WHAT IS THE <strong>OUTREACH</strong><strong>PHARM</strong>ACY SERVICE?The Outreach Pharmacy Service is an <strong>Alfred</strong>Health program based in the PharmacyDepartment at the <strong>Alfred</strong> <strong>Hospital</strong>.A Pharmacist from the hospital is availableto visit you in the comfort of your own hometo discuss your medications.They will assist you to obtain the best possibleresults from your medications and help to avoidunnecessary side effects.This service is provided free of charge.Interpreters are available.The Outreach Pharmacist can:HOW CAN <strong>OUTREACH</strong><strong>PHARM</strong>ACY HELP?Make recommendations about thebest way to organise your medicationsReview the appropriateness of yourmedications for you and your conditionsExplain why and when to take yourmedicationsShow you how to take and store yourmedications correctlyCheck that your medications (includingnon prescription medications and herbalproducts) are appropriate to take togetherAn Outreach Pharmacist will telephone youto arrange a suitable time for the home visit.During the visit you will be asked toWHAT WILL HAPPEN TO ME?Display all your medicines includingnon prescription medicationsand herbal products.Show the Pharmacist howyou use these medicationsYou will be given the opportunity to askany questions about your medications.Check you are using equipment withthe correct technique eg. inhalersWHAT ABOUT MY GP?The findings and recommendations from thisvisit are sent to both your GP and CommunityPharmacy. Other health care professionals willbe contacted to pass on relevant informationas required with client consent.
