The Pareto Epidemic…
The Pareto principle (also known as the 80:20 rule) states that roughly
80% of the output comes from 20% of the input. In the past few weeks
we’ve seen something very similar in the surgery.
Following extensive national media coverage regarding Meningitis B, many concerned parents are focussed on getting
their children vaccinated. Meningitis B is thankfully very rare and is statistically unlikely to affect your family, although
vaccination can take much of the credit for that fact. There are however, many ‘everyday’ factors that are much more
likely to impact on our health and the wellbeing of our children, yet we do very little to address their causes.
As one of many doctors who see the damage caused by poor diet, lack of exercise and the occasional accident or
mishap almost every day, I urge you to invest a proportionate amount of your time and energy in your own
wellbeing, and that of your family.
Eat well, cook with fresh ingredients and
avoid convenience foods as much as you can.
Build regular exercise into your family life, walk to school or work, go for
family bike rides at the weekend (wearing a safety helmet of course) and
take the dog to the park more often.
Accidents will
happen
By Dr. Dalia Bialostocky
Accidental injuries are one of the leading causes of
serious harm and death in young children in the UK.
Each year two million children under the age of 15 go
to A&E having been injured at home, and about 25% of
them are under 5 years old. Many more are treated
with first aid by their parents.
Accidents can be caused by cuts, falls, burns, choking,
suffocating, drowning, poisoning and more. Many
should initially be treated at home, so as a parent, it is
important to have a basic knowledge of first aid – there
are many classes available if you need some training.
CALL 999 IMMEDIATELY IF YOUR CHILD…

stops breathing or is struggling to breathe
has a seizure for the first time
is unconscious or will not wake up
Otherwise take you child to the nearest A&E if:
They have a very high temperature and are lethargic
Your child is breathing very fast, is panting or
very wheezy
Your child has a cut that won’t stop bleeding
Your child has a limb injury and is unable to use it
Your child swallows tablets or any poison
If you are not sure what to do, you can contact
JDoc365 on 0800 043 1492 or 111 for advice.
It is important to prevent accidents by providing close
supervision, making your home safe and recognising
that accidents are almost a part of growing up.
Exploring and playing is essential in any child’s
development.
If and when an accident happens and the situation can’t
be sorted with first aid at home, it is essential to
attend an emergency department for medical
assessment. Timely care provided by a health
practitioner will minimise and
often resolve the damage
in most cases .
EMMA CITRON
OCD
Intrusive habit or
serious disorder?
By Emma Citron Consultant Clinical Psychologist
We have all heard of Obsessive Compulsive Disorder (OCD)
and may have worried at some point about whether we have
OCD tendencies ourselves.
OCD-like behaviours are especially common around
Passover, Spring cleaning time or during the recent
SARS, Ebola and Zika outbreaks.
We may find ourselves washing our hands more than
usual or recommended, or becoming more rigid in our
routines. Perhaps we derive some comfort in this by,
for example, straightening the books on the shelf, lining
up all the shoes in the rack or checking our email ‘sent
documents’ folder again and again for errors.
OCD affects about 2% of the
population, or may affect any one of
us with a 2% likelihood.
What is going on here? Is this OK, and when do our
behaviours become a cause for concern? Obsessivecompulsive
disorder is best described as when people's
preoccupations get out of hand. It may be that they
worry about contracting a disease and then ‘neutralise’
this worry by engaging in one or several compulsions
which act to neutralise it.
Many of us become more obsessive and compulsive in
our behaviours at times of stress, and the behaviours
serve as a ‘quick fix’. However, compulsive behaviours
where they escalate, actually make anxiety worse and
professional help should be sought by a registered
Clinical Psychologist.
New in-house minor
surgery service
MINOR
SURGERY
BOTOX
We will soon be opening a new JDoc365 facility at the Golders Green surgery. From there,
Dr. Ajay Bhundia will be providing minor surgery, Botox and dermal filler therapies.
DERMAL
FILLERS
Dr. Bhundia trained at Kings College Hospital and at the Plastic Surgery Department
at The Royal Free Hospital, and holds a Diploma in Dermatological Surgery
For over six years, Dr. Bhundia has been operating on
benign skin lesions, providing cryotherapy for warts and
skin tags, and treating painful joints including shoulders,
knees, fingers and wrists with special injections.
Although often described as ‘minor’ surgery, these
procedures provide major benefits and relief from longstanding
conditions. There are no significant side effects,
and apart from cosmetic and mobility benefits, they can
also make a positive impact on patients emotional
wellbeing too.
