Vessel Name2. Call Sign / IMO <strong>No</strong>.3. <strong>Port</strong> of Registry.4. Year of Built5. LOA and Maximum Beam.6. Distance Bow <strong>to</strong> M/Fold & M/Fold <strong>to</strong> Stern7. Maximum height of manifold above water line .8. Summer and Tropical Draft.9. Arrival and Departure Draft.10. Last <strong>Port</strong> of Call.11. Next <strong>Port</strong> of Call12. Type of Tanker / SBT or CBT.13. Number / Size and Distance between Manifolds.14. Maximum Load and Discharge Rate per Line /Hours.15. Possibility of Loading or Discharging MultiGrades, If so, How many?16. Last Cargo17. Vessel condition (Ballast or Loaded)18. If loaded, type and quantity of cargo onboard19. Type and quantity of nominated cargo <strong>to</strong> be loaded/ discharged.20 Is vessel inerted / confirm inert gas plan<strong>to</strong>perational.21 Number and type of mooring ropes or wiresforward, aft and their SWL's22 Marsec Level of ship23 If Level more than 1, reason for higher level24 Name and Rank of ship security officer (SSO)25 Master's Name
