YMCA CAMP ICAGHOWAN<strong>2013</strong> Summer RegistrationTo RegisterRegistrations accepted now and throughout summer <strong>2013</strong>.. One camper per registration form. Please make copies foradditional campers.. Each registration form must be accompanied with a $150.00 nonrefundabledeposit. The completed registration form, along withthe deposit will secure your place at camp.. Online Registration Available!Download a registration form at: campicaghowan.orgPayment Plan. The balance of the session fee is due May 1, <strong>2013</strong> unless youhave a pre-approved payment plan established with the CustomerService Center. If the balance is not paid by May 1, <strong>2013</strong>, you mayforfeit your place at camp along with the deposit.. If you have questions about our payment plan policy or would liketo establish a payment plan, please contact the Customer ServiceCenter at the time of registration, or before May 1, <strong>2013</strong> at612-822-2267.Confirmation. Confirmation will be sent within three weeks upon receiving thecompleted registration form and deposit.. The Parent Handbook can be found at campicaghowan.org. TheParent Handbook contains emergency information, medical/healthexamination information, bus schedules, packing list, and sessioninformation.. An updated Health History form must be completed by a parentor guardian and submitted each year. A Health Exam form shouldbe completed and submitted for campers with chronic healthconditions or if it will help camp better care for your child.AccessibilityIf your child has a disability requiring an accommodation or a specialneed you would like us to be aware of, please let us know. Thisinformation enables us to better meet the needs of your child withinavailable resources. For campers requiring special accommodation fordisabilities, developmental disorders and behavioral issues, decisionsare made on a case-by-case basis. Cancellations made 2 weeks or less before your camp sessionstart date are non-refundable and non-transferable.. If cancellation is due to a camper’s illness or medical reasons, orother uncontrollable circumstance, your camp fees will be refundedminus the deposit when cancellation is accompanied by a doctor’sstatement/official statement.. Should a camper’s behavior or a discipline problem affect our workwith other campers or their enjoyment of camp, we reserve theright to dismiss him or her, without refund, for the remainder ofthe session.Personal Pricing Plan –Financial Assistance. The YMCA welcomes all who wish to participate in our programs.The YMCA annually raises funds through our Y-Partners campaignto help make that possible. For the convenience of camp families, the YMCA offers safeand reliable transportation to and from camp. The departureand return bus stop is conveniently located at the REI store inBloomington, MN, just off Lyndale Avenue South and Hwy 494.Bus transportation must be reserved and paid for at the time ofregistration.
