Meeting opened at 7:05 pm with a quorum present and webcasting in progress. Chairman Gardiner<br />

welcomed all in attendance and led the group in the Pledge of Allegiance, followed by a moment of<br />

silence for those departed, in defense of our country, and in our service. In particular, Walter Horn, an<br />

active volunteer EMS provider with the Flanders-Northampton VAC, who has answered his last alarm.<br />

As is customary, all <strong>REMSCO</strong> members and guests introduced themselves.<br />

Mr. Gardiner opened the <strong>meeting</strong> calling for a motion to approve the July 2012 Meeting Summary as<br />

distributed. No discussion, motion passed.<br />

The Chair referenced an email he received from a constituent in the Terryville Fire District (TDF). The<br />

constituent has cerebral palsey and other medical complications and is a frequent user of the TFD‟s<br />

Ambulance Service. The constituent wrote a heartfelt email praising the services of the TFD,<br />

acknowledging their always-professional response and caring demeanor. Mr. Gardiner passed along the<br />

email, with a letter of thanks to the TFD Chief for their exemplary customer service.<br />

Mr. Gardiner then reported that he had the pleasure of presenting with Ed Stapleton at the AHA‟s ECC<br />

Conference on CPR, drawing parallels to the <strong>REMSCO</strong>‟s current initiatives on promoting high-quality<br />

CPR.<br />

Mr. Gardiner reminded all <strong>REMSCO</strong> members that the annual Awards Ceremony & Dinner is scheduled<br />

for October 25 th , that Mrs. Komosinski has already sent out invitations, and encouraged active<br />

participation by <strong>REMSCO</strong> members to recognize those being honored.<br />

Mr. Gardiner continued by directing the Nominations Committee to review all current seats,<br />

membership terms and constituencies represented to ensure that we have active and efficient<br />

representation from key stakeholders, particularly those that are representative of ambulance services, in<br />

accordance with required <strong>REMSCO</strong> make-up.<br />

Mr. Gardiner concluded by referencing a letter that he received from the NY State DOH to the SCPD<br />

terminating their Course Sponsor Agreement based on a new and recent investigation into practices at<br />

the academy‟s EMT Unit, and cancelling ten (10) referesher courses previously approved. He stated that<br />

Mr. Delagi would report on how the EMS Division has assumed administrative oversight of the training<br />

during the staff report. Mr Gardiner stated that the while he had not seen the results of the investigation,<br />

it would be inappropriate to debate the merits of the State DOH‟s action until such time as those results<br />

were made public. He then yielded the floor to Dr. Coyne to offer brief remarks on behalf of the SCPD.<br />

Dr. Coyne explained that the Police Commsioner did receive the letter that contained no specific<br />

allegations of findings of any investigation. He stated that SCPD administration has had productive talks<br />

with the State DOH and stated that all previously scheduled refresher training will continue under the<br />

leadership of <strong>Suffolk</strong> County EMS Division. He concluded by stating that the police department was<br />

waiting for information from State to identify and understand the issues that transpired leading to the<br />

letter.<br />

Vice-Chairperson – Jane Mccormack, RN<br />

Mrs. McCormack informed the <strong>REMSCO</strong> that Dr. Harry Soroff, founder of the Burn Center at Stony<br />

Brook, and a world renowned Burn Surgeon, recently passed away. She spoke of Dr. Soroff‟s<br />

significant contributions to burn care and his vision leading to the creation of the <strong>Suffolk</strong> County<br />

Firefighters Burn Center.<br />

Continued by reminding the <strong>REMSCO</strong> that the “Night at the Vineyards” fund raiser for the Burn Center<br />

will be held on October 6, tickest are available.<br />

In Mr. Mirras‟ absence, Mr. Delagi read the report into toe record, citing the current balance and<br />

referencing all bills paid and up-to-date. He also stated that the First Quarter reimbursement check was<br />

received, but not yet deposited and reflected in the totals. The report is provided as attachment 2. The<br />

Chair called for a motion that was seconded to approve the report as distributed.<br />

EMS Division Staff Report – R. Delagi, T. Lateulere, G. Pigott, MD<br />

Referring to Mr. Gardiner‟s opening remarks regarding support the SCPD‟s EMT Training Unit, Mr.<br />

Delagi stated that Police Commissioner Webber did reach out to Health Commissioner Tomarken<br />

advising the Health Department of the letter, and requested the Health Department‟s assistance in<br />

providing administrative oversight of EMT training through the EMS Division‟s Course Sponsorship,<br />

for the forseeable future, to ensure un-interrupted training of police officers while the police department<br />

has an opportunity to work with the State DOH to understand the issues and determine possibility of<br />

alternative conclusions they seek. Mr. Delagi stated that several <strong>meeting</strong>s have taken place between<br />