CASE STUDY
Removal of Mole
The patient was getting married in three months. She
had a conspicuous mole on her neck, which she
wanted to be removed. Dr. Bhundia duly excised it
using a suturing technique that left no significant scar.
The bride-to-be was very happy with the outcome and
looked beautiful in her wedding pictures.
CASE STUDY
Removal of Cysts
A young man of 23 years suffered with cysts in his
scalp. There was hair loss over the cysts and he was
worried about further loss. Dr. Bhundia removed the
cysts with minimal scarring, and the patient reported
full hair regrowth shortly afterwards.
CASE STUDY
Removal of Lipoma
A 27 year old professional volleyball player had a large
lipoma (fatty tissue growth) on her right shoulder
which impeded her game. Dr Bhundia excised the
lesion and the patient went on to represent her club
and country with no further hindrance.
PROFBIOTICS
THE FOOD FIGHTERS
by Tara Whyand M.Sc, B.Sc, specialist dietitian and nutrition advisor to ProfBiotics.com
Following my article on irritable bowel
syndrome in the last edition (page 7), this
time I look at food supplements which may
be of use to IBS sufferers.
As previously mentioned, the probable main cause of
altered movements is down to changes in our gut
bacteria. This can include stress, busy lifestyles, drugs
and poor dietary choices. Unless you address the
bacteria imbalances directly and work out what is
affecting them, your symptoms will never go away. If
you think about it, how you act and what you consume
creates the environment for particular strains of
bacteria to either flourish, or die off. What we want is
to have a huge variety of strains living in the bowel.
Leaving imbalances alone can lead to inflammation of
the bowel lining.
Some food supplements have been shown
to help IBS, so if reducing stress and
managing a busy lifestyle has not made a
difference, they may be worth a try.
The first method to consider is adding live bacteria into
your diet. These are called probiotic bacteria and they
help colonise the bowel to correct any imbalances that
can exist. You should try a strong multistrain dose daily
for four weeks, and if it does not help, there are some
other methods to consider raising with your doctor.
We know that vitamin D deficiency is common, but it
may be even more common in people with IBS. We
do not know why this is, but it may be a good idea to
get tested as deficiency puts you at risk of more serious
problems in the bowel and throughout the body.
well as sunlight, dietary sources are just as important,
for example oily fish such as salmon, or beef liver,
mushrooms and egg yolks make good choices. To treat
deficiency however, supplements are recommended.
Although green tea has not been studied in regards to
inflammation during IBS, the active ingredients called
polyphenols inside have been helpful in treating
inflammation during inflammatory bowel disease. Some
people may wish to drink green tea, and others try
capsules. Those sensitive to caffeine will need to limit
their intake but those on medication interacting with
green tea are best not to take any at all.
Curcumin is an active compound within turmeric,
packed full of polyphenols which have been shown to
lead to a 60% reduction in IBS prevalence, including
reduced abdominal pain and discomfort. Getting this
through the diet is difficult as you would need to have
several teaspoons of turmeric everyday, therefore a
supplement is more appropriate for most IBS sufferers.
WHIPL
YOUR ROAD TO RECOVERY AFTER A TRAFFIC ACCIDENT
By Dr. Evan Kabir
Whiplash refers to a neck injury that is
caused by sudden movement of the head.
This can occur forwards, backwards or sideways and
often occurs following a road traffic accident.
Common symptoms include:
Pain and stiffness causing difficulty in
moving the neck
Headaches
Tenderness over the muscles of the neck
Pain in the arms or shoulders
Commonly, symptoms do not occur immediately but
can take at least 6-12 hours to develop after the injury
and may continue for several days. Severe symptoms
can cause concern but investigation often fails to find
any abnormality. A description of events and the
symptoms and examination will often reveal the
diagnosis. Further tests rarely reveal any further
problems but may be requested if a fracture is
suspected.
PAIN RELIEF
Pain associated with whiplash can be initially treated
with simple painkillers. Paracetamol can be helpful in
treating mild neck pain. Non-steroidal antiinflammatory
drugs (NSAIDs), such as Ibuprofen, can
also help to reduce pain and inflammation. These
options are usually sufficient for most people.
Paracetamol or Ibuprofen can be taken separately ‘as
required’ or regularly if needed. If pain persists,
Paracetamol and Ibuprofen can be taken regularly
together but this depends on the severity of the pain
and/or patient preference.
