<strong>Form</strong> S – <strong>Student</strong> <strong>Permission</strong>To be precise, the Privacy Act definition of Personal Information is:"... information or an opinion (including information or an opinion forming part of a database), whether trueor not, and whether recorded in a material form or not, about an individual whose identity is apparent, orcan reasonably be ascertained, from the information or opinion."What Personal Information does ACARA collect in relation to the Work?The Personal Information ACARA collects from the <strong>Student</strong> in relation to the Work comprises:the <strong>Student</strong>’s name;the Parent or Guardian’s namethe School name and location;the <strong>Student</strong>’s current age;the <strong>Student</strong>’s Work;the <strong>Student</strong>’s current school Year level;whether English is an additional language/dialect for the <strong>Student</strong>;whether the <strong>Student</strong> is a student with disability; andwhether the <strong>Student</strong> identifies as an Aboriginal student or a Torres Strait Islander student.How will ACARA use and disclose the Personal Information?Any Personal Information ACARA collects in relation to the Work will only be used for the Purpose. Other thanfor the Purpose, ACARA will not disclose the <strong>Student</strong>’s Personal Information without your consent, unlessrequired by law.In order to comply with this requirement, the <strong>Student</strong>’s name will not be connected to the Work if it is publishedon the internet. In the licence agreement (Part A), the <strong>Student</strong> consents to ACARA communicating the Work onthe internet without attributing authorship (clause 2 of Part A).Access to the <strong>Student</strong>’s Personal InformationACARA will provide the <strong>Student</strong> with access to the <strong>Student</strong>’s Personal Information upon request and will allowthe <strong>Student</strong> to update their Personal Information if incorrect.Complaints regarding the handling of the <strong>Student</strong>’s Personal InformationIf you are concerned about the way that ACARA has handled the <strong>Student</strong>’s Personal Information, ACARA’sPrivacy Officer will investigate any concerns that you send to us in writing. ACARA’s email address is info@acara.edu.auIf you are dissatisfied with ACARA’s investigation of the concerns, you can complain to the Office of theAustralian Information Commissioner who is independent of ACARA. Under the Privacy Act,you should address your complaint to ACARA in the first instance before lodging a complaint with theInformation Commissioner.
