Our team of colleagues worked very hard towards elevating our care beyond the basic fabric, by<br />

restoring self-worth and dignity back to our seniors. We started the year re-focusing our efforts<br />

across our different facilities, centralising many critical functions and identified key programmes<br />

and initiatives towards supporting our work.<br />

Identifying and Meeting the Needs of our Seniors<br />

Our seniors are the reason for our existence. We have met care needs of these seniors and have<br />

taken up the challenge of co-creation of programmes with our seniors. We find success in many<br />

first co-creation of programmes and meeting these needs. One of which was “We are Home”<br />

Project the replacement of the beds and frame for our 102 seniors in our Sheltered Home.<br />

Building Capability and Capacity<br />

With the crunch in manpower, Mentorship was introduced through the creation of Programme<br />

Leaders who with their vast experiences in the ILTC sector, together with life experiences are<br />

able to guide younger and new entrants into our industry. This has led to better integration and<br />

an improved understanding of the sector.<br />

Standards are important in our line of work. Our Policy & Audit Division was set up to standardise<br />

our Standard Operation Procedures (SOP) and ensure compliance. Through the use of technology,<br />

there was increased efficiency in information sharing across departments.<br />

Moving Forward<br />

We are pleased to share that SASCO will be operating two Senior Activity Centres in 2017 to<br />

support active and successful ageing, expanding our reach across the eldercare continuum. Our<br />

team will also be looking forward to new initiatives through sascoHOMECARE and sascoPOM to<br />

expand our services of care towards our ageing population.<br />

Refocusing our efforts was the key task this year. As we look forward to consolidate our efforts<br />

and embracing the year ahead, we rely on your continued support for our seniors.<br />

ABOUT US<br />

SASCO Senior Citizens’ Home is a registered charity since 1981. Currently, we<br />

run 1 Sheltered Home and 4 Day Care Centres. In total we serve more than 250<br />

seniors across different programmes offered by Ministry of Health (MOH) and<br />

Ministry of Social and Family Service (MSF).

Give n Take Campaign<br />

The demographics of our greying population is changing, the care needs of our<br />

seniors has to be redefined. In view of this, our new service standards is aligned<br />

towards the Ministerial Committee on Ageing’s Action Plan for Singaporeans to<br />

age successfully.<br />

The Give n Take Campaign is created on the fact that “Everyone is unique as each<br />

has something intrinsic to Give and something that we need to Take from the<br />

community we live in”. This spirit promotes value exchanges between our elderly,<br />

staff, volunteers, donors and communities for our new models of care.<br />

We see our seniors as active givers, moving away from the conventional passive<br />

receivers. They are empowered to have a say in the programme and activities to<br />

be involved in. Encouraging seniors in life-long learning supports the curiosity<br />

of every individual to learn in their areas of interest and equal opportunities for<br />

skills development. This initiative restores self-pride, self-worth and dignity to our<br />

seniors.<br />

Facilities Management (Renovations & Transport)<br />

SASCO Facilities Management Department was formally setup to oversee<br />

the renovations and enhancements required across our different facilities. The<br />

department has built in-house capability to serve our seniors better in terms of<br />

improved infrastructure and efficient transportation service through our fleet of<br />

vans.<br />

SASCO @Telok Blangah was renovated and enhanced. This was an expansion of the<br />

space to include a new pilot SPICE programme. With more space, the centre is now<br />

able to take in a total of 50 clients, an additional 20 under the SPICE Programme.<br />

The renovation was supported by Agency for Integrated Care (AIC) and funded by<br />

Ministry of Health (MOH).<br />

SASCO @Hong Kah North also received a makeover for the centre and its kitchen.<br />

More utilisation of space at the centre was maximised with the renovation. They<br />

were also able to have more storage for the essentials at the centre.

We Are Home<br />

Recognising the physical needs of our seniors, “We Are Home” started as a project<br />

to create a senior citizens’ friendly home at SASCO @Choa Chu Kang.<br />

Replacements of firm mattresses and sturdy bedframes and a sitting corner together<br />

with a coffee station at our Sheltered Home, SASCO @Choa Chu Kang sees the first<br />

step towards creating a homely feel for our seniors. The highlight is a gourmet coffee<br />

machine that our seniors make coffee as part of life-long learning.<br />

SascoDINE, led by our Head Chef, introduces new recipes and menus in<br />

consultation with our seniors. The department has a significant 47% cost savings<br />

due to the restructured procurement process and increased efficiency in the kitchen.<br />

