Evidence base<br />
Many people <strong>to</strong>ld Healthwatch Enfield<br />
that there was an urgent need for local<br />
residents with mental health issues<br />
<strong>to</strong> experience seamless support and<br />
care. This aligns with national and<br />
local initiatives <strong>to</strong> improve integrated<br />
working across professions, services<br />
and teams, and also fits well with a<br />
more truly person-centred approach<br />
<strong>to</strong> care. With NHS England asking local<br />
areas <strong>to</strong> create fully integrated health<br />
and social care systems by 2020-21,<br />
there is an urgent need <strong>to</strong> make further<br />
progress in this area.<br />
Service users and carers identified<br />
issues of poor linkages between<br />
inpatient and community mental<br />
health care; between GPs and other<br />
services; between physical and<br />
mental health care; between handovers<br />
from one shift and another, and even<br />
between different clinicians within t<br />
he same service. This can cause<br />
anxiety <strong>to</strong> service users and also<br />
increase demands on the system if<br />
service users’ health is affected; in<br />
severe cases, the person’s health<br />
could be put at serious risk.<br />
People <strong>to</strong>ld us:<br />
‘There is a lack of continuity of<br />
care, both in MH services and GP<br />
practices as there is a very high<br />
turnover of doc<strong>to</strong>rs and social<br />
workers. This is particularly<br />
disadvantageous <strong>to</strong> people with<br />
complex MH conditions, as it is<br />
important for professionals <strong>to</strong> be<br />
aware of their his<strong>to</strong>ry, and also <strong>to</strong><br />
build up trust between professional<br />