Health Department Administration, Police Department Administration, and Academy Administration<br />

and the Division has assumed oversight of courses, with current academy training staff working under<br />

Tom Lateulere, serving as the Course Sponsor Administrator and Dr. Pigott, serving as the Medical<br />

Director, and that this relationship would continue for the forseeable future, as the police receive<br />

answers to their questions regarding the investigation and subsequent actions taken.<br />

Staff report was distributed, as attachment 3. Mr. Delagi highlighted the recent BLS Narcan Pilot<br />

Demonstration Project activity, citing a current total of nine hundred two (902) providers trained and<br />

twenty three (23) cases of INH Narcan being administered by BLS providers to date.<br />

Mr. Lateulere reported on the changes to original, refresher and CME training currently underway in the<br />

region, based on NY State transition to the National Standard Education Curricula. He stressed the point<br />

that these changes are not the doing of the EMS Division, but rather, being implemented as required by<br />

NY State. We are releasing information to keep the system informed as we receive it from NY State and<br />

spending a great deal of time updating course curricula, presentations, student manuals, skills sheets, etc<br />

to meet the new requirements. Regarding the changes to CME, <strong>summar</strong>y information has been placd on<br />

the website and is also available on the State EMS website. Plans are in place to hold informational<br />

sessions for agency CME Liaisons.<br />

Dr. Pigott commented on the continuous quality reviewes done by the Division, yielding positive results.<br />

There were several issues that required technician remediation; however, system performance is at<br />

acceptable levels, particularly in invasive skills. Protocols up for review reflect advances in system<br />

performance and the Division‟s monitoring in QI program.<br />

Committee Reports<br />

Resource Needs – B. Blower<br />

Mr. Blower stated that the Resource Needs Committee met earlier this evening to address the<br />

Clarification of Operating Territory (COT) request concerning the East Moriches VAC. He read the<br />

Summary, provided as attachment 4 into the record, described the detailed review of coroborrating<br />

documentation, and brought forth the following seconded motion: The <strong>Suffolk</strong> <strong>REMSCO</strong> concurs that<br />

the most accurate representation of the East Moriches VAC’s authorized operating territory should be<br />

changed to: East Moriches Fire District and the Brookhaven Town Portion of the Eastport Fire District<br />

as detalied in the Clarification of Operating Territory request. There was no opposition. The<br />

President/Chief and the Vice President/Assistanct Chief were on hand to answer any questions. Motion<br />

unanamously passed.<br />

Response Committee – B. Blower<br />

Mr. Blower reported that the committee was working on the 2008 NY State Data CD, with the<br />

assistance of Jane McCormack and the data review staff at University Hospital. The CD contains<br />

188,000 calls, and some of the data appears suspect. Mr. Blower stated that he had been in contact with<br />

FRES Commissioner Williams to obtain FRES PSAP data so comparisons could me made and a report<br />

generated. FRES has agreed to suppy whatever data is available to the <strong>REMSCO</strong>.<br />

REMAC – L. Cox, MD<br />

Dr. Cox reported that the REMAC last met on July 24 th , and the <strong>meeting</strong> Summary was distributed. The<br />

main focus of the <strong>meeting</strong> was to finalize and approve the revised ALS protocols, which have been<br />

submitted to the SEMAC for review at their upcoming <strong>meeting</strong>. Dr. Cox stated that the revised protocols<br />

were very progressive with significant trust given to ALS providers in terms of expanded standing<br />

orders, reflection of new medications and procedures, and improves the standard of care in the region.<br />

He acknowledged that this is a by-product of the creativity and self-improvement seen in many agencies<br />

and by many technicians.<br />

Communications – T. Lateulere<br />

Mr. Lateulere reported that he met with the webmaster earlier in the day to discuss website<br />

improvements and enhancements, such as moderated interactive forums, and creating space for a<br />

collection of „how-to” videos for EMS providers. Work continues daily to keep the site updated and the<br />

site continues to be very popular and helpful to providers and agency leaders.<br />

PIO – E. Komosinski<br />

Mrs. Komosinski reminded all that the Awards ceremony and Dinner is October 25 th at Peconic<br />

Landing in Greenport, and that RSVPs were due on or before October 12 th . Thus far, the response is<br />

low.<br />

Disaster Planning – R. Delagi<br />

Mr. Delagi reported that disaster preparedness activity is ongoing and is documented in the EMS<br />

Division staff report. He highlighted the bullets describing on-going health care facility evacuation and<br />

how that has paid off, first with the facility evacuations last June with Hurricane/Tropical Storm Irene,<br />

and more recently last week with an emergency evacuation and subsequent repatriation of the Cedar<br />

