To enroll for this service, the student must pay a<strong>Health</strong> <strong>Service</strong> Fee <strong>of</strong> $466.00 per year which entitles the student to services available at the<strong>University</strong> <strong>Health</strong> <strong>Service</strong>. The <strong>Health</strong> <strong>Service</strong> Clinic is located on the second floor <strong>of</strong> the Centerfor Student Development. All care and services are provided on a confidential basis.Hours <strong>of</strong> <strong>Service</strong> Monday thru Friday: 8:30am to 5:00pmThe <strong>University</strong> <strong>Health</strong> <strong>Service</strong> follows the undergraduate academic calendar. It is closed whenclasses are not in session and on all <strong>University</strong> holidays. There is limited service during the intersession.Staff <strong>Health</strong> <strong>Service</strong> is staffed by full time nurse practitioners, registered nurses, part timephysicians, a medical assistant and a medical secretary.Clinical <strong>Service</strong>s Evaluation <strong>of</strong> Illness or Injury (no charge) Well Visits such as Sports Physicals or GYN Exams/Contraception ** Limited Vaccines (Flu, Menomune, PPD, MMR, HepB, Td, HPV) ** Laboratory and Pharmacy <strong>Service</strong>s Available ** Referrals as Needed to Community Medical Resources **** Additional fees may applyClients are seen by appointment. Ext. 3156 (254-3156)Routine Gynecological Care / Contraception:GYN care is provided by a Nurse Practitioner who is licensed to provide advanced level care. Acomplete GYN exam is $60.00 Pap smear is $46.00. Plan B is available over the counter for$25.00.Laboratory <strong>Service</strong>s:Specimens obtained at our clinic are sent to East Side Clinical Lab. Clients are responsible forthe cost <strong>of</strong> the lab test. These tests may be covered by your private health insurance or theStudent Injury and Sickness Insurance if you have subscribed.Pharmacy <strong>Service</strong>s:Clients given prescriptions may take it to the pharmacy <strong>of</strong> their choice, or, if it is moreconvenient, have it delivered to <strong>Health</strong> <strong>Service</strong> by our contracted pharmacy. Prescriptionsdelivered to <strong>Health</strong> <strong>Service</strong> must be paid for at the time they are picked up unless the cost iscovered by the client’s insurance. Insurance approval for medication is done at the Pharmacy.You may enroll by completing the enclosed form and returning it to the Student <strong>Service</strong>s<strong>of</strong>fice on the second floor <strong>of</strong> the <strong>School</strong> <strong>of</strong> <strong>Law</strong>.
