Accreditation Statement<br />
This activity has been planned and implemented in accordance with the accreditation requirements and policies of the Accreditation<br />
Council for Continuing Medical Education (ACCME) through the joint providership of Christian Medical & Dental Associations<br />
and Christian Community Health Fellowship. The Christian Medical & Dental Associations is accredited by the ACCME to provide<br />
continuing medical education for physicians.<br />
AMA Designation Statement<br />
The Christian Medical & Dental Associations designates this educational activity for a maximum of 5 AMA PRA Category 1<br />
Credits(s). Physicians should only claim credit commensurate with the extent of their participation in the activity.<br />
AAFP<br />
Application for CME credit has been filed with the American Academy of Family Physicians. Determination of credit is pending.<br />
Dental Credit<br />
The Christian Medical & Dental Associations is an Approved PACE <strong>Program</strong> Provider by the Academy of General Dentistry. The<br />
formal continuing education programs of this program provider are accepted by AGD for Fellowship/<br />
Mastership and membership maintenance credit. Approval does not imply acceptance by a state or provincial<br />
board of dentistry or AGD endorsement. The current term of approval extends from 1/1/2015 to 12/31/<strong>2018</strong>.<br />

Physician Assistant<br />
AAPA accepts certificates of participation for educational activities certified for AMA PRA Category 1<br />

Nurse Practitioner<br />
The American Academy of Nurse Practitioners Certification <strong>Program</strong> (AANPCP) accepts AMA PRA Category<br />
1 Credit from organizations accredited by the ACCME. Individuals are responsible for checking with the<br />
AANPCP for further guidelines.<br />
Nurse practitioners may receive up to 5 credits for completing this activity.<br />
Nursing<br />
This activity has been submitted to the American Association of Occupational Health Nurses (AAOHN) for<br />
approval to award contact hours. AAOHN is an accredited approver of continuing nursing education by the<br />
American Nurses Credentialing Center’s Commission on Accreditation. Determination of credit is pending.<br />
DOCTOR OF OSTEOPATHY (DO)<br />
The American Osteopathic Association will award credit to DO’s when they mail their certificate to American<br />
Osteopathic Association, 142 East Ontario Street, Chicago, IL 60611 or email the certificate to: cme@<br />
osteopathic.org. The AOA awards credits for educational activities that are accredited to provide AAFP<br />
