<strong>Club</strong> Rules<br />

1. <strong>Siemens</strong> <strong>Hearing</strong> <strong>Instruments</strong>, a division of <strong>Siemens</strong> Canada<br />

Limited (“<strong>Siemens</strong>”) reserves the right to pay rewards on a<br />

monthly basis.<br />

2. <strong>Siemens</strong> customers will be automatically enrolled each fiscal year<br />

(October 1 – September 30) and placed in the appropriate Rewards<br />

Level based on annual <strong>Siemens</strong> purchases (net sales and net units)<br />

as per <strong>Club</strong> <strong>Siemens</strong> Qualification conditions.<br />

3. Net purchases is defined as gross hearing aid purchases net of<br />

credits and before volume discount.<br />

4. Net units is defined as gross hearing aid purchases net of credits.<br />

5. Purchases made under certain government funded programs are<br />

ineligible for inclusion in such rewards programs and therefore can<br />

not be included in the calculation of net units and net purchases for<br />

the purpose of Qualification.<br />

6. For customers joining <strong>Club</strong> <strong>Siemens</strong> after October 1,<br />

<strong>Club</strong> <strong>Siemens</strong> Rewards will be pro-rated based on the respective<br />

qualification date.<br />

7. <strong>Siemens</strong> reserves the right to cancel accrued rewards on dormant<br />

accounts after 24 months.<br />

8. <strong>Siemens</strong> reserves the right to invoice new customers* for all<br />

monetary rewards if 75% of their respective annual qualification<br />

target is not achieved during the year. Re-payment to <strong>Siemens</strong> is<br />

required within 30 days of the invoice.<br />

9. The customer is responsible for reporting and remitting any<br />

applicable taxes related to a <strong>Club</strong> <strong>Siemens</strong> Rewards.<br />

10. Unused rewards will be carried forward at the end of each fiscal<br />

year for use in the following year.<br />

11. Bonus Rewards are based on performance in the fiscal year<br />

(October 1 -September 30) and will be awarded as of October 1<br />

of the next fiscal year.<br />

12. The customer is responsible for initiating any review of account<br />

discrepancies.<br />

13. The customer is responsible for initiating any request to move<br />

up <strong>Club</strong> Rewards Levels during the fiscal year.<br />

14. Proof of business related purchase is required in order to claim<br />

monetary rewards.<br />

15. In order to claim monetary rewards, all customer accounts<br />

with <strong>Siemens</strong> must be current and in good standing.<br />

16. Use of <strong>Club</strong> <strong>Siemens</strong> logo is prohibited without the express written<br />

consent of <strong>Siemens</strong>.<br />

17. Qualification for <strong>Club</strong> <strong>Siemens</strong> and <strong>Club</strong> <strong>Siemens</strong> Rewards Level is<br />

at the sole discretion of <strong>Siemens</strong>.<br />

18. <strong>Siemens</strong> reserves the right to add or remove customers from <strong>Club</strong><br />

<strong>Siemens</strong> in its sole discretion without notice.<br />

19. <strong>Siemens</strong> reserves the right to modify or discontinue its <strong>Club</strong><br />

<strong>Siemens</strong> program at any time, without penalty.<br />

20. <strong>Siemens</strong> Terms and Conditions shall continue to apply to<br />

all purchases.<br />

* New customers who gain entry into <strong>Club</strong> <strong>Siemens</strong> are those who have had a<br />

dormant purchase history for 12 months and/or have not been part of a <strong>Siemens</strong><br />

incentive program for at least 3 fiscal years.<br />
