PART ISAFETY REQUIREMENTSResponsibilities of Master of ship Moored <strong>at</strong> SPM Terminal• Responsibility for the safe conduct of oper<strong>at</strong>ions onboard vessel whilstmoored <strong>at</strong> SPM terminal berths rests with ship master. Nevertheless,since our people, assets and other shipping may suffer serious damagein the event of accident aboard your vessel, we wish, before oper<strong>at</strong>ionsstart, to seek your full cooper<strong>at</strong>ion and understanding on the SafetyRequirements set out in the Ship/shore Safety Check List.• Safety Requirements have been drawn up in conjunction with otherTerminal Oper<strong>at</strong>ors in the Middle East and a represent<strong>at</strong>ive section ofthe Intern<strong>at</strong>ional Safety Guide for Oil Tankers and Terminals(ISGOTT). They are based on the provision of the Codes of SafePractice currently accepted by the Oil & Tanker industries.• We therefore expect ship’s Master and all under his command toadhere strictly to these throughout ship stay <strong>at</strong> the terminal. We, forour part, will ensure th<strong>at</strong> our and terminal personnel do the same andcooper<strong>at</strong>e fully with you in the mutual interest of safe and efficientoper<strong>at</strong>ions.• In order to assure ourselves of ship’s Master compliance with theseSafety requirements, we and SPM’s terminal shall from time to timeinstruct of our staff to visit vessel and other reporting to ships Masteror his deputy, join one of ship’s officers for a routine inspection ofcargo deck and accommod<strong>at</strong>ion spaces.If Pilot/Loading Master observes any infringement/s on board of anysafety Requirements, we shall bring this immedi<strong>at</strong>ely to your <strong>at</strong>tentionfor corrective action. 3. Liability of Shipsa) The owner and Master of a ship including an Excepted Ship, ship shallbe jointly and severely liable irrespective of the cause thereof for allloss or damages to piers, jetties, buoys, or other port install<strong>at</strong>ions orport facilities, property or machinery wh<strong>at</strong>soever and for loss of life orpersonal injury to any person lawfully within the limits of the <strong>Port</strong> inconnection with the navig<strong>at</strong>ion by such ship with or without a pilotwithin the limits of the <strong>Port</strong> or its use of the <strong>Port</strong> facilities. No shipshall be permitted to leave the <strong>Port</strong> unless and until security acceptableto the <strong>Port</strong> Authority has been given for the amount of any loss ordamage so caused.b) The <strong>Port</strong> Authority shall not be liable, whether resulting from itsnegligence or from any other cause wh<strong>at</strong>soever, for :i) Any loss, damage or delay to any ship using the <strong>Port</strong> or portfacilities or to her cargo on board or to any other property ashoreor aflo<strong>at</strong>, fixed or moveable wh<strong>at</strong>soeverii)Any loss of life or personal injury to the Master or crew usingthe <strong>Port</strong> as aforesaid.c) Any ship person using any port facility shall indemnity and saveharmless all losses, claims, demands and suits for damages, includingde<strong>at</strong>h or personal injury and including court costs and <strong>at</strong>torney’s feeincident to or resulting from their oper<strong>at</strong>ions in the <strong>Port</strong> and the use of<strong>Port</strong> facilities.d) These conditions shall be constructed to the law of U.A.E, and if sorequired by the Company, the ship and her owners shall submit to thejurisdiction of U.A.E. courts.Version No. 2 / D<strong>at</strong>e : 01.04.2012 Revision No. 7/ 30.01.2013 Page 13 |6 3


4. Pilotage4.1 The Pilot, once on board, shall be deemed to be an employee andservant of the Owners, Charterers or Authorized Agent of the Owners.They shall be liable for the Pilots acts, neglect or default in the courseof his employment.4.2 The Authority shall not be liable if the services of a Pilot are notavailable nor shall any liability <strong>at</strong>tach to the Authority if the Pilot isunable, for any reason wh<strong>at</strong>soever to perform his duties on board thevessel.4.3 Pilotage both inbound and outbound is compulsory through <strong>Fujairah</strong><strong>Port</strong> Authority. 