High food hygiene standards have also been upheld through the re-training of<br />

staff and standardising the food preparation process. The team was also challenged<br />

to provide palatable food in terms of texture and taste as well as to ensure the<br />

nutritional needs of the seniors were met.<br />

Through co-creation with seniors on these menus, memories are relived through<br />

the taste, bringing back past experiences they have lived through.


sascoTHERAPY<br />

SascoTHERAPY is a unique blend of therapy to improve overall quality of life.<br />

Maintaining a core strength is important for seniors to continue to be independent<br />

in their Activities of Daily Living (ADL). Through different activities such as<br />

stretching and breathing techniques, incorporation of therapy-based equipment and<br />

technology.<br />

The HUR Equipment was included to improve rehabilitation of our seniors,<br />

enhancing their mobility and providing up to date technology support productivity.<br />

The exercises build the strength of the seniors in their ability to move. 80 seniors<br />

benefitted from the programme and gained strength towards ADL.<br />

By integrating therapy equipment and technology, our seniors are able to increase<br />

their self-awareness and appreciation through task orientated activities, elevating<br />

their self-worth and confidence.<br />

Geriatric Pharmaceutical Care<br />

GPC is an initiative under National Pharmacy Strategy to promote Pharmaceutical<br />

Care Excellence. This is a collaboration bringing together sasco@DACE, Agency<br />

for Integrated Care (AIC) and Ministry of Health Holdings (MOHH). This service<br />

is offered to Singapore Programme for Integrated Care for the Elderly (SPICE)<br />

Centres and Dementia Day Care. Our SASCO @Telok Blangah has a total of 44<br />

clients involved in the pilot study which seeks to provide medical reconciliation<br />

and review services to reduce drug-related problems and medical errors. A total of<br />

19 clients received GPC Services.<br />

This ensures the totality of care plan process and educated clients, caregivers and<br />

healthcare staff that the needs of the clients should be communicated and reviewed<br />

by a multi-disciplinary team to prevent fragmented care.<br />

SASCO @JurongWest<br />

April 2016 marks a new chapter for SASCO @JurongWest who joined us as our fourth<br />

Day Care Centre offering the Singapore Programme for Integrated Care for the Elderly<br />

(SPICE). The immediate task was to ensure a smooth transition of the clients from its<br />

previous service provider to our new care models and staff.<br />

SASCO @JurongWest cares for clients within the Jurong and Bukit Batok area, serving a<br />

much needed population who require higher care needs during the daytime. In FY 2016,<br />

we served 18 clients. Renovation plans are in the pipeline to improve our facilities for the<br />

direct benefit to our clients.


Supporting Community Care Funding<br />

for Seniors<br />

With more than half of our seniors still ageing within the community, our Social<br />

Work Team works closely with our various Community Development Councils<br />

(CDCs) where our Centre-based ILTC clients reside within. With a comprehensive<br />

list of financial assistance available to respective residents, our clients are educated<br />

on the different funds that they are able to utilise. Clients and their families are<br />

now more aware of the community resources they have access to. This is especially<br />

important for needy seniors who require additional Financial Assistance.<br />

Connecting the seniors back to their own community and grassroots ensures<br />

a consolidated effort between different agencies working towards the common<br />

benefit of our seniors. This echoes the Many Helping Hands Approach to build a<br />

more cohesive community.<br />

SASCO Human Resource Enhancement<br />

SASCO Career Enhancement Programme (CEP) 2016 was introduced to<br />

achieve quality care by promoting work-life balance to our caregivers and support<br />

staff. Our direct care staff has the option to choose a 4-Day work week, allowing<br />

them a longer rest time to recharge, as well as tend to their family needs. This is a<br />

welcoming benefit especially for staff who would like a greater balance to tend to<br />

their family.<br />

The Community Engagement Benefit (CEB) under this programme supports<br />

our staff to be engaged with community programmes such as outings to places<br />

of interest, encouraging sporting activities to support creativity and innovation.<br />

With the exposure to non-work related activities, a more supportive and enriched<br />

social life, our staff returns to work focused and motivated.<br />

With the ILTC sector depending larging on foreign workers, 2016 saw SASCO<br />

moving towards increased dependency on local staff in the care of our seniors. A<br />

replacement of 39% of foreign workers to local staff. With similar cultural contexts,<br />

our seniors felt more understood when communicating in local dialects. This is a<br />

significant step in improving the interaction between our direct care staff and has<br />

elevated quality of care for our largely communicative seniors.<br />

Policy & Audit<br />

The Policy & Audit Department oversees the standards that all centres and<br />

Home are looked after, centralising the standards of operations in all our centres.<br />

