<strong>Benefits</strong> Summary<br />
The Community Action Response Plan<br />
is GBUACO’s initiative to unite churches,<br />
community based organizations, and<br />
medical partners in Western New York for a<br />
comprehensive approach to combat the<br />
coronavirus and mitigate its effects on the<br />
community by:<br />
• Conducting outreach efforts to<br />
provide education and connections to with<br />
primary care providers, social support, and<br />
community resources<br />
• Launching accessible, multi-site<br />
testing centers to test high risk,<br />
symptomatic patients<br />
• Utilizing tele<strong>health</strong> and<br />
telemonitoring systems to address the<br />
virus and maintain patient <strong>health</strong><br />
• Creating a voluntary isolation unit<br />
within hotels to reduce the burden on<br />
hospitals and families for those with<br />
mild-to-moderate coronavirus symptoms<br />

put together various YOGA classes that can<br />

Doctor/PHN visits: For those patients<br />
that need to meet with their Primary<br />
Care Physicians or Patient Health<br />
Navigators, can do so through<br />
video/tele-visit by accesing the Patient<br />
Portal through the apps, website or<br />

COVID-19 screenings: Anyone who<br />
has downloaded the GBUAHN, GBUACO<br />
or UFP apps can complete a COVID-19<br />
survey if they are experiencing<br />
symptoms. For those that do not have<br />
a smartphone, they can call our Nurse<br />