If further pain relief is required, your GP will be able to
prescribe stronger painkillers. Always refer to the
manufacturer’s recommended instructions when taking
any painkillers. Ibuprofen should be avoided if you have
a stomach ulcer or had this in the past. You should also
avoid Ibuprofen if you have a history of severe heart
failure or liver disease. Take medical advice if in doubt.
EXERCISE
Try to keep your neck mobile with
There is evidence immobilising the
collars can actually delay recovery.
Therefore, early mobilisation and return to normal
activities is encouraged. This may initially be painful but
this is not harmful or a sign of further damage.
If symptoms persist despite self-managing symptoms, a
referral to physiotherapy may be required.
Physiotherapy can help improve pain and stiffness
associated with a neck injury. Your physiotherapist can
give you exercises to continue at home as well.
It is also important to maintain good posture when
sitting, standing or walking, by keep your back straight
and avoid slouching. You should also ensure you have a
supportive pillow when sleeping.
Most people with whiplash will find their symptoms
improve with these simple measures, but occasionally
symptoms persist for several months after the initial
injury. Consult your GP for further advice if your neck
pain is interfering with work and your day-to-day
activities.
PGPT
Get out more!
By Peter Gaffney
With the good weather fast approaching,
it’s a great time to take your exercise
outdoors.
This is a great opportunity to take in the fresh air and
some vitamin D from the sunshine in your garden or a
local park. The great thing about exercising outdoors is
that it’s free and there’s enough space to do it with
friends to make the experience more fun, and even
competitive. Exercise doesn’t have to be a chore and it
doesn’t have to take up your whole day.
Exercising outdoors can be daunting and we
understand that, so here is our guide to a great
workout, using nothing but your surroundings, your
bodyweight and maybe a mat.
Start off by warming up. By elevating your heartrate
you increase your body’s potential to burn fat, and help
to avoid injury by loosening your muscles. Start with a
jog for a minute, take up the speed for another minute
then flick those heels up towards your bottom. Finish
off with a minute of high knees (knees to chest) and
then a final minute of jogging…make sure you’re really
pumping your arms while you do it.
We recommend interval training. There’s nothing
better at burning calories and keeping them burning for
the next day or two. This type of high intensity training
will trigger the EPOC effect. This stands for ‘Excess
Post-exercise Oxygen Consumption’. During your
recovery period your metabolism is elevated due to
the increased intake of oxygen into the body following
strenuous exercise. This can continue for up to 38
hours following high intensity exercise, meaning your
daily energy expenditure is increased and more calories
are burnt for longer.
Time yourself for one minute and complete the
following: Burpees, tricep dips (using a bench or wall),
press ups, squat jumps, a plank and hill sprints. Repeat
this routine three times. Follow up with a good stretch.
So simple, yet ever so affective!
This routine will take no more than 30 minutes, so put
on your trainers and give it a go.
I guarantee you’ll have a clearer
head, less stress and feel fitter
in no time!
PATIENT UPDATE
Following national publicity surrounding Meningitis B, our vaccination service has seen
unprecedented demand. Whilst we have done all we can to satisfy the need for Meningitis B
vaccinations, we are also mindful of the impact this has had on our service for other patients.
When the sad story broke about a child fatality from Meningitis B, the Bexsero ® vaccine was in short supply as the
manufacturer temporarily withdrew stock from the open market. JDoc365 nevertheless managed to maintain a
regular supply and, as word spread, our telephone lines quickly became overwhelmed with calls from concerned
parents trying to arrange for their children to be vaccinated.
Despite setting up a dedicated line and hiring additional staff to cope with the volume of calls,
some JDoc365 customers were unable to get through before being re- re-directed to our
message service. We are sorry if you were inconvenienced by the temporary disruption this
situation caused, and that you will find our normally excellent reception service is back to its
best the next time you call.
Please be assured that all JDoc365 staff and doctors are working tirelessly to
ensure that our service and care are not compromised in any way.
Thank you for your understanding in this matter.
Caroline Katz knows how precious time
can be. When someone in the family needs
to see a doctor, time can be of the essence.
Not everything demands urgent attention though.
Sometimes Mrs Katz can wait to see her NHS doctor -
after all, they do a fantastic job.
Sometimes you need to see a
doctor sooner rather than later.
That’s when Mrs Katz and her
family turn to JDoc365.
