There is a laboratory and pharmacy on site. Both accept orders<br />
from outside providers. X-rays and other diagnostic imaging services<br />
are available through an agreement with a local radiology<br />
clinic. Some services are available at Foust location only.<br />
Clinical Services Not Available: <strong>University</strong> Health Services<br />
does not provide major or elective surgery; hospital inpatient care;<br />
emergency room care for life-threatening injuries and illnesses;<br />
obstetrical care; physical therapy; fitting of glasses or vision care;<br />
or dental care. Referrals to area specialists and other health care<br />
providers are arranged as needed.<br />
Health Education Services Available: A Health Educator is<br />
available to assist with programs and to act as a resource for students.<br />
Emphasis is on creating and maintaining an environment<br />
that fosters healthy decision making and lifestyles. Self-ordered<br />
HIV testing and light therapy appointments are also available.<br />
Payment: There are fees for the care provided, but payment<br />
is not required on the date of service. The charges may be paid<br />
by cash, check, money order, credit or debit card in the Business<br />
Office, Foust 202 within three business days or be placed on the<br />
student’s personal CMU account and paid by the statement due<br />
date. If the student provides proof of current health insurance<br />
coverage (a health insurance card or photocopy of it) and signs<br />
an authorization permitting Health Services to bill the health<br />
plan for the services, insurance billing is provided. The student<br />
remains responsible for payment of any co-pays, deductibles and<br />
non-covered services. Health Services participates with a number<br />
of health insurance and prescription drug plans. For additional<br />

Student Health Insurance: It is highly recommended that all<br />
CMU students carry health insurance coverage. Having health<br />
insurance is important for several reasons:<br />
• Uninsured people receive less medical care and less timely<br />
care.<br />
• The uninsured typically have worse health outcomes.<br />
• To minimize the financial burden of an unforeseen illness.<br />
The student should carry their health insurance card or a photocopy<br />
of it and have at least a basic understanding of how the<br />
plan works, e.g., benefits, exclusions, pre-authorization requirements,<br />
co-pays and deductibles. A national health insurance<br />
provider offers and administers a health insurance plan for CMU<br />
students, their spouses and dependents at a very competitive<br />
cost. Plan benefits, premium costs, enrollment deadlines and<br />
application forms are available at the Health Services Business<br />
Office, Foust 202.<br />
Medical Records: Medical records are maintained for students<br />
and other authorized users of <strong>University</strong> Health Services. At age<br />
eighteen, students are legally adults. The information in their<br />
medical record is confidential and will not be released to anyone<br />
without the student’s knowledge and written authorization, except<br />
as required or permitted by law. Students are asked to complete<br />
a pre-entrance health history report and pre-matriculation immunization<br />
record to provide a baseline of important health<br />
information to help assure safe, effective, and appropriate care<br />
when the student is seen at Health Services or in the event of a<br />
medical emergency.<br />
Concerns, Comments, Suggestions: <strong>University</strong> Health Services<br />
seeks to continually improve services. Feedback from CMU<br />
students is valuable in that process. Concerns, comments and<br />
suggestions regarding the services can be shared by completing<br />
a Patient Satisfaction Survey at the time of the visit, speaking<br />
with a staff member or unit manager, or contacting the Director<br />
by calling 989-774-3944 or sending an email to healthservices@<br />
cmich.edu.<br />

Counseling Center<br />
The Counseling Center is located in 102 Foust Hall; to make an<br />
appointment call 989-774-3381. Hours are 8:00 a.m. to noon and<br />
1:00 p.m. to 5:00 p.m. Monday through Friday when the university<br />
is open. The Counseling Center’s website (www.counsel.cmich.<br />
edu) provides information on center services and counselors,<br />
current groups, community resources, confidential on-line mental<br />
health screening, emergency services and links to a wide variety<br />
of information. Counseling Center faculty includes licensed<br />
professional counselors, psychologists, and social workers. On<br />
occasion, trainees who are advanced graduate students in various<br />
counseling related graduate programs may also provide counseling<br />
services. Call the Counseling Center or stop by to schedule<br />
an appointment with a counselor. Daily urgent and emergency<br />
appointment times are available. Please tell our receptionist if<br />
you need to see a counselor the same day you call for an appointment.<br />
In these situations contact the center as early in the<br />
day as possible.<br />
The Counseling Center provides free and confidential services for<br />
currently enrolled CMU students for various issues and concerns<br />
that may negatively impact one’s academic success, interpersonal<br />
relationships, health or safety. Services include short-term / timelimited<br />
individual and group counseling, consultation and referral<br />
for: 1) dealing with personal issues such as an urgent situation<br />
or crisis, anxiety, depression, loneliness, transition to college<br />
concerns, identity, alcohol/drug abuse, eating concerns, stress<br />
management, relationship concerns, couples concerns, family<br />
stress, loss, relapse prevention, sexual orientation, strengthening<br />
coping skills, and personal growth and development; 2) improving<br />
academic success by reducing stress and focused anxiety (e.g.,<br />
test, math, speech), and learning time management skills; and 3)<br />
assisting primary and secondary survivors of sexual and domestic<br />
violence, stalking, and harassment. Group services are designed<br />
to address many topics and vary depending upon student needs<br />
from semester to semester.<br />
Other Counseling Center services include providing consultation<br />
and referral information for students, parents, faculty and staff;<br />
groups and workshops; in-service training; outreach programs;<br />
referral to on and off-campus agencies and services; providing a<br />
supervised practicum or internship site for psychology, counselor<br />
education and social work students; and selected mental health<br />
screening. During the academic year counselors are on-call after<br />
hours and weekends to assist students and CMU faculty and<br />
staff when there are urgent / emergency student mental health<br />
concerns. Call CMU Police at 989-774-3081 to contact the on-call<br />
counselor. There is no charge for counseling, consultation and<br />
related services; however there may be a modest fee to cover<br />
the costs associated with test administration.<br />
