<strong>Better</strong> <strong>Living</strong> Community Services: A New Case Management Model<strong>Better</strong> <strong>Living</strong>’s New Case ManagementModel<strong>Better</strong> <strong>Living</strong> has introduced a new Case Managementmodel to effectively promote the inclusion and integrationof disadvantaged individuals into a community setting.Case Management focuses on building relations withindividuals through the use of the empowerment orientedcrisis intervention model, advocating for individual rightsand devalued people, and building community-basedconnections and relationships. Case Managers rely oneffective listening skills, conflict resolution skills, andfocusing on a person’s strengths and attributes to enhancetheir quality of life. There are numerous steps taken inorder to accurately place a client within a service that bestsuits there needs. A Case Manager must observe a person’slifestyle and try to explore options that are not generallyconsidered in order to navigate clients to the appropriateservice provider within the Central Service Support Systemof <strong>Better</strong> <strong>Living</strong> <strong>Health</strong> and Community Services.Case Management begins once the Intake Coordinatoractivates a client in the system. An e-mail is sent out toCase Management and from there an appointment ismade to set up service. Depending on the services that areneeded, the outcome of client assessment is reviewed andthe information provided is passed on to the appropriatedepartments.
