Introduction How well and how quickly a product design is developed largely determines the product’s longevity and, ultimately, its gross revenues and profitability. In the design cycle, the importance of prototyping during the machining of orthopaedic implants and surgical instruments cannot be overestimated. In the first phase of development, prototypes are machined from materials that are easy and less expensive to work with and, later in the development process, from materials in which the end product will be produced. Iterative Prototyping The key to a strong and mature design is to maximize the number of iterations of the design and to thoroughly test them. All the up-front thinking in the world can often overlook a minor or even major flaw that a prototype will expose. Every successive prototype and lab experiment adds insight leading to a more mature design. While computer modeling is a valuable tool for design, it cannot supplant the benefits gained from prototypes and associated lab experimentation. As an example, a start-up company developing a spinal implant machined the first prototype in brass from the design solid model. The prototype was one step beyond a Rapid Prototype and was a useful visualization tool. The next three to five prototype iterations were also non-functional, but were machined in 303 S.S. and used in plastic skeletal mock-ups. As the design progressed to the point where initial lab studies were needed, the prototypes were machined in both 303 S.S. (for ease of machining) and 17-4PH S.S. (for use in gauging stresses). This phase consisted of eight to ten iterations. As the engineers came to understand the issues in play and the design was close to fully-functional, the prototype iterations were produced in the anticipated production materials of titanium and cobalt-chrome. Over ten iterations were made, and these prototypes represented the final stage of the process before the design was frozen and the product taken to trials. Larry E. James President New Concepts Prototyping and Production PRODUCT DEVELOPMENT The Value of Prototype Iteration As the design developed to a point of functionality that was near expectation, the company launched a schedule of weekly cadaver labs in which the latest iterations were tested. A team of engineers and physicians evaluated components and techniques and, in one or two days after each lab, would have new designs for further prototyping. A small number of new prototypes were built every week for the next week’s lab, giving this team fullyfunctioning labs every week with the ability to adjust and improve their designs. Knowledge gleaned from the successive lab experiences accelerated the development process exponentially, leading to more functional designs and refined techniques. The speed with which the design iterations were produced enabled the company to enter human trials much sooner than anticipated. Although the intense schedule required major commitments from both company and supplier, the dividends were large in terms of enabling the introduction of a superior product and the achievement of maximum profitability. Basic Reverse Chart In the 1980s, Harvard Business Review published a tool used in the prototyping example given above and known as the Reverse Chart. This tool helped decision-makers assess the value of expediting a product’s time-to-market and evaluate specific design feature-sets in light of overall company impact. For a product development team, the chart can help visualize the financial impact of their decisions. As seen in Figure 1 on the following page, the Reverse Chart is simply a 2-axis graph that plots anticipated Cumulative Development Expenses and anticipated Cumulative Gross Income. The horizontal axis represents time and the vertical axis represents dollars in logarithmic scale. In a typical project, R&D expenses start out slowly and accelerate until product introduction, at which time they flatten slowly. Eventually, the product is mature and no further development costs are incurred.
