Autoimmune disorders, metabolic diseases,and hormonal dysfunctions are some of the endocrine complicationsobserved during or after treatment with immunotherapy (mostlynovel agents) and/or chemotherapy conditioning for transplantation.Although successful treatment of the underlying hematologicalcondition often improves the dysfunction, endocrinopathies canhave an impact on prognosis and are associated with poor survival;therefore, it is important to detect and treat them as early as possible.An increased incidence of cardiovascular diseases and metabolicsyndrome has been observed after transplantation mostly in longtermsurvivors. In addition, chemotherapy and radiation along withthe prolonged use of corticosteroids can contribute to the onsetof thyroid and gonadal dysfunctions. The aim of this article is todescribe metabolic dysfunctions occurring in patients who underwentallogeneic cell transplantation.Keywords: Diabetes, Metabolic complications, Hematological disease,Hematopoietic cell transplantation, Endocrine disordersÖzKemoterapi tedavisi ve otolog ve allojeneik hücre transplantasyonları,metabolik ve endokrin bozuklukların başlaması ile sıklıkla komplikeolmaktadır. Otoimmün bozukluklar, metabolik hastalıklar, vehormonal disfonksiyonlar immünoterapi (çoğunlukla yeni ajanlar)ve/veya transplantasyon için uygulanan hazırlama rejimi sırasındaveya sonrasında gözlenen bazı endokrin komplikasyonlardır. Alttayatan hematolojik durumun başarılı tedavisi endokrin disfonksiyonusıklıkla iyileştirmekle birlikte, endokrinopatilerin prognoz üzerineetkisi olabilir ve kısa yaşam süresi ile ilişkilidir; bu nedenle mümkünolduğu kadar erken saptanmaları ve tedavi edilmeleri önemlidir.Çoğunlukla uzun dönem sağkalan hastalarda transplantasyon sonrasıkardiyovasküler hastalıklar ve metabolik sendromun insidansındaartma gözlenmektedir. Ek olarak, kortikosteroidlerin uzun sürelikullanımı ile birlikte kemoterapi ve radyoterapi tiroid ve gonadalbozuklukların başlamasına katkıda bulunabilir. Bu yazının amacıallojeneik kök hücre transplantasyonu uygulanan hastalarda metabolikbozuklukların anlatılmasıdır.Anahtar Sözcükler: Diyabet, Metabolik bozukluklar, Hematolojikhastalık, Hematopoietik hücre transplantasyonu, Endokrin bozukluklarIntroductionPatients with hematological diseases undergoing chemotherapyand/or hematopoietic cell transplantation (HCT) couldexperience endocrine and metabolic complications affectingtheir quality of life in a chronic way [1,2,3]. The occurrenceof metabolic complications can be related to different factorsincluding hematological disease, preexisting risk conditions,cancer treatments, and HCT conditioning regimen modalities(total body conditioning and type of chemotherapy).Cancer treatment often consists of a combination ofcorticosteroids with chemo-immunotherapy that can favor thedevelopment of metabolic alterations. Furthermore, the use ofimmunosuppressive agents in HCT settings is another iatrogeniccause (Table 1). Nevertheless, the majority of available dataon the occurrence of endocrine complications refers topediatric populations. Reports on the endocrine consequencesof allogeneic transplantation at an adult age are poorer anddisparate.Progress made in the cure of cancer has allowed for an increasein the numbers of survivors of hematological diseases.
