ICU Liaison NurseThe role of the ICULN is to collaborate with ICU and parent multidisciplinaryteams regarding the transfer of patients to the wards, assisting nursing staffwith clinical decision making, planning, implementing and evaluating caregiven to patients recently discharged from ICU and <strong>for</strong> other patients asrequested or referring potentially at risk patients to ICU medical staff. Ifrequired the ICULN can facilitate re-admission to ICU, liaising with appropriatemedical and nursing staff. The ICU Liaison Nurse can be contacted on pager1429.RESOURCE ROLES AND SERVICES ACROSS THE HEALTH SERVICEPatient Services Clerk (PSC)Reception, clerical, admission of patients, computer updates, waiting listmanagement, outpatient appointments, histories, filing, customer service,collect medical records and X-rays.Support Services Associate (SSA)Orderly duties, assistance with patient turns, prep shaves (some), cleaning, runmessages, food service, and inpatient transport.SecurityAt <strong>St</strong> Vincent’s Health the Security Department are located on the ground floorof the Inpatient Services Building and have Security Officers on duty 24hoursper day. They provide a variety of services and can be contacted by phoning ext.4444 or 4391ALERT: Assessment, Liaison, & Early Referral TeamThe objective of the ALERT service based at STV is to provide value added careto patients ‘at risk’, and to work more closely with care providers and thecommunity. This aims to enhance discharge planning and continuity of care <strong>for</strong>patients with complex social and medical needs. Patients who are referred tothe ALERT team are those with complex discharge planning needs or thosewho are at risk of unplanned readmission. Nursing staff can make referrals toALERT by paging 204.Allied HealthIncorporates the Social Work, Physiotherapy, Speech Pathology, Podiatry,Occupational Therapy, and Nutrition Departments. Nursing staff can accessmembers of the multidisciplinary team via pager or by faxing a referral tospecific departments. Some allied health members attend Discharge PlanningMeetings.Angels at <strong>St</strong>. V’sAngels at <strong>St</strong>. V’s is a service coordinated through the Volunteer Department atthe Fitzroy campus. An ‘angel’ can be requested online using an e <strong>for</strong>m <strong>for</strong>example to sit with a patient who: may be a falls risk, confused, wandering,impulsive, or needing companionship. They are not available <strong>for</strong> aggressivepatients under section 16B of the Mental Health Act.11
