Hello, my name is Kamil Pachalko<br />

and I have the privilege of<br />

managing the Adult Service<br />

Transformation Team. We<br />

support our colleagues, partners<br />

and reach out to communities<br />

to foster integration so that<br />

residents interacting with<br />

social or health care have to<br />

“tell their story only once”.<br />

We promote strength and asset<br />

based approaches starting with<br />

“What matters to you” and<br />

“What is strong” to build on<br />

peoples gifts. This way we hope<br />

to promote sustainable wellbeing<br />

while addressing real challenges<br />

present in people’s lives.<br />

We have been supporting the<br />

development of the Integrated<br />

Multidisciplinary Team in<br />

the Localities to strengthen<br />

relationships and collaboration<br />

amongst staff and partners to<br />

improve care. With partners we<br />

stimulated the development<br />

of community hubs as spaces<br />

where both professionals, charity<br />

workers and the community<br />

members can work together<br />

to provide one stop shops for<br />

support and involve people in<br />

making their communities more<br />

resilient. We supported the<br />

placement of social workers<br />

in GP surgeries and Hubs to<br />

take pressure off primary care<br />

and have early, preventative<br />

conversations. We are running a<br />

Moderate Needs Multidisciplinary<br />

meeting to discuss amongst a<br />

diverse team complex cases to<br />

provide support early before<br />

the crisis point is reached and<br />

we building on the adults and<br />

the professional and social<br />

networks assets. We have<br />

rolled out training in strength<br />

and asset based approaches,<br />

started mapping community<br />

resources to make them more<br />

accessible to the public and<br />

services and have stimulated<br />

conversations around Asset<br />

Based Community Development<br />

to strengthen positive action by<br />

individuals and communities,<br />

with the local authority being<br />

there to help where needed.<br />

We have run events focused<br />

on wellbeing to promote local<br />

services and resources and to<br />

engage community members<br />

in a conversation about what<br />

does living well mean to them.<br />

We are testing how technology<br />

might facilitate wellbeing by<br />

using Pepper the Robot in care<br />

homes and community groups<br />

and exploring how Virtual Reality<br />

could help people improve their<br />

wellbeing through, for example,<br />

experiencing a woodland walk<br />

in Belfairs Woods even though<br />

they might be housebound or<br />

help someone work through their<br />

anxiety of visiting one of the<br />

local Hubs.<br />
