Day TherapyDay Therapy provides a range of services for patients with a diagnosis of an advanced,progressive, life-limiting illness who have associated complex Specialist Palliative Care needs.Due to funding arrangements, some Day Therapy services are only available in certain areas.<strong>Referral</strong> criteria for all Day Therapy servicesA referral can be made for one or more of the following reasons:• Symptom control• Psychological, spiritual and social need• Rehabilitation assessment following radiotherapy, chemotherapy, surgery and palliative careinterventions.The referral form must be fully completed. If we need to contact you for further details,there will be a delay in activating the referral.Services available within the Day Therapy Units (See contact details pg. 12)Day Care<strong>Referral</strong> criteria for <strong>Hospice</strong> Day Care<strong>Referral</strong>s are accepted for patients with a diagnosis of an advanced, progressive life-limiting illnesswho have associated complex specialist palliative care needs. Patients will need to be well enoughto attend Day Care.The Day Care Service provides:• Access to the multi - disciplinary team, co-ordinated by a keyworker.• A management plan which is discussed and agreed with the patient. The management planwill be subject to ongoing review by the multi disciplinary team.• Liaison with other health and social care professionals (hospital/community)Most patients will attend Day Care for approximately twelve weeks. Each patient will have a nurseassigned as their key worker who will undertake an initial assessment, agree a management planwith the patient and refer the patient to relevant services in order to meet the patient’s specific needs.How to refer to Day Care• <strong>Referral</strong>s can be made by General Practitioners (GP), District Nurses (DN), hospitalmedical/surgical teams or by specialist teams in the community or hospital• <strong>Referral</strong>s may also be initiated by the patient, but this will be in consultation with their GP.• A <strong>Hospice</strong> referral form should be fully completed by the assessing health care professional. Forreferrals from health care professionals other than the medical lead (GP/hospital consultant) theymust also sign to confirm that the medical lead has been informed of the referral.• The referral form may be submitted by post or fax.<strong>Referral</strong> response times• Verbal contact will be made with the referrer within 5 working days of receipt of referral8
