5. If the reimbursement request is disapproved, the Department Director shall state thereason(s) why the request for reimbursement was denied (typed only).6. If approved, the Department Director shall authorize the processing <strong>of</strong> a separate SpecialPayroll Request Form for each pre-approved <strong>and</strong> successfully completed course toreimburse the employee for the allowed 50% reimbursement as shown on the approvedPre-Approval Request Form C.a. The Special Payroll Request Form must be typed <strong>and</strong> must contain the following:Employee’s NameEmployee’s Social Security NumberEmployee’s Administration GroupGross Amount <strong>of</strong> <strong>Reimbursement</strong> RequestTitle <strong>of</strong> CourseCourse NumberInstitution AttendedGraduate, Undergraduate or Other (specify) Level <strong>of</strong> Courseb. Attached to the Special Payroll Request Form must be:The completed <strong>Reimbursement</strong> Request Form (all copies<strong>of</strong> the form); ANDThe <strong>of</strong>ficial transcript relating to the reimbursementrequest; ANDCopy <strong>of</strong> paid invoice, receipt or cancelled check showingthat the employee has paid for the course.7. The approved reimbursement shall be charged to <strong>and</strong> be payable from eachdepartment’s Education <strong>and</strong> Training Code Account.8. The completed <strong>Reimbursement</strong> Request Form, with all attachments, must be sent tothe Director <strong>of</strong> Personnel <strong>and</strong> Civil Service Commission. If the Department Directorhas approved the <strong>Reimbursement</strong> Request the Director <strong>of</strong> Personnel <strong>and</strong> CivilService Commission will review all information submitted for compliance with theprocedures <strong>and</strong> if approved will transmit it to the Payroll Manager for processing<strong>and</strong> return copies to the Department Director <strong>and</strong> employee. If the <strong>Reimbursement</strong>Request is disapproved by the Director Personnel <strong>and</strong> Civil Service Commission theforms will be returned to both the Department Director <strong>and</strong> employee with an explanation<strong>of</strong> the disapproval.- 11 -<strong>Tuition</strong> <strong>Reimbursement</strong> <strong>Program</strong> - <strong>Union</strong> <strong>and</strong> Non-<strong>Union</strong> Employees 2012.doc
