REQUIRED FIELD TRIP FORMM. E. RINKER, SR. SCHOOL OF BUILDING CONSTRUCTIONWAIVER OF LIABILITY AND HOLD HARMLESS AGREEMENT1. In consideration for receiving permission to enroll and to participate in Undergraduate and Graduate coursesorganized by the M. E. Rinker, Sr. School of Building Construction of the University of Florida, I hereby RELEASEAND DISCHARGE the University of Florida, any University-affiliated organization, the Board of Regents of theState of Florida, the State of Florida, and their officers, servants, agents, and employees (hereinafter referred to asRELEASEES) from any and all liability, claims, demands, actions and causes of action whatsoever arising out of orrelated to any loss, damage, or injury, including death, that may be sustained by me, or to any property belonging tome, while participating in said field trips, or while in, or upon the premises where the field trips are being conducted,while in transit to or from the premises, or in any place or places reasonably connected with such field trips, andfurther WAIVE any right I might otherwise have, and COVENANT NOT TO SUE said RELEASEES inconnection with any such liability, claim, demand, action or causes of action.2. I am fully aware there may be risks and hazards connected with participating in said field trips, and am fully awarethat there may be risks and hazards unknown to me connected with participating in said field trips, and I hereby electto voluntarily participate in such field trips, knowing that conditions may be hazardous or dangerous to me and myproperty. I VOLUNTARILY ASSUME FULL RESPONSIBILITY FOR ANY RISKS OF LOSS, PROPERTYDAMAGE OR PERSONAL INJURY, INCLUDING DEATH, that may be sustained by me, or any loss ordamage to property owned by me, as a result of my participation in said field trips.3. I further hereby AGREE TO INDEMNIFY AND SAVE AND HOLD HARMLESS the RELEASEES and eachof them, from any loss, liability, damage or costs, including court costs and attorneys= fees, they may incur due to myparticipation in said field trips.4. It is my express intent that this Release shall bind the members of my family and spouse, if I am alive, and my heirs,assigns and personal representative(s), if I am deceased, and shall be deemed as a RELEASE, WAIVER,DISCHARGE AND COVENANT NOT TO SUE the above named RELEASEES.IN SIGNING THIS RELEASE, I ACKNOWLEDGE AND REPRESENT THAT:1.
