How to Request Accommodationsfor a DisabilityIn order to receive and maintain services, a student with adisability must:–Be otherwise qualified for admissions to or participation in aprogram of study with or without a reasonable accommodation.–Disclose (notify) and provide the appropriate documentation tothe Dean of Student Affairs, North Campus and the Special NeedsFacilitator, South Campus.–Appropriate documentation includes medical or psychologicalevaluations not more than 3 years old which contain intellectualand achievement scores, strengths and weaknesses related to theclassroom performance, and recommendations for academicaccommodations.
