KAMIL PACHALKO
Hello, my name is Kamil Pachalko
and I have the privilege of
managing the Adult Service
Transformation Team. We
support our colleagues, partners
and reach out to communities
to foster integration so that
residents interacting with
social or health care have to
“tell their story only once”.
We promote strength and asset
based approaches starting with
“What matters to you” and
“What is strong” to build on
peoples gifts. This way we hope
to promote sustainable wellbeing
while addressing real challenges
present in people’s lives.
We have been supporting the
development of the Integrated
Multidisciplinary Team in
the Localities to strengthen
relationships and collaboration
amongst staff and partners to
improve care. With partners we
stimulated the development
of community hubs as spaces
where both professionals, charity
workers and the community
members can work together
to provide one stop shops for
support and involve people in
making their communities more
resilient. We supported the
placement of social workers
in GP surgeries and Hubs to
take pressure off primary care
and have early, preventative
conversations. We are running a
Moderate Needs Multidisciplinary
meeting to discuss amongst a
diverse team complex cases to
provide support early before
the crisis point is reached and
we building on the adults and
the professional and social
networks assets. We have
rolled out training in strength
and asset based approaches,
started mapping community
resources to make them more
accessible to the public and
services and have stimulated
conversations around Asset
Based Community Development
to strengthen positive action by
individuals and communities,
with the local authority being
there to help where needed.
We have run events focused
on wellbeing to promote local
services and resources and to
engage community members
in a conversation about what
does living well mean to them.
We are testing how technology
might facilitate wellbeing by
using Pepper the Robot in care
homes and community groups
and exploring how Virtual Reality
could help people improve their
wellbeing through, for example,
experiencing a woodland walk
in Belfairs Woods even though
they might be housebound or
help someone work through their
anxiety of visiting one of the
local Hubs.
I hope you will find something
relevant for you in the list above.
Please come and talk with us at
the locality events. If you wanted