This ensures that staff at all facilities were able to focus on their operations towards<br />

direct care of our seniors. The department focuses on 1. Policy Formulation and<br />

Compliance to Service Standards, 2. Internal and External Audits, 3. Automation<br />

and Technology.<br />

In FY 2016, there were a total of 17 audits across our 4 Day Care Centres and<br />

Home. This ensures that the same level of service standards are maintained across<br />

our different programmes.<br />

Through the VWOs-Charities Capability Fund (VCF) from National Council of<br />

Social Services (NCSS), a review to convert our previous ISO Standard 9001:2008<br />

to the new certification to ISO 9001:2015 is in progress.<br />

The direct integration of our Cloud Computing to Intermediate Long Term Care<br />

(ILTC) Portal has been completed with the assistance of Agency for Integrated<br />

Care (AIC). The implementation of Cloud Computing supports the platform for<br />

clients’ data to be stored in one central place, ensuring the completeness and of<br />

data including not limited to nursing notes and social work reports.<br />

This allows a more efficient way of sharing information across our centres and<br />

Home. A secure and larger data storage enables our different functions and<br />

departments to operate in a seamless manner. HR and Finance module has also<br />

been introduced to improve productivity for our inter-department work.<br />

Donor Payment Migration<br />

Through the streamlining of our donor payment process, the migration of our<br />

processing bank is completed and is now managed with our local bank. This<br />

ensures that any transactions that need to be completed for donors are efficiently<br />

attended to in a shorter timeframe. The migration has reported a savings of<br />

$12,000 a month.<br />

This is also in line with the Payment Card Industry Data Security Standard (PCI<br />

Temasek Foundation Cares –<br />

Carer Support@Centres<br />

Our SASCO @Hong Kah North is one of the pilot centres for Temasek Foundation<br />

Cares – Carer Support@Centres with 3 other agencies led by Agency for Integrated<br />

Care (AIC). The programme aims to establish a new care model that supports both<br />

the seniors and caregivers in the caregiving journey. Key programme elements<br />

look into supporting caregiving stress and sustaining care for their loved ones<br />

in the community. 40 caregivers at SASCO @Hong Kah North have benefited<br />

from the programme and have reported to cope better with caregiving stress and<br />

managing their loved ones.<br />

needs and requirements of our seniors. Each of our Home and Centre has a Social<br />

Worker to care for the needs of the seniors and as well expand this care to their<br />

Extending Our Reach -<br />

Moving into Active Ageing – SACs<br />

There is a critical need to support active seniors to remain healthy and socially<br />

engaged, a challenging need for our younger generation to step up and remain<br />

active in their social relationships with their ageing parents and grandparents, to<br />

promote “Home First” initiatives within our community. With this, our team is<br />

ready to seek more opportunities in the eldercare continuum.<br />

SASCO has been awarded two Senior Activity Centres - SASCO @Khatib and<br />

SASCO @Compassvale to operate for 2017. This is a huge step towards building<br />

our capacity and capability in supporting successful ageing. This adds another<br />

dimension of eldercare into our work.<br />

Senior citizens are encouraged to utilise the facilities at the Senior Activity Centre.<br />

Facilities such as the Cafe, Cooking Studio and Library Corner at Khatib and<br />

Training Studio and Artist Corner at Compassvale. Certain facilities are also<br />

available for public use. With these two SACs, we challenge our team to innovate<br />

and introduce refreshing programmes that encourage different communities to<br />

come together to promote our Give n Take spirit.<br />

sascoHOMECARE<br />

SascoHOMECARE is an integrated care service that allows our seniors to stay<br />

in their homes while delivering the required care to them. This chapter is a huge<br />

step forward in terms of partnership with a private healthcare provider to serve<br />

the needs of our seniors in the comfort of their own homes.<br />

Bringing affordable and quality care to seniors at their homes can support<br />

their independent living and maintain their dignity. sascoHOMECARE model<br />

is designed to provide holistic high quality and continuous care for acute and<br />

complex patients at home. This is also modelled upon Japan’s quality of eldercare<br />

sascoPOM<br />

SascoPOM is a pilot programme, partnering South West CDC and Agency for<br />

Integrated Care (AIC), extending our Give n Take Spirit towards community<br />

funding and assistance. This model of co-funding with authorities will subsidise<br />

programmes under AIC to ensure inclusion of needy seniors. Through this<br />

programme, multi-agencies are able to capitalise on one another’s strength to<br />

provide assistance for our needy Singaporeans. This extends beyond benefiting<br />

only seniors under our care, but includes needy residents within the district. This<br />

benefits the group between the income level $900 - $2400.


