragement on a dailybasis. Through the process of assessment and intensive planning, a wide array of life-enriching activitiesare scheduled and facilitated monthly, daily,and hourly to ensure the highest quality of lifefor each resident.It is our goal to help residents enjoy vital,fulfilling lives with all of the comforts they haveenjoyed throughout their lifetime. Dailyprograms include music, exercise, dance,creative art, sensory stimulation, andreminiscence groups. Additional regularlyscheduled programs include pet therapy,devotional programs, musical entertainment,intergenerational groups, crafts, and bakinggroups.The activity staff plans extraordinary seasonal events, whichinclude celebrations around major holidays, summerbarbeques, safe trick or treat night, family day, and aChristmas candlelight dinner. We offer on-site beauty andbarber shop services as well. We also have a communityouting program to strive to integrate the residents into thecommunity as much as possible. Shopping trips, lunchoutings, trips to the fair, seasonal tour rides, fishing trips, andtrips to baseball games are some examples.The department provides an extensive one-to-one programfor those who cannot or choose not to participate in dailygroup activities.


<strong>Homewood</strong> also offers a state-of-the-art sensory stimulation program, the Jean Glass LingenfelterSensory Lounge. The sensory lounge is geared toward residents who are unable to participate in othergroup activities. A variety of stimuli are employed to enhance perception including visual, auditory,olfactory, gustatory, and tactile.Our wonderful team of caring volunteers graciously shares their talents and brightens the lives of theresidents each and every day. We continually welcome kind and compassionate individuals and groupsof all ages to volunteer at <strong>Homewood</strong>.Benevolent Fund and Medical Assistance<strong>Homewood</strong> maintains a Benevolent Fund to assist residents whose funds are depleted. All licensednursing beds at <strong>Homewood</strong> are approved for residents who may need Medical Assistance. The potentialneed for financial assistance will be discussed prior to admission.<strong>Homewood</strong> <strong>Retirement</strong> <strong>Center</strong> does not require individuals and/or their families to sign contractsagreeing to pay private pay rates for a specified period before converting to Medical Assistance coverage.Our facility also does not require individuals and/or their families to supplement Medical Assistancereimbursement as a condition of admission or continued residence in the healthcare facility.<strong>Homewood</strong> <strong>Retirement</strong> <strong>Center</strong> does not require contributions from residents or from persons related toresidents as a precondition of admission or continued residence in the healthcare facility.<strong>Homewood</strong> <strong>Retirement</strong> <strong>Center</strong> does not discharge a resident except on grounds that are enumerated inthe state codes which include: 1) medical reasons; 2) his/her welfare or that of other residents; or3) non-payment.


Care and Payment: Cost of Care and Payment GuidelinesThose individuals who qualify for Medical Assistance (also known as Medicaid) will be given detailedinformation upon admission or prior to depletion of their funds. Rates are set annually on the basis ofaudited costs. The basic rate includes room and board, laundry, 24-hour nursing service, social service,pastoral care, a structured activity program, and administrative costs. The daily rate also includes thefollowing nursing services: catheter care, colostomy care, decubitus care, feeding (hand, tube, special diets),and suctioning. Supplies used for these services are not included in the daily rate.Upon admission, residents are designated as to the type of payment for accounting andbilling purposes required by the Commonwealth of Pennsylvania. The sources of payment options are:Private Pay, where residents and the responsible party acknowledge that their financialresources are sufficient to pay the facility’s per diem rates for all services rendered.Private Pay residents that are eligible for Medical Assistance when all assets noted on theApplication for Admission are expended. The resident must also meet medical criteria requiredby the Commonwealth of Pennsylvania for nursing care. The resident and responsible partyagree to assist the facility with any application for Medical Assistance benefits.Medical Assistance, where the resident is approved on day of admission to be eligible for thirdparty payment of Title XIX funds. The resident and responsible party agree to ensure that allallowable co-payments or charges and fees for non-covered items and services shall be paidfrom the resident’s funds.. . . 1.2.3.4.5.6.7.8.9.10.Conditions for AdmissionThe resident will meet financial obligations for accommodations, cost of care and incidentals received,recognizing that quoted rates are subject to change.The resident will utilize assets noted on the Application for Admission—Financial Information to meethis/her needs.The resident and/or responsible party will notify <strong>Homewood</strong> ninety (90) days prior to personal resourcesbeing depleted.If the resident is enrolled in the Medical Assistance program, the resident will abide by home visitregulations required by the state.Should the resident be away for medical or social reasons, the resident will be responsible for the daily ratefor the accommodations reserved in his/her behalf.The resident and/or responsible party is required to give a minimum of three (3) day prior notice tovoluntary withdrawal from <strong>Homewood</strong>.<strong>Homewood</strong> <strong>Retirement</strong> <strong>Center</strong> strongly suggests that the resident have two individuals as Durable Power ofAttorney to handle financial obligations and medical decisions in case the resident should become unable todo so. The second person would serve as Durable Power of Attorney only if the first should become unableto do so. The resident authorizes his/her designated Durable Power of Attorney to provide periodic financialdisclosure to <strong>Homewood</strong>.The resident will continue to carry insurance on personal valuables and release <strong>Homewood</strong> from liability forpersonal possessions unless loss or damage is caused by any willful or neglectful action of the facility.The resident is responsible financially for medical insurance, personal car insurance, life insurance,professional and civic club dues, as well as personal care items.Any loss or damage to real or personal property of <strong>Homewood</strong> caused by the negligence of theresident or his/her family shall be charged to and paid for by the resident.


11.12.13.14.Funeral arrangements are the sole responsibility of theresident, resident’s family or responsible agent.<strong>Homewood</strong> offers iron-on name tag labels for all clothing itemsfor $10.00. This service is made possible by our HousekeepingDepartment.The resident will consent to release medical information when arequest is made, through the Medical Director of <strong>Homewood</strong>, toany physician, hospital, or other medical provider who isproviding, or who has provided, any medical services orconsultation to the resident.The resident acknowledges that <strong>Homewood</strong>’s failure at any timeto implement medical orders applicable to the resident couldexpose <strong>Homewood</strong> to liability. In the event, therefore, that theresident should request that <strong>Homewood</strong> not implement anymedical order, the resident agrees to execute waivers of liabilityas may be reasonably requested by <strong>Homewood</strong>.15.If the resident suffers mental or physical deterioration, contracts a communicable disease or threatensviolence or causes injury to themselves or another person, to the degree that he/she can no longer receiveappropriate care at <strong>Homewood</strong>, the resident agrees to withdraw from <strong>Homewood</strong> and be placed in a moreappropriate facility.16.The resident agrees to be transferred to alternateaccommodations within the facility as care needs increaseor requirements of the facility dictate.17. The resident agrees to maintain personal behavior that willnot violate the tranquility of the facility, or infringe upon therights and freedoms of other residents, and to have visitorsand family members abide by these same guidelines.18.The resident, visitors, and family will abide by the rules andregulations as outlined in the Residents’ Handbook,established by the Board of Trustees, including thoseimposed upon long-term care facilities by the state andfederal governmental regulatory agencies.
