STUDENT RESPONSIBILITIESWhen on Clinical Placement at <strong>St</strong> Vincent’s Health• <strong>St</strong>udents at all times must wear a University Identity Card whilst on hospitalgrounds.• <strong>St</strong>udents must identify themselves to patients, staff and visitors as a studentnurse.• <strong>St</strong>udents must at all times adhere to <strong>St</strong>. Vincent’s <strong>Hospital</strong> Policy andProcedure <strong>Guidelines</strong> in regard to patient care. Additionally students shouldfamiliarize themselves with the hospital student policy.• <strong>St</strong>udents must adhere to the uni<strong>for</strong>m code of their University.• Neat and professional attire must be maintained during clinical experienceincluding hair tied back and jewellery kept to a minimum. Small stud earringsor sleepers (no more than 2 in each ear) are acceptable as is a wedding ring, butno other jewellery is to be worn. Nail polish also not to be worn. A wristwatchinstead of a fob watch is acceptable.• The reputation and image of <strong>St</strong>. Vincent’s <strong>Hospital</strong> is upheld by theperceptions that patients and visitors <strong>for</strong>m of staff. For this reason students areexpected to be courteous and considerate to patients, visitors and staffmembers.• <strong>St</strong>udents must be punctual at all times. Should the student be unavoidablydelayed the clinical teacher must be notified as soon as possible.• <strong>St</strong>udent must in<strong>for</strong>m allocated buddy when leaving the clinical area <strong>for</strong> mealbreaks or at end of shift.• Patient in<strong>for</strong>mation is to be regarded as confidential at all times. <strong>St</strong>udentswill be required to read <strong>St</strong>. Vincent’s <strong>Hospital</strong> ‘Privacy and Confidentiality Policyand Procedures’ document and must have signed a ‘Confidentiality Agreement’with either the University or the <strong>St</strong>. Vincent’s Health.• <strong>St</strong>udents must ensure efficient use of materials and equipment.• All entries in patient records and medication charts must be countersignedby buddy/preceptor (Division 1 RN).• Any changes or concerns in regard to patient condition must be reported toyour buddy/preceptor or nurse in charge immediately.• <strong>St</strong>udent nurses are not permitted to take a blood sample <strong>for</strong> cross matchpurposes.• Textbooks or educational material belonging to ward or clinical areas maynot be removed. LIAISON AND RESOURCE NURSESNurse Education Coordinators / Clinical Nurse EducatorsAssist with the professional and practice development of nursing staff, bycoordinating local and health service wide education. Acts as a resource andsupport to nursing staff at all levels. There are Nurse EducationCoordinators/Clinical Nurse Educators at all campuses within <strong>St</strong>. Vincent’s andin specialty areas. Reporting to the Nurse Education Manager, contact one ofthe team via NurseEducation@svhm.org.au.Practice Development NurseResponsible <strong>for</strong> supporting the transition and clinical learning of newgraduates and staff to the general acute and sub-acute clinical environmentsacross the Fitzroy campus by providing direct clinical support and supervisionto new nurses to ensure patient care standards and best practice is promotedand maintained.Diabetes EducatorThe Department of Endocrinology and Diabetes based at STV offers acollaborative, consultant nursing and medical service to inpatients at Fitzroy,outpatients, and the community. The Diabetic Educators assist patients toaccept and integrate diabetes management successfully into their lifestyle.Inpatient referrals should be made as early as possible to maximise individualassessment, ongoing teaching, review, and efficient discharge. Drug & Alcohol Services can be contacted<strong>for</strong> assessment and in<strong>for</strong>mation regarding the management of a specificpatient, as well as <strong>for</strong> general management or education regarding drug andalcohol issues. Vincent’sHealth who act as a clinical resources, clinical advisers, conduct surveillanceand provide education sessions as required. The PCLN can be called by any memberof nursing staff <strong>for</strong> patient assessment, treatment and advice. RDNS Liaison NurseAll STV patients requiring community-nursing care should be referred to theRDNS Liaison Nurse or a SVAH Liaison Nurse as appropriate. Geographicalboundaries exist at SVHM when referring <strong>for</strong> community nursing services;please make contact with either SVAH or RDNS to ascertain the appropriateservice or refer to the Discharge Planning Departmental Site on the Intranet.All patients referred to RDNS have a comprehensive assessment while