7 Boarding Arrangement.a) Safe Embark<strong>at</strong>ion and Disembark<strong>at</strong>ion of the Pilot / LMThe master of a ship navig<strong>at</strong>ing in the port or approaches shall affordsuch suitable “We<strong>at</strong>her lee” and speed reductions as dict<strong>at</strong>ed by thepractices of good seamanship in order to provide a safe embark<strong>at</strong>ion ordisembark<strong>at</strong>ion oper<strong>at</strong>ion.Pilot ladder and associ<strong>at</strong>ed equipment must comply with SOLASstandards and the requirements of Intern<strong>at</strong>ional Pilot's Associ<strong>at</strong>ion.b) Pilot LadderA clean and efficient pilot ladder fitted with spreaders and manropesshall be made available for the pilot to embark or disembark. At nightthe ladder need to be illumin<strong>at</strong>ed with efficient and safe light.c) Supervision of Pilot LaddersThe rigging of pilot ladders for the embark<strong>at</strong>ion/disembark<strong>at</strong>ion ofpilots shall be supervised by a responsible ship’s officer, and shall beso effected th<strong>at</strong> the ladder is well clear of any overboard discharge andth<strong>at</strong> each step of the ladder rests firmly against the side of the ship.Pilot boarding / disembarking shall also be supervised by a responsibleship’s officer.d) Accommod<strong>at</strong>ion and Pilot Ladder (combin<strong>at</strong>ion)Whenever the height of the deck above the w<strong>at</strong>er level exceeds 30 ft(9.0 metres), the accommod<strong>at</strong>ion ladder shall also be used inconjunction with the pilot ladder and shall be lowered to above 22 ft(7.0 metres) above the w<strong>at</strong>er level with the pilot ladder immedi<strong>at</strong>elyadjacent to the bottom of the accommod<strong>at</strong>ion ladder.e) Mechanical HoistsA mechanical hoist of an approved p<strong>at</strong>tern shall be considered anacceptable altern<strong>at</strong>ive to a conventional pilot ladder to be used inconjunction with the accommod<strong>at</strong>ion ladder.NOTE:Mechanical hoists whose oper<strong>at</strong>ion relies upon a single wire are notof approved p<strong>at</strong>tern under these rules.Version No. 2 / D<strong>at</strong>e : 01.04.2012 Revision No. 7/ 30.01.2013 Page 15 |6 3

Terminals do not have ballast w<strong>at</strong>er receiving facilityand therefore, vessels coming from other regions to <strong>Fujairah</strong> OilTerminals should follow Ballast W<strong>at</strong>er Management Plan andexchange of ballast w<strong>at</strong>er <strong>at</strong> sea.In general, there are no restrictions on ballasting and de-ballasting forSegreg<strong>at</strong>ed Ballast Tanks (SBT) during cargo loading or dischargeoper<strong>at</strong>ions. However, the following consider<strong>at</strong>ions should be takeninto account:• During deballasting to avoid pollution due to contamin<strong>at</strong>edsegreg<strong>at</strong>ed ballast, the surface of the ballast should be sighted,prior to commencing deballasting. When segreg<strong>at</strong>ed ballast isbeing discharged, it is prudent to monitor the ballast dischargedoverboard by means of ballast w<strong>at</strong>er monitor or visual w<strong>at</strong>chshould be established. This may give the earliest warning of anyinter-tank leakage between cargo and ballast tanks th<strong>at</strong> may havebeen undetected. Clean ballast to mean oil in w<strong>at</strong>ercontent to be not more than 5 parts per million (ppm). Noballast is to be discharged unless agreed with L.M / Pilot.The Master is to ensure th<strong>at</strong> his vessel is adequ<strong>at</strong>ely trimmedmaximum (3 meters by the stern) with an adequ<strong>at</strong>e draft toensure safe berthing.There could be instances such as prevailing we<strong>at</strong>her conditions<strong>at</strong> the time, where deballasting cannot commence until loadinghas started. However, it is important to liaise with the L.M priorto carrying out such an oper<strong>at</strong>ion.PoF follows a clean sea policy, and the Pilot / L.M is authorizedto inspect your Oil Record Book which should be up to d<strong>at</strong>e andready for inspection. Ballast w<strong>at</strong>er samples are frequently takenand analyzed. 2.2 Hose Connection/DisconnectionA minimum of 15 t S W L crane is required for cargo hose handling.ADCO <strong>Fujairah</strong> Terminal personnel will connect and disconnect cargohoses to the tanker’s manifold under the supervision of LoadingMaster. Tanker is to provide crane driver and general assistance asdirected by the Pilot/Loading Master. The tanker’s officer needs to bepresent <strong>at</strong> the manifold during hose connection and disconnection..2.3 Flash PointThe flash point of Murban crude is below 32 degrees Fahrenheit.2.4 Crude Oil Temper<strong>at</strong>uresEvery effort is made to keep the loading temper<strong>at</strong>ure of export crudebelow 150 F but there could be times when the temper<strong>at</strong>ure exceedsthis. Masters are however advised when the temper<strong>at</strong>ure is likely toincrease. These increases are normally due to changes in theproduction, loading r<strong>at</strong>e, tank levels etc. The temper<strong>at</strong>ure for ship’scalcul<strong>at</strong>ions.2.5 H2S ContentMurban crude contains concentr<strong>at</strong>ion of H2S (Hydrogen Sulphide).H2S has disagreeable odour characteristics of rotten eggs whichquickly deaden the sense of smell; dangerous concentr<strong>at</strong>ion may bepresent which cannot be detected. Emergency ProcedurePART IIIEMERGENCIESIn the event of any emergency situ<strong>at</strong>ion arising onboard <strong>tankers</strong> <strong>at</strong> <strong>Fujairah</strong>Terminal, the Pilot/ Loading Master shall be responsible for ensuring coordin<strong>at</strong>ionbetween <strong>Port</strong> C.T and the Shore Terminal and tanker involved in the emergency andshall take appropri<strong>at</strong>e actions.1. SPM Terminals Incidents th<strong>at</strong> may be declared.