Leucovor<strong>in</strong>, Oxaliplat<strong>in</strong>, Ir<strong>in</strong>otecan, Capecitab<strong>in</strong>e, Bevacizumab, Panitumumab, and<br />

Cetuximab for patients with advanced colorectal carc<strong>in</strong>oma conf<strong>in</strong>ed to <strong>the</strong> peritoneal<br />

surface is unknown.<br />

A standardized, evidence-<strong>based</strong> approach is currently lack<strong>in</strong>g for patients with<br />

peritoneal surface malignancy from colorectal orig<strong>in</strong>. A collaborative trial with<br />

surgical quality assurance and modern multi-drug chemo<strong>the</strong>rapy <strong>in</strong>corporat<strong>in</strong>g critical<br />

assessment <strong>of</strong> disease burden, determ<strong>in</strong>ants <strong>of</strong> complete cytoreduction, <strong>treatment</strong>related<br />

toxicity, quality <strong>of</strong> life and survival is imperative. The NKI trial, although not<br />

perfect, has provided <strong>the</strong> basis and impetus for fur<strong>the</strong>r study utiliz<strong>in</strong>g a new reference<br />

study arm for future randomized trials <strong>of</strong> appropriately selected patients with<br />

potentially curable regionally advanced colorectal carc<strong>in</strong>oma conf<strong>in</strong>ed to <strong>the</strong> parietal<br />

peritoneal surface.<br />

Based on <strong>the</strong> consensus statement <strong>of</strong> <strong>the</strong> Peritoneal Surface Oncology Group<br />

(PSOG) published <strong>in</strong> 2007 [84], a prospective randomized cl<strong>in</strong>ical trial was designed<br />

by <strong>the</strong> PSOG and <strong>the</strong> United States Military Cancer Institute (USMCI). This,<br />

multicenter cl<strong>in</strong>ical trial will start accru<strong>in</strong>g patients <strong>in</strong> 2009. Included will be patients<br />

with PC <strong>of</strong> colorectal orig<strong>in</strong> with a PCI
