<strong>The</strong> <strong>Wellness</strong> department now provides services<br />

via<br />

telephone, video visits and e-mail!<br />

Registered Dietitian, Yoga Instructor, Personal Trainers and<br />

<strong>Wellness</strong> Chef work together as an interdisciplinary team to help<br />

treat and prevent chronic diseases as well as improve quality of<br />

well-being.


How to Schedule a <strong>Wellness</strong>Appointment<br />

Online Services<br />

If a member would like to use one of our online services, please contact the corresponding<br />

wellness staff through skype, triage, or e-mail to coordinate appointment times (do not<br />

schedule without approval from <strong>Wellness</strong> staff member or Director of <strong>Wellness</strong>, Deanna)<br />

About:<br />

Medical Nutrition <strong>The</strong>rapy, often abbreviated as MNT, is an evidence-based medical approach to treating<br />

certain chronic conditions through the use of an individually- tailored nutrition plan. This nutrition plan is<br />

ordered and approved by a primary care physician and implemented by a Registered Dietitian.<br />

Why do I need Medical Nutrition<strong>The</strong>rapy?<br />

Diet is an important part of everything we do. Food gives our bodies and our minds the fuel to think clearly and<br />

execute daily tasks. Everyone should see a dietitian every year, in the same way that you see the dentist or general<br />

practitioner every year. Diet affects every facet of our lives, from how we look, to how we feel, to how we sleep, to<br />

how we move, and even to how we think. With diet affecting our lives in so many ways, it’s important to have regular<br />

check-ups and interventions with a dietitian to assure that your diet is in good condition in the same way that you<br />

have check-ups and interventions with a doctor to make sure your body is in good condition. MNT can play a lifechanging<br />

role in reducing complications associated with certain chronic conditions, such and Type 2 Diabetes and<br />

Chronic Kidney Disease (CKD).Our dietitians use the Academy of Nutrition and Dietetics’ Evidence-Based Nutrition<br />

Practice <strong>Guide</strong>lines to counsel patients on lifestyle changes and self- management skills.<br />

Nutrition Coaching/Consultation<br />

New members must first schedule an appointment with <strong>Wellness</strong> Chef Martin to complete their first set<br />

of documents called, “Initial Nutrition Questionnaire” where food preferences and food frequency<br />

questionnaires are completed. This not only assists the Registered Dietitian with her consultation but<br />

also promotes awareness <strong>for</strong> the member of their current dietary issues. This document also indicates<br />

the members’ health patterns and behavior prior to <strong>Wellness</strong>. After this document is completed in<br />

Medent, Martin will then review USDA’s Choose My Plate, schedule nutrition appointment over the<br />

phone, and recommend the member to log dietary intake in a food journal <strong>for</strong> upcoming telephone<br />

appointment.<br />

<strong>The</strong> Registered Dietitian will review members’ medications, conditions, vitals, and labs to provide<br />

individualized medical nutrition therapy. A nutrition note/assessment is completed <strong>for</strong> each initial and<br />

follow up appointment in Medent where goals are developed and progress is noted. Follow ups are<br />

scheduled as needed-weekly or biweekly.<br />

After nutrition appointment, the Registered Dietitian will triage one of the Personal Trainers, either<br />

Danette or Andre, to create an individualized exercise plan as well as triage <strong>Wellness</strong> chef, Martin to<br />

create meals/distribute recipes based on given nutrition recommendations and members’ food


Recipe Assistance & Development<br />

OBJECTIVE: Continue promoting healthy cooking and eating habits of GBUAHN members<br />

through the setting of achievable goals.<br />

PLAN: Members work with the Chef to add new dishes to their diets based on the Dietitians<br />

recommendations and goals<br />

PARTICIPATION: Members were chosen to participate in this program based on referrals to the<br />

<strong>Wellness</strong> department and member outreach. Members are contacted by the Chef via telephone to<br />

personalize the Dietitians recommendations based on their personal food preferences and cooking<br />

abilities. Members are also encouraged to use the available on line resources such as the live sessions<br />

and cooking videos on the GBUAHN Facebook page or the recipes available on the GBUAHN<br />

Instagram page.<br />

FOLLOW UP: Contact via telephone with members bi-weekly checking on their successes,<br />

failures and attempts. At the same time assistance and encouragement is given along with<br />

practical advice in the <strong>for</strong>m of emails containing recipes or suggestions on how to go about<br />

changing their eating habits. This in<strong>for</strong>mation is tracked in the Well Recipe document in Medent.<br />

EVALUATION: Members are encouraged to provide pictures or videos of their finished recipes to<br />

be evaluated and to give feedback to members. Every six months members are given a follow up<br />

assessment to evaluate how they have improved their nutrition/cooking knowledge, behavior,<br />

skills, and self-efficacy.


FOLLOW UP: Follow up is done over the phone weekly to monitor<br />

progress and success of work outs.<br />

Personal Training/HealthCoaching<br />

OBJECTIVE: To promote physical fitness through coordination and<br />

distribution of weekly at home work outs plans to members. Also to<br />

access members needs and limitations to come up with an individualized<br />

plan to assess, plan, and monitor weekly goals.<br />

PLAN: To keep members motivated, active, and physically and mentally happy<br />

while at home. Recordings of workout videos to demontrate proper <strong>for</strong>m and<br />

challenge members. <strong>The</strong>se video include cardio based, seated, in<strong>for</strong>mation,<br />

and Yoga. <strong>The</strong>se workouts will challenge members and push them to stay<br />

moving. Member can access videos through Facebook, Youtube, Vimeo,<br />

Gbuahn app, and Email.<br />

PARTICIPATION: Participants are chosen based on regular attendance of gym<br />

and wellness center and accessibility to email. However, all members have been<br />

contacted through wellness or phns through promotion of online services.<br />

Registered Dietitian also triages personal trainers <strong>for</strong> interdisciplinary work.<br />

TRACKING: Personal trainer track members progress by filling out<br />

Fitness Trackers after every workout.

Smokers’ Quitline<br />

OBJECTIVE: Assist in the smoking cessation of members to improve overall health<br />

PLAN: Referral members to NYStateSmokers’ Quitline<br />

• <strong>The</strong> New York State Smokers’ Quitline (NYSSQL) is a service of the New York State<br />

<strong>Department</strong> of Health Tobacco Control Program and based at Roswell Park Comprehensive<br />

Cancer Center in Buffalo, N.Y. It is a free and confidential program providing evidencebased<br />

services to New York State residents who want to stop smoking or using other <strong>for</strong>ms<br />

of tobacco. A program with experience, care, and expertise.
