<strong>Maple</strong> <strong>Grove</strong> <strong>High</strong> <strong>School</strong> <strong>Athletic</strong> <strong>Training</strong> & <strong>Sports</strong> <strong>Medicine</strong> Services<br />

<strong>Athletic</strong> training services are provided to <strong>Maple</strong> <strong>Grove</strong> <strong>High</strong> <strong>School</strong> through the Institute<br />

for <strong>Athletic</strong> <strong>Medicine</strong> and Fairview Health Services.<br />

All student athletes may utilize the services that are offered by the Institute for <strong>Athletic</strong><br />

<strong>Medicine</strong> and <strong>Maple</strong> <strong>Grove</strong> <strong>High</strong> <strong>School</strong> once the appropriate consent to treat form has<br />

been signed by a parent or guardian. These forms are provided during sports registration.<br />

The athletic trainer is able to provide the following services:<br />

� Injury / Illness prevention and wellness protection including taping, wrapping<br />

and movement analysis and exercise prescription<br />

� Clinical evaluation and diagnosis<br />

� Immediate and emergency care<br />

� Treatment and rehabilitation<br />

� Organizational and professional health and wellbeing<br />

Every effort is made to provide athletic training services at most scheduled home JV and<br />

Varsity events. It is my goal to contact parents / guardians in the event of an injury to<br />

discuss treatment options and return to play decisions.<br />

Concussion Information: In the event your son or daughter were to sustain a<br />

concussion, the athletic trainer will work carefully and diligently to manage that<br />

injury appropriately and direct you to the appropriate medical care provider(s).<br />

<strong>Maple</strong> <strong>Grove</strong> <strong>High</strong> <strong>School</strong> will follow the MSHSL Return to Play Policy on caring<br />

for a student athlete with a concussion:<br />

Baseline neuropsychological testing is available and offered through Fairview<br />

<strong>Sports</strong> and Orthopedic Care (FSOC). Click here for more information on ImPACT<br />

Doctor/Therapy Information: If a serious injury occurs and you must seek further<br />

medical assistance, the athlete must present a doctor’s note to clear the athlete<br />

to resume play before they can return to practice or games. Here are some<br />

helpful in providing efficient services to your student athletes:<br />

What is an <strong>Athletic</strong> Trainer?<br />

<strong>Athletic</strong> training is practiced by athletic<br />

trainers (AT), health care professionals<br />

who collaborate with physicians to<br />

optimize activity and participation of<br />

patients and clients across age and care<br />

continuums. <strong>Athletic</strong> training<br />

encompasses the prevention, diagnosis,<br />

and intervention of emergency, acute<br />

and chronic medical conditions<br />

involving impairment, functional<br />
