Qualifications:• Current registered nurse license in the State of Alaska• Graduation from an accredited school of nursing. Bachelor’s Degree in Nursing; AND one (1) year ofclinical care or nursing internship experience; OR an equivalent combination of education andexperience.• Current Basic Cardiac Life Support Certification• Experience in doing a full range of inpatient med/surg nursing care as well as proven outpatient nursingexperience. Experience with the health needs of the population served is preferred. Other workexperience may be considered.• <strong>Case</strong> management certification is preferred. Related work experience may be substituted, with therequirement of obtaining <strong>Case</strong> Management Certification from a national accrediting organizationwithin 3 years of hire.• Within the last 2 years: has attended continuing education appropriate to case management (16 hoursevery 2 years) in addition to continuing education for professional licensure (30 hours every 2 years) ispreferred.• Proven ability to apply the principles and practices of CM. Work experience and proven ability to applyknowledge of case management duties that includes three out of the following four: Utilization Review(resource utilization and cost management of the treatment course and services), Discharge Planning(discharge planning practices and ability to coordinate multidisciplinary care of the inpatient), FieldHealth Coordination, (field health referral system and the ability to coordinate travel/quarters, appts.,procedures) or Quality Assurance.Knowledge, Skills and Abilities:• Knowledge of professional nursing principles, practices procedures and case management as applied tothe podiatric patient, including nursing diagnosis, problem and outcome oriented plan of care,assessment of patient’s physical, emotional, spiritual and psychological-social-cultural needs. Current<strong>Case</strong> Management-Certification preferred but not required.• Knowledge of pharmaceuticals, their desired effects, side effects, contraindications, and complicationsof their use.• Knowledge of the scope of patient care services provided by other field facilities, contract services,service centers, and departments.• Knowledge and ability to independently determine the further need for pre-operative, post-operativeand discharge care and to provide appropriate patient education. Ability and skills to evaluate,develop, initiate, the patient’s discharge plans. This requires the Podiatry Care Coordinator to have anunderstanding of the resources available in the city and state, including primary care, after-care, andreferral services. The PCC must then be able to match this with the individual needs of the patient andfamily.• Knowledge of quality management and improvement processes to assure accordance with internal andexternal standards such as JCAHO and Contract Health Regulations and Procedures.• Knowledge of Utilization Review process and its application to patients specifically• Knowledge of:1. Commonly seen diagnosis2. Surgical procedures3. The importance of related historical data4. Estimated probable length of stay• Communicate appropriate information to third party payers as needed• Obtain pre-authorization for procedures• Obtain authorization for continued hospital stays2