- Fire or explosion <strong>at</strong> the Terminal and on or around a moored tanker.- Major escape of flammable and/ or toxic vapours, gases, oil orchemicals.- Collision (Tanker to tanker, tanker and SPM buoy.- Drifting and breakaway from SPM Buoy, dragging anchor orgrounding.- Major terminal accidents involving <strong>tankers</strong>, tugs, mooring bo<strong>at</strong>s, crewbo<strong>at</strong>s etc.- Meteorological hazards such as adverse we<strong>at</strong>her conditions and heavyelectrical storms.- Attack, sabotage and thre<strong>at</strong>s against <strong>tankers</strong> or terminal.2. Actions to be taken shall be determined by the type and severity of theincidents, namely.- Stop loading- Disconnect loading hoses- Standby to unmoor- Comply with emergency reporting procedures- Fire fighting facilities standby- Depart from the SPM buoy.-3. 4. Calm We<strong>at</strong>her, counter current/tide and windDead calm conditions cause special difficulties with SPM berths as there isno tendency for the moored tanker and the flo<strong>at</strong>ing hoses to stream in anyparticular direction. Special danger exists th<strong>at</strong> the moored tanker will tend tomove up and override the SPM buoy under the influence of slight we<strong>at</strong>herchanges, probably due to reversal of tidal flow.This danger may be avoided by having a tug permanently moored/fast <strong>at</strong> thetanker stern and towing <strong>at</strong> minimum power; thus maintaining steady tensionon the mooring hawser and also maintaining safe distance off the buoy.The Pilot is responsible to ensure actions are taken to avoid contact with theSPM buoy.5. Emergency Response.• Pilot/Loading Master will immedi<strong>at</strong>ely advise the <strong>Port</strong> Control Towerand Shore Terminal Control Room (CR)• The Shore terminal CR personnel shall initi<strong>at</strong>e the emergencyshutdown procedure.• The tanker master shall be requested to depart from SPM buoy underPilot advice.• The <strong>at</strong>tending tug <strong>at</strong> tanker’s astern would be used to assist th<strong>at</strong> tankeroff the berth.• The tanker shall keep clear of the flo<strong>at</strong>ing hoses.• The Pilot shall direct the Marine support craft in <strong>at</strong>tendance and ensureflo<strong>at</strong>ing hose strings are kept clear of the tanker.• Communic<strong>at</strong>ion will be by U.H.F Radio.6. Communic<strong>at</strong>ions FailureIt is essential th<strong>at</strong> communic<strong>at</strong>ions are maintained between the tankerand SPM terminal CR. Communic<strong>at</strong>ions network has been designedfor high reliability.VHF channel 10 shall be used In case of major breakdown ofcommunic<strong>at</strong>ions of UHF . Sabotage and Thre<strong>at</strong>s against Tanker / Terminal.All time ship’s gangway to be up, lowering only for official oper<strong>at</strong>ion.If necessary, stop cargo oper<strong>at</strong>ions and close manifold valves afterdepressurizing the lines.If imminent, raise the tanker alarm.Inform Pilot/Loading Master who will inform <strong>Port</strong> Control Tower andShore Terminal.Place Main Engines on standby and crew to standby to disconnecthoses.Implement ‘Tanker and Terminal Security Plan’ <strong>at</strong> Highest SecurityLevelAfter lowering hoses, Pilot/ Loading Master will coordin<strong>at</strong>e theunmooring oper<strong>at</strong>ion and unmoor from the SPM.11. c) When tanker commences her final approach and on request of Pilot,hoses could tow away, clear of tanker approach and 45 degrees fromoriginal stream direction.d) Approach course and speed is determined by the Pilot with dueconsider<strong>at</strong>ion given to all relevant factors (wind speed, sea swell,current, tanker draft, visibility, windage area and engine horse power).e) The approach is made on a course of approxim<strong>at</strong>ely 30 degreesbetween the ship’s head and th<strong>at</strong> of the freely streaming hoses.f) The tanker will proceed in a minimum speed constant with maintainingthe required course of approach. Use of ship’s radar may be consideredby the