aninpatient and referrals should be made early in the admission to facilitate asmooth discharge.The RDNS Liaison Nurse attends Discharge Planning or Family Meetings whenpossible if requested and is contactable at STV between 9.00 am and 3.30 pmMonday to Friday via pager 114, ext. 3823, or fax 3848, and at SGH between9.00 am 3.00 pm Tuesday to Friday via pager 054, ext. 8225 or mobile 0410 417107 during business hours. In the absence of a RDNS Liaison Nurse, referralscan be directed to the RDNS Head Office on 9536 5222 or RDNS After HoursOffice on 9379 0577.Respiratory EducatorThe Respiratory Clinical Nurse Consultant is part of the Department ofRespiratory Team based at STV. The Respiratory Educator provides educationand consultant services <strong>for</strong> patients with asthma or chronic obstructivepulmonary disease. Advice, assistance and in<strong>for</strong>mation can be sort <strong>for</strong> anypatient who requires education to optimise management of their respiratorydisease. Education sessions are also provided <strong>for</strong> staff on respiratory diseasesand inhaler techniques. To contact Maria Loder page 984, or phone ext. 3136.<strong>St</strong>omal Therapy Clinical Nurse ConsultantThe <strong>St</strong>omal Therapy Clinical Nurse Consultant based at STV providescounselling and education <strong>for</strong> patients with an enteral or urinary stoma, andadvice on wound care, pressure ulcers and incontinence management. ContactElizabeth Davis pager 121, ext. 3487 or fax 3489.<strong>St</strong>. Vincent’s at Home (SVAH) Liaison NurseAll patients referred to SVAH are reviewed by a Liaison Nurse to assess theirsuitability. The referral must be made as early as possible so the Liaison Nursecan assess the nursing needs of the patient and assist to identify problemsprior to discharge. There are several SVAH Liaison Nurses covering all clinicalareas of the hospital. They can be contacted directly via pager, or phone ext.3817. Referrals can be made between 9.00 am and 3.30 pm, Monday to Friday.Urgent referrals out of business hours can be made to the on call SVAH LiaisonNurse via switch.Cardiac Nurse ConsultantEmployed by Cardiology and Restoring Health (the chronic diseasemanagement program), heart failure and cardiac patients with chronic cardiacproblems are referred to the Cardiac Nurse Consultant to provide education topatients & their families and linking them with appropriate communitysupports. The Cardiac Nurse Consultant can also review patients and assistnursing staff with the management of such patients. For referrals, educationor support, the Cardiac Nurse Consultant can be contacted via pager on 1174.10


ICU Liaison NurseThe role of the ICULN is to collaborate with ICU and parent multidisciplinaryteams regarding the transfer of patients to the wards, assisting nursing staffwith clinical decision making, planning, implementing and evaluating caregiven to patients recently discharged from ICU and <strong>for</strong> other patients asrequested or referring potentially at risk patients to ICU medical staff. Ifrequired the ICULN can facilitate re-admission to ICU, liaising with appropriatemedical and nursing staff. The ICU Liaison Nurse can be contacted on pager1429.RESOURCE ROLES AND SERVICES ACROSS THE HEALTH SERVICEPatient Services Clerk (PSC)Reception, clerical, admission of patients, computer updates, waiting listmanagement, outpatient appointments, histories, filing, customer service,collect medical records and X-rays.Support Services Associate (SSA)Orderly duties, assistance with patient turns, prep shaves (some), cleaning, runmessages, food service, and inpatient transport.SecurityAt <strong>St</strong> Vincent’s Health the Security Department are located on the ground floorof the Inpatient Services Building and have Security Officers on duty 24hoursper day. They provide a variety of services and can be contacted by phoning ext.4444 or 4391ALERT: Assessment, Liaison, & Early Referral TeamThe objective of the ALERT service based at STV is to provide value added careto patients ‘at risk’, and to work more closely with care providers and thecommunity. This aims to enhance discharge planning and continuity of care <strong>for</strong>patients with complex social and medical needs. Patients who are referred tothe ALERT team are those with complex discharge planning needs or thosewho are at risk of unplanned readmission. Nursing staff can make referrals toALERT by paging 204.Allied HealthIncorporates the Social Work, Physiotherapy, Speech Pathology, Podiatry,Occupational Therapy, and Nutrition Departments. Nursing staff can accessmembers of the multidisciplinary team via pager or by faxing a referral tospecific departments. Some allied health members attend Discharge PlanningMeetings.Angels at <strong>St</strong>. V’sAngels at <strong>St</strong>. V’s is a service coordinated through the Volunteer Department atthe Fitzroy campus. An ‘angel’ can be requested online using an e <strong>for</strong>m <strong>for</strong>example to sit with a patient who: may be a falls risk, confused, wandering,impulsive, or needing companionship. They are not available <strong>for</strong> aggressivepatients under section 16B of the Mental Health Act.11