Lodge Nursing Home in Center Moriches, due to a gas leak. He also thanked Joel Vetter from FRES for<br />

his work on finalizing the purchase process for the Medical Ambulance Bus, which is being constructed<br />

by Sartin Services, with expected delivery soon.<br />

Education & Training –<br />

In Mr. Zuksosky‟s absence, Mr. Gardiner reported that there was no new business before the committee<br />

and that Mr. Zukosky is up to date with matters before the committee.<br />

Legislative – E. Boyd<br />

Mr. Boyd reported that there is little legislative movement as we are in campaign season. Legislator<br />

Calarco commented that the budget process was underway in the county.<br />

Budget –<br />

In Mr. Mirras‟ absence, Mr. Smyth reported that the Audit Committee is underway, and that Mr. Mirras<br />

and Mr. Franz were going through records. There is one more <strong>meeting</strong> scheduled and a final report will<br />

be ready for the next <strong>meeting</strong>.<br />

Nominations – H. Rosenblum<br />

Ms. Rosenblum reported that there was no business before the Nominations Committee.<br />

Ad Hoc Committee on Recruitment –<br />

In Mr. Cronogue‟s absence, Mr. Gardiner reported that the survey prepared by the committee was<br />

distributed and results are pending.<br />

Ad Hoc Committee on Citizen CPR – E. Stapleton<br />

Mr. Stapleton reported that there was no committee <strong>meeting</strong> since the last <strong>REMSCO</strong> <strong>meeting</strong>. He<br />

commented on University Hospital‟s ongoing study using mass media video to successfully teach CPR<br />

and offered comments on how this be translated to use in the public arena. Mr. Stapleton continued,<br />

advising the <strong>REMSCO</strong> that he met with the President of Laerdal Corporation, who is developing a CPR<br />

Kiosk, which contains a mannequin and a video to mediate “self-learning” of CPR. The device is being<br />

field tested in the Dallas-Fort Worth Airport and plans are in place to have University Hospital as a test<br />

site.<br />

Co-chair Mr. Vetter reported that there are multiple agencies in the system doing regular and ongoing<br />

CPR training, and that the county‟s website is undergoing revisions, including a place to feature CPR<br />

training information and success stories. Mr. Vetter also discussed the National AED Registry Program,<br />

which he expected to roll out in the next two (2) months. This program supports the EMS Division‟s<br />

annual re-registration of AEDs in public places by using technology to communicate with PAD<br />

Organizations regularly for updates on AED status, location, in-date pads, etc. He concluded by<br />

discussing a new iPhone app linked to this initiative.<br />

Ad Hoc Committee on Recruitment & Retention –<br />

In Mr. Cronogue‟s absence, Mr. Gardiner reported that satisfaction surveys were sent out, results<br />

pending.<br />

NYS EMS Council Delegate – T. Lateulere<br />

Old Business<br />

Congratulations to Mr. Lateulere as he assumes official duties as the <strong>REMSCO</strong>‟s Delegate, successfully<br />

completing the Governor‟s vetting process. Mr. Delagi will remain his alternate. There was no <strong>meeting</strong><br />

since the May <strong>meeting</strong>, Mr. Lateulere and Mr. Delagi will be attending the October 2-4 <strong>meeting</strong>s.<br />

New Business<br />

Mr. Lateulere stated that he spoke to the vendor (Jones & Bartlett) regarding concerns raised last month<br />

by Mr. Cronogue, who was concerned about the validity of this vendor‟s test bank, and contacted Mr.<br />

Cronogue, asking to provide specific information as to which questions are poor questions and why.<br />

Mr. Lateulere also spoke to the author of the test bank who has asked for specifics about the questions<br />

being challenged so that he can review and respond to the concerns.<br />

Mr. Gardiner asked for the sense of the <strong>REMSCO</strong> on his thoughts of adding a campaign bar, to be<br />

issued with certificates, for CPR saves. Recognizing that most agencies have by-laws that address<br />

uniform components, he felt that provider would appreciate them, not only for uniforms, but also for<br />

non-uniform attire, such as jacktets. There was overwhelming support for the idea.<br />

EMS DIVISION STAFF REPORT<br />

AMBULANCE SERVICE OPERATIONS<br />

Provided staff support to the <strong>REMSCO</strong> Executive Committee.<br />

Provided staff support to the <strong>REMSCO</strong> for their July, 2012 <strong>meeting</strong>.<br />

Provided staff support to the REMAC for their July, 2012 <strong>meeting</strong>.<br />

In follow up to previous <strong>meeting</strong> with FRES Dispatch to submit proposal to NAEMD to modify select emergency<br />

medical dispatch determinant code for cases of chemical suicide, held one (1) additional discussion and reviewed<br />

dispatch determinants in an upcoming version 12.2, to be released soon, continuing to work to address the<br />

problems.<br />

Investigated a complaint regarding “unsafe atmospheric conditions” in a day care center, working in conjunction<br />

with Town of Islip Haz Mat Team and the Health Commissioner to address a public health concern. Issue was<br />

unfounded.<br />

Staff met with the new Executive Director of Administration and Associate Executive Director of Patient Care<br />