Pilot to provide inform<strong>at</strong>ion with regard to the aspect of thehose string and distance of the buoy from the bridge.g) The final approach should be with the SPM fine of the port bow of thetanker, keeping the SPM on the port bow will minimize the risk of overrunning the SPM.h) 600 meters from the SPM, L/M to continue reporting the distance fromthe mooring bo<strong>at</strong>.i) When the tanker is approxim<strong>at</strong>ely 150 meters from the mooring bo<strong>at</strong>,ship’s speed not more than one knot. On Pilot’s request mooring tugbo<strong>at</strong> will stretch the mooring pickup rope, and once the mooring bo<strong>at</strong>alongside and made fast the pickup rope to the tanker messenger linethey must be brought in board without delay. o) Once the chafing chain in the chain stopper, tanker head way is to bereduced to zero speed over ground with SPM mooring hawser justclear from the w<strong>at</strong>er then the tanker is then considered all fast.p) Mooring assembly should not be allowed to come under tension duringmooring oper<strong>at</strong>ion and after all fast.q) Control tower to be advised with all fast time.r) Mooring Tug Bo<strong>at</strong> become free and to proceed to hoses to hold onehose.s) When the Pilot s<strong>at</strong>isfies from the tanker position and there is ship’sseaman with good communic<strong>at</strong>ion <strong>at</strong> the forecastle to report SPMposition to the bridge, Pilot can request from the L/M to proceed to themanifold for hoses connections.t) Assistant tug to make fast astern of the tanker with tug’s line and pullin minimum speed or as required from the Pilot.u) Connecting hoses will be under supervision of SPM Terminal’s LMand his responsibility.v) Pilot to request from the closer working tug to bring the first hose andafter connecting to the crane hook, tug can be released.w) On connection of the first hose and once the lifting wire of the secondhose is connected to ship’s hook, second mooring bo<strong>at</strong> can then bereleased as per Pilot’s orderx) Pilot will request from ship’s Master to have 24 hours responsibleofficer duty in the bridge.y) L/M of SPM Terminal on board the tanker will observe the tanker crewwho will run loading oper<strong>at</strong>ion.z) L/M will be responsible for loading oper<strong>at</strong>ion and will observe safetyon board the tanker on regular basis..4. c) On completion of cargo hoses disconnection under supervision of SPMTerminal L/M, hoses to be lowered down to the mooring bo<strong>at</strong> one byone and the working tug bo<strong>at</strong> to tow the hoses away from the tanker,undue stress should not be applied on the hoses.d) All shore equipment should then be loaded onto the tool box andlowered to the tug bo<strong>at</strong> and non essential personnel should disembark.e) L/M to proceed to the forecastle and report to the Pilot th<strong>at</strong> the ship isready for unmooring.f) Pilot to release the escort tug and be sure th<strong>at</strong> ship’s propeller is clearbefore using the engine.f) Pilot to instruct L/M to commence unmooring.g) Under the observ<strong>at</strong>ion of L/M and Chief <strong>Of</strong>ficer and before commenceunmooring L/M to be sure th<strong>at</strong> there is no tension on the hawser andmooring assembly will not lowered over the cargo hoses.h) The pickup rope should be heaved on the winch and the chafing chainto be letting go from the bow stopper, pickup rope to be slowlylowered down until chafing chain in the w<strong>at</strong>er.i) While lowering the mooring/chafe chain and pickup rope, the tankercan manoeuvre slowly away from the SPMj) L/M to keep informing Pilot about unmooring situ<strong>at</strong>ion and distance,direction from the SPM and one of working bo<strong>at</strong> will <strong>at</strong>tend to take thepickup rope after lowered in to the w<strong>at</strong>er.k) After clearing the tanker from mooring hawser/pickup rope, L/M willreport to the Pilot so th<strong>at</strong> he can proceed manoeuvring the tanker awayfrom the SPM.l) Pilot must use assistant tug for unberthing to swing the tanker towardsthe channel in the SPM manoeuvring area.m) After the tanker well clear from the SPM, L/M to disembark.n) Pilot to proceed with the tanker to out bound channel and disembark inthe pilot st<strong>at</strong>ion, periodically for loaded tanker to turn to enter thechannel and ballast tanker (inbound) for berthing to keep the area clearbetween buoys no. 7 & 8 of SPM channel for loaded tanker.o) Pilot to inform control tower disembarking time.p) Pilot bo<strong>at</strong> with the Pilot to proceed back to the harbour or as controltower instruction.Version No. 2 / D<strong>at</strong>e : 01.04.2012 Revision No. 7/ 30.01.2013 Page 45 |6 3

(For Identific<strong>at</strong>ion purpose).
