Dear Contractor/Supplier,<br />

To become a preferred Contractor/Supplier and remain as a preferred Contractor/Supplier<br />

<strong>McDonald</strong> <strong>Jones</strong> <strong>Homes</strong> (MDJH) you are required to supply a Housing Industry Site Safety<br />

‘Subby’ Pack (HISSP) annually. To avoid delays in processing payments and issuing<br />

purchasing / work orders to you please complete the enclosed HISSP Pack and return it<br />

to me as soon as possible via email: lmaddock@mcdonaldjones.com.au or mail to<br />

the Newcastle address below for review & acceptance into our contractor<br />

management system.<br />

Please be advised that this pack and checklists contain the minimum content requirements<br />

for a MDJH contractor/supplier.<br />

If you would prefer to receive a copy of the pack as a MS Word document please email me<br />

on the email address above.<br />

“Certificates of Currency” are attached!<br />

Please note: Accident and Sickness insurance is compulsory for Sole Traders and<br />

Partnerships as Sole Traders and the Partners of a Partnership are not covered by<br />

Workers Compensation insurance<br />

Form 2 – WHS Policy.<br />

o Please return signed and dated WHS Policy.<br />

Form 3 – Roles & Responsibilities.<br />

o Please ensure signed and dated in the relevant areas.<br />

Form 4 – Site Specific Risk Assessment (SSRA).<br />

o Example attached can be used – or you can provide your own SSRA.<br />

Please return completed example.


Form 5 – Safe Work Method Statement (SWMS).<br />

Additional mandatory aspects which should be include in your scope of works Insert<br />

these basic additional unloading processes addressing at least the following subject<br />

matter:<br />

1. BASIC TRAFFIC MANAGEMENT Only if blocking trafficable areas i.e. footpaths &<br />

roadways!<br />

o 4 Cones/ Witches Hats<br />

o Relevant Signage “Danger Unloading Truck”<br />

o Crane outriggers / Stabilizers (flags & lights)<br />

2. ENVIROMENTAL PROTECTION AUTHORITY (EPA) Applicable to ALL<br />

o Mud on Road<br />

o Cleaning requirements (broom)<br />

o Noise from the site & work outside of DA hours<br />

3. USE OF TRUCK / CRANE / HI-AB Only if applicable<br />

o Servicing / Maintenance Records (Where would these be in the truck)?<br />

o Work near overhead power lines (If closer than the 3m)?<br />

o Damage to Service / Man-way Covers & Kerb’s (Placement of Crane outriggers /<br />

Stabilizers)<br />

o Unloading procedures (Crane) &<br />

o Manual Handling (By Hand)<br />

o Working at Heights on the truck<br />

o Falling Objects from the truck or crane<br />

4. SITE SECURITY<br />

o Site to be locked and secure as per MDJH Site Safety Rules.<br />

Declaration by Contractors and Workers<br />

Ensure this is signed by all employees or it will be rejected/returned for completion


Form 6 – Toolbox example<br />

o See completed example attached above which can be used or adjusted to suit and<br />

there is also a blank template attached.<br />

o Please return completed example, signed by attending employees.<br />

Form 7 – Employee Training Register Provide details and copies of all WorkCover<br />

Induction to Construction cards. Complete register for all employees attending<br />

<strong>McDonald</strong> <strong>Jones</strong> <strong>Homes</strong> sites.<br />

o Please note this could also include number of years experience in trade/work activity<br />

e.g. 8 years carpentry.<br />

Form 8 – Plant & Equipment Register (if no plant or equipment used please put a line<br />

through the form and mark N/A i.e. not applicable and we will review).<br />

o Please note this should include all major tools used in trade/work activities e.g. Bob<br />

Cat, Cement mixer, compressor, generator, elevators, brick-saws, lasers etc.<br />

Form 9 – Electrical Equipment Register (if no electrical equipment used please put a<br />

line through the form and mark N/A i.e. not applicable and we will review).<br />

o Please note this should include all electrical tools used in trade/work activities (except<br />

battery power tools) however battery chargers must be include if used onsite.<br />

Form 10 – Hazardous Substance Register and Risk Assessment (if no hazardous<br />

substances used please put a line through the form and mark N/A i.e. not applicable<br />

and we will review).<br />

o Please also provide current MSDS if possible (i.e. no older than 5 years).<br />

Form 11 – Incident and Injury report (blank copy) please note you need a form to<br />

complete for your insurance company and MDJH in the event of an accident or injury.<br />

o Example attached which can be readily used in the event of accident.<br />

Subcontractor Statement of Declaration (attached separately).<br />

o Example attached can be used – please ensure signed and dated in the relevant<br />

areas.<br />

Please note that if your trades have not been MDJH Company inducted to visit our<br />

construction sites please contact me to make arrangement for inductions.<br />