Services at Southside Hospital to discuss enhanced hospital services and implications on the EMS System.<br />

Completed the 2013-2014 Regional Training Plan and Budget Template, submitted to the NY State DOH.<br />

Distributed information regarding the NY State EMS Educational Conference and Trade Show – Vital Signs 2012<br />

– to the EMS System. The conference will be held in Syracuse, NY 10/18/12 – 10/21/12.<br />

Consulted with FRES on the potential health effects on firefighters at a recent fire in a recycling plant.<br />

Met with representatives of the Community Ambulance Company and the Bayport Fire Department/District on<br />

unauthorized EMS response and steps to become a recognized BLS-FR agency.<br />

Met with repreentatives of the Sayville FD to address appropriate levels of emergency medical care from<br />

members of a fire department without EMS agency designation.<br />

Provided on-site technical support and developed the health & safety plan for the annual Cross Bay Swim.<br />

Provided on-site technical support in coordinating services at the Wounded Warrior Project ride in Amagansett.<br />

Established a working relationship with the East Moriches VAC to assist with a Clarification of Operating<br />

Territory (COT) proceeding. Attended several mettings with agency representatives and FRES Communications<br />

to address formilization of geographical boundaries.<br />

Provided technical support to the <strong>Suffolk</strong> County Office of the Aging to assure EMS coverage and cooling<br />

stations at a senior citizens picnic.<br />

Participated in several planning <strong>meeting</strong>s and conference calls reference a Part 18 application for the Great South<br />

Bay Power Boat Races. The event was not permitted as the promotor could not overcome concerns addressed in<br />

the Public Safety Impact Statement.<br />

Provided updated “Appendix B” Budget templates for the Program Agency contract for 2013-2014, and 2014-<br />

2015.<br />

Provided notification to the EMS System about the PCR shortage, as reported by NY State DOH, along with<br />

temporary solutions for duplicating PCRs in the interim. The PCR shortage has since been resolved, and<br />

distribution sites across <strong>Suffolk</strong> County have received new alotments.<br />

Linked to the initiative described above, the EMS Division has partnered with Brookhaven Memorial Hospital as<br />

out newest distribution site.<br />

Provided assistance to the HCFAS as they developed an Emergency Medical Action Plan (EMAP) pursuant to a<br />

Part 18 application for the Long Island Fall Festival.<br />

Staff provided on-site technical support at a house explosion in Brentwood with multiple casualties.<br />

Provided on-site inspection at a Part 18 permit funtion, the Hampton Classic Horse Show, in Bridgehampton.<br />

Staff maintained admintrative oversight of the county’s BLS Narcan program, reviewing PCRs and processing<br />

training records.<br />

Attended informational <strong>meeting</strong> with HCFAS for entry into the ePCR Program.<br />

Held two (2) <strong>meeting</strong>s with event organizer and public safety agencies affected by Part 18 event, a fireworks<br />

display in Patchogue.<br />

Held two (2) <strong>meeting</strong>s with event organizer and public safety agencies affected by Part 18 event, the Annual<br />

Seafood Festival at the LI Maritime Museum in West Sayville.<br />

Attended two (2) <strong>meeting</strong>s with event organizer and public safety agencies affected by Part 18 event, the Central<br />

American & Mexico Independence Festival, in Brentwood.<br />

Provided initial Controlled Substance informational package to the Ridge Fire Department<br />

Provided logistical support and coordinated EMS resources for the Annual Smith Point Triathlon<br />

Provided initial Controlled Substance informational package to the Ridge Fire Department<br />

Conducted site visit at North Amityville FD, as they upgrade to the ALS level.<br />

Reviewed final submission of Mastic Beach Controlled Substances Plan.<br />

Attended Safety & Logistics <strong>meeting</strong> with the National Multiple Sclerosis Society for the up-coming MS Bike<br />

event, to be held in September.<br />

Provided final review and notifications as Bay Shore-Brightwaters Rescue Ambulance implements their<br />

Controlled Substances Program.<br />

Staff continued with planning and coordinating the annual awards dinner and ceremony.<br />

CISM Team responded to twenty three (23) calls for assistance.<br />

Staff processed multiple PAD applications or notifications of changes to existing PAD programs.<br />

Staff attended one (1) <strong>meeting</strong> of the county’s Recruitment and Retention Committee, and the Division notified<br />

the EMS System that recently enacted legislation now recognizes student/volunteers from VACs as eligible.<br />

Continue working with FRES and