Mrs Katz is a fitness instructor and her husband is CEO
of a telecoms business. The family have been members
of JDoc365 since the practice was first established in
2006. Mr and Mrs Katz and their children are relatively
healthy despite a busy lifestyle, pressure of work and
school, family commitments and the demands of a
close-knit and active community.
They really appreciate the fact that Dr. Abelman
and his team always find the time to see them, often at
short notice. It’s not always a medical crisis needing
emergency attention, it’s more that time is such a
precious commodity, and scheduling everything into
busy days can be quite a challenge.
Mina Mikram, the JDoc365 Golders Green Clinic
Manager, always recognises Mrs Katz when she visits
the surgery, and his warm greeting is much appreciated.
It’s the time, care and attention given by Dr. Abelman
that Mrs Katz is particularly grateful for though. Mrs
Katz explains, “It’s all in the detail. Dr. Abelman makes
it his business to understand his patients – whether
it’s me, my husband, one of my sons or my mother. He
is so calm and knowledgeable.
On one occasion I brought my eleven year old
in three times in ten days. Whilst we were
there, chest x-rays were organised and we
just went straight downstairs for them.
Because of that, my son was diagnosed
accurately, quickly and conveniently.
A precise dose of antibiotics were
prescribed and I collected them
from the pharmacy whilst I was
there. My son made a speedy
recovery, and the impact on
my business through my own
absence was minimised.
Even when Dr. Abelman himself isn’t available,
he has such a good team around him, it
makes no difference who you see.
Dr. Lewin saw us on one occasion and
he was great. Dr. Abramov is also a
fantastic doctor. She is so understanding
and has such a caring bedside manner.
My family and I owe them all a huge
vote of thanks.”
SEEING
RED
By Mr. Daniel Ezra
MA(Cantab) MBBS MMedEd MD(Cantab.) FRCS FRCOphth FHEA
Mr. Ezra is consultant ophthalmic surgeon to
Moorfields Eye Hospital, and practices privately.
Without doubt, almost all of you will have
experienced an episode of red eyes at some
point. There are many conditions that cause
this, and some can be quite alarming.
Eye problems contribute up to
7% of GP visits, and the majority
of these relate to red eyes
No discussion of red eyes in primary care can be made
without mention of blepharitis - an inflammatory
disorder of the eyelid margins. It is extremely common,
affecting up to 20% of people, and tends to present
with non-specific symptoms of redness, grittiness and
dryness. There are approximately 30 oil producing
glands in each eyelid that supply oil for the tear film.
This oily layer covers the watery layer of the tear film
preventing evaporation and providing stability. Changes
in the constitution of the oil secretions make them
solid rather than liquid at body temperature, clogging
the glands and allowing bacteria to colonise the eyelid
margins. These bacteria in turn produce toxins that
cause irritation (see figure 1). The best treatment is hot
compresses and massage which act to heat the eyelid
margin and liquefy the secretions. For more persistent
or serious complaints, steroids and antibiotics may be
required but this is unusual. Another option may be
deep cleaning using specialised equipment which usually
gives relief for 6-9 months.
Another very common condition is infective
conjunctivitis. This usually arises after an upper
respiratory tract infection such as a cold or the flu.
There are two types of infective conjunctivitis: bacterial
and viral. Bacterial conjunctivitis is easy to recognise; it
is characterised by significant mucous discharge which
typically causes the eyelids to stick together in the
morning, requiring considerable effort to reopen them
(see figure 2). Bacterial conjunctivitis is treated with
antibiotic eye drops. The viral form of conjunctivitis
feels very different. It causes a more watery discharge
with less stickiness, but paradoxically causes more
irritation and grittiness. Viral conjunctivitis is not cured
by antibiotic drops, but is treated with supportive
measures such as lubricants until the infection resolves,
usually after just a few days. Infective conjunctivitis,
especially the viral type is highly contagious. It is
important to prevent the spread of infection by
ensuring that hands are always washed and not to
share towels or bedsheets.
The red eye condition with probably the most alarming
appearance is also probably the most benign;
subconjunctival haemorrhage. This is a condition where
blood collects under the conjunctiva after a small
capillary ruptures. This is usually entirely benign and
tends to resolve after a few days (see figure 3).
Figure 1 Figure 2 Figure 3
Whilst these conditions are not sight threatening,
please remember that if you have a red eye with
symptoms of significant pain, aversion to light or
decrease in vision, this may indicate a more serious
problem and you should see an Ophthalmologist (eye
specialist) or a GP at the earliest opportunity.
