Achieving effective implementation of interprofessional care requires a comprehensive action plan.
Achieving effective implementation of
interprofessional care requires a plan that identifies
the roles each partner or participant should
undertake. The plan developed in this Blueprint is
a starting point and will need to be updated and
refined as the key parties assume responsibility for
its implementation.
Tables 4.1 to 4.4 outline a phased approach
to engaging, developing, implementing and
evaluating the proposed interprofessional care
implementation activities over the short, medium
and long term. Some of these activities have
already been initiated and are at various stages of
implementation.
