An allocated staff member will fill out an adverse events form.For all incidents involving a student a University incident report needs to becompleted in addition to a hospital incident report.STUDENT LOGONWhilst undertaking clinical placement within St. Vincent’s Hospital Melbourneyou will have access to the computer system by way of the Student Logon.
