The <strong>Case</strong> <strong>Manager</strong> acts as a patient advocate and primary contact for patients, their families, andfield health care providers to ensure inter and infra facility communication.The <strong>Podiatric</strong> <strong>Case</strong> <strong>Manager</strong> (CM) is responsible for the delivery of appropriate, timely and beneficial servicein the Podiatry Department which promotes quality and cost-effective health care outcomes working withinestablished standards for case management practice. ANMC Nursing practices theory based nursing andShared Governance.Essential Job Responsibilities/Functions To Include But Not Limited To:• Provides professional level nursing support and case management for the podiatric provider.• Handles routine referrals from the field in conjunction with the orthopedic staff to schedule diagnosticstudies and podiatric clinic appointments, assures travel/quarters arrangements, and reschedulesappointments as required.• Assures that complete, accurate and appropriate clinical information from the field service is at handprior to the patient’s arrival and that pertinent information regarding the patient’s care is forwarded tothe appropriate multidisciplinary services after the patient visit is complete.• Reviews medical chart to anticipate patient needs and to determine if patient should be seen in otherservice centers and arrange care in order to avoid duplication, conflict, and omission of services.• Requests referral letters, and necessary background on patient as required for the patient visit.• Maintains case management system in order to accurately follow-up patients for repeat diagnosticprocedures and/or podiatric clinical evaluations for both ambulatory and hospitalized patients.• Participate in multidisciplinary patient care conferences for the high risk or medically complex patient.• Facilitates interagency implementation of care for patients who may receive or require services fromother professional services such as: Physical Therapy, Diabetes, Alternate Health Resources, SocialServices, Contract Health Care, Home Health Care, etc.• Independently plans continuity of patient care and maintains high-risk diabetes foot care registry.• Assists Billing and Utilization Review in the process of identifying and certifying for pre-admission,pre-procedure and continued stay authorizations for reimbursements.• Assists with clinical work such as preparing injections or casting materials, cleaning instruments andsending them for sterilization, and maintaining efficient flow of patients by prepping examinationrooms after visits.


Qualifications:• Current registered nurse license in the State of Alaska• Graduation from an accredited school of nursing. Bachelor’s Degree in Nursing; AND one (1) year ofclinical care or nursing internship experience; OR an equivalent combination of education andexperience.• Current Basic Cardiac Life Support Certification• Experience in doing a full range of inpatient med/surg nursing care as well as proven outpatient nursingexperience. Experience with the health needs of the population served is preferred. Other workexperience may be considered.• <strong>Case</strong> management certification is preferred. Related work experience may be substituted, with therequirement of obtaining <strong>Case</strong> Management Certification from a national accrediting organizationwithin 3 years of hire.• Within the last 2 years: has attended continuing education appropriate to case management (16 hoursevery 2 years) in addition to continuing education for professional licensure (30 hours every 2 years) ispreferred.• Proven ability to apply the principles and practices of CM. Work experience and proven ability to applyknowledge of case management duties that includes three out of the following four: Utilization Review(resource utilization and cost management of the treatment course and services), Discharge Planning(discharge planning practices and ability to coordinate multidisciplinary care of the inpatient), FieldHealth Coordination, (field health referral system and the ability to coordinate travel/quarters, appts.,procedures) or Quality Assurance.Knowledge, Skills and Abilities:• Knowledge of professional nursing principles, practices procedures and case management as applied tothe podiatric patient, including nursing diagnosis, problem and outcome oriented plan of care,assessment of patient’s physical, emotional, spiritual and psychological-social-cultural needs. Current<strong>Case</strong> Management-Certification preferred but not required.• Knowledge of pharmaceuticals, their desired effects, side effects, contraindications, and complicationsof their use.• Knowledge of the scope of patient care services provided by other field facilities, contract services,service centers, and departments.• Knowledge and ability to independently determine the further need for pre-operative, post-operativeand discharge care and to provide appropriate patient education. Ability and skills to evaluate,develop, initiate, the patient’s discharge plans. This requires the Podiatry Care Coordinator to have anunderstanding of the resources available in the city and state, including primary care, after-care, andreferral services. The PCC must then be able to match this with the individual needs of the patient andfamily.• Knowledge of quality management and improvement processes to assure accordance with internal andexternal standards such as JCAHO and Contract Health Regulations and Procedures.• Knowledge of Utilization Review process and its application to patients specifically• Knowledge of:1. Commonly seen diagnosis2. Surgical procedures3. The importance of related historical data4. Estimated probable length of stay• Communicate appropriate information to third party payers as needed• Obtain pre-authorization for procedures• Obtain authorization for continued hospital stays2