sascoFUTURE<br />

Inspired by the model of the Enabling Village, SASCO hopes to work with<br />

agencies to support an assisted eldercare living in Singapore. With the changing<br />

demographics of seniors, there will be a vacuum between seniors being cared for<br />

in a centre-based day care and the nursing home. This group of baby boomers<br />

may be single with no immediate families to care for them.<br />

The collective services and conducive active ageing environment will provide<br />

assisted living while at the same time maintaining the independence of this<br />

group of seniors. Capitalising on the synergy of collective and group living, this<br />

is envisioned to be a collaborative effort among the community in the heartlands,<br />

providing essential services and a sense of belonging.


sascoMANAGE<br />

SascoMANAGE is a platform for skilled volunteers and regular donors to come<br />

together to make a difference for our seniors. These volunteers will be sharing<br />

their expertise in either direct or non-direct care to our seniors. Recognising<br />

them as a valued stakeholder in the care journey of our seniors is an important<br />

step towards building a culture of sharing expertise and value exchanges between<br />

different stakeholders in the community.<br />

My Childhood, my Companion, my Love.<br />

With fond eyes and a gentle voice, Mr Muthusamy shared about his childhood with us. Brought<br />

up in the workers’ quarter, he remembers the days when football and rabbits were his love. His<br />

constant walk out to the temple behind the Home has a simple reason – he wanted to feed the<br />

rabbits and relive the days when he was growing up.<br />

One day, Mr Muthusamy’s friend found out that he no longer visit the rabbit shelter and offered<br />

to bring rabbits to our Home. This was the start of our sascoACT Rabbit Therapy. Since then,<br />

there was no looking back but enjoying the company of these little fluffies, bringing lots of joy<br />

to our seniors.

Mdm Yeo Muey Kia, SASCO @Hong Kah North<br />

Since attending the various activities at the Centre, Mdm Yeo is more cheerful and is more meaningfully<br />

engaged. She enjoys karaoke and exercise sessions, and loves interacting with her friends. Her daughter<br />

is thankful for her care in SASCO @Hong Kah North as it provides respite for her and her family.<br />

Mdm Yeo has never heard nor visited the Universal Studios. Her mobility challenge has been the<br />

obstacle to a much cherished family outing. Through the partnership of SASCO with Community<br />

A Quiet Loss of Memory.<br />

Mdm Teo Ah Yan, SASCO @Telok Blangah<br />

85 year old Mdm Teo Ah Yan was a home-maker with 5 children. She now attends our Dementia Day<br />

Care Programme and have gained more independence in standing, exercise and walking with frame.<br />

Her son also benefited from caregiver respite as this programme allows her to take on odd jobs to earn<br />

income when he can and gives him a peace of mind.<br />

Although Mdm Teo was diagnosed with dementia, she could know who was missing from the Day<br />

Care that day. She has a loud voice and can always be seen and heard exchanging a conversation with<br />

other seniors at the centre. Due to her recent admission to hospital, we are working with Mdm Teo to<br />

manage her episodes of outbursts to maintain her hypertension.


Immobility does not stop you from Moving Forward.<br />

Mr Wong Kong Yong, SASCO @Jurong West<br />

Since the onset of his medical condition in August 2006, Mr Wong has been ADL dependent and<br />

requires care. Mdm Lim, his wife who has been taking care of him has experienced tremendous stress<br />

and her health has been affected. Mr Wong and Mdm Lim is staying in a fully-paid 4-room HDB flat.<br />

Their marital relationship is good despite strained at times. They have no children and seek financial<br />

and emotional support from both their siblings.<br />

Since coming to SPICE, Mr Wong no longer has to be in bedridden during the day when Mdm Lim<br />

is at work. He enjoys engaging in exercises and activities with other clients in SASCO @Jurong West<br />

and carries a positive attitude towards life. Although one side of his body is immobile, he is often seen<br />

making the effort to move short distances, shuffling with his foot on his wheelchair.

Mdm Soon Kim Teow, SASCO @West Coast<br />

Mdm Soon used to have difficulty engaging in ADL, such as putting her arm to her back or reaching<br />

to the cabinets to retrieve items. Ever since the implementation of sascoTHERAPY, she has been<br />

diligently attending every therapy session and has seen remarkable results. Her limbs are no longer as<br />

stiff and she feels more independent then before.<br />

Mdm Soon’s easy-going personality also helps her to stay mentally healthy and socialise with her<br />

friends at the centre. She was selected and being presented ‘Best Fitness Award’ by Sasco and received<br />

her certificate from Dr. Amy Khor, Senior Minister of State.
