Acknowledgments I want to thank absolutely everybody that contributed to this book. We never really counted the testimonials we received over the years. Most of them are not included in this book because they run in hundreds, maybe thousands. I want to thank the Students and Patients for those, unpublished stories, as well. Nothing would have been possible if Stephanie Coté didn’t collect and archive them. Then the whole collection of hundreds and hundreds of notes, emails and letters was sent to New Zealand, for Cathie Lindsey to sift through it, find the “top stories” and then the “top of the tops”. Thank you Stephanie and Cathie. We tried to keep the words the way people wrote them. There are American, British and Canadian spellings.