Deaf & Hard of HearingSTV has established Victoria’s first Deaf and hard of hearing friendly healthservice.All units have a resource guide to assist with caring <strong>for</strong> members of the Deafand hard of hearing community whilst they are utilising our health careservices. We also have hard of hearing inpatient kits which contain assistivelistening devices <strong>for</strong> use. To access this inpatient kit or <strong>for</strong> further assistance incommunicating with Deaf and hard of hearing patients you can contact theSpeech Pathology department on ext. 3840Interpreter ServiceAt STV an interpreter can be booked by phoning ext. 3483. Except <strong>for</strong>emergencies, please give as much notice as possible. If the patient speaks alanguage not serviced by a hospital interpreter, page the Chief Interpreter on253 who will organise an interpreter from outside the hospital. Avoid using theservices of unqualified interpreters such as staff, friends or relatives of thepatients. MEDICATION ADMINISTRATIONWhen administering medications you must ensure: -• The right patient• The right medication• The right dose• The right time• The right route• The right expiry date• The patient allergy historySTUDENTS MUST BE SUPERVISED AT ALL TIMES WHILE PREPARING ANDADMINISTERING MEDICATIONS.Pre registration student and re-entry nurses and Division 2 Registered Nursesundertaking a medication course, are permitted to administer medicationunder the direct supervision of a <strong>St</strong>. Vincent’s Health Registered Nurse Division1 Grade 2 or above, Division 3 Registered Nurse, or a Clinical Teacher also aRegistered Nurse Division 1 or 3 directly responsible <strong>for</strong> that student, employedby the education institution where the student is enrolled.Exceptions to this are: Injectable drugs (except S/C heparin) & intravenous drugs Drugs of addiction (S8) Drugs of Dependence (S11) Oral anticoagulants PRN medication Nurse initiated medicationFor these administrations the student must be under the direct supervision of a<strong>St</strong>. Vincent’s Health Registered Nurse Division 1 or 3 responsible <strong>for</strong> thatpatient.• In instances where the Clinical Teacher is a Registered Nurse seconded from<strong>St</strong>. Vincent’s Health they must adhere to the policy as a Clinical Teacher.• If patient refuses medication, the student must in<strong>for</strong>m the nurse responsible<strong>for</strong> the patient as well as document on medication administration chart.• If the patient is fasting the student must speak directly to the nurseresponsible <strong>for</strong> the patient in regards to which medications should beadministered.• Non <strong>St</strong>. Vincent’s Heath Registered Nurses undertaking clinical experience aspart of post graduate studies must be supervised by a <strong>St</strong>. DOCUMENTATION GUIDELINESWhen you document in a patient medical record always be brief and factual. Donot use subjective or defamatory statements, and limit the use ofabbreviations. Every entry in the record must be made in a date sequentialmanner and every entry must be dated and signed. When you document youmust clearly print your name next to your signature and print your year leveland University.Progress notes are to be documented daily on the early shift or at any othertime where there is a significant clinical change in a patients' condition, e.g.return to ward post operatively.Under no circumstances should documentation be destroyed or deleted. Anyerrors made in patient medical records must have a single line drawn througheach line of text. The person responsible will write, "Entry made in error" andthen sign in the usual manner.Fluid Balance Charts (FBC)As a general rule of thumb, FBC are required <strong>for</strong> patients who are receiving IVtherapy; prescribed a fluid restriction or who are unwell. Each clinical area maydiffer as to how they complete the FBC’s. HANDOVER GUIDELINESThere is only 30 minutes allocated <strong>for</strong> handover, there<strong>for</strong>e it is essential thatyou are prepared and know exactly what is relevant and what is not. Your handover should take no longer than 5-6 minutes. Be brief and to the point. There isno need to hand over anything that can be obtained from reading the patientbedside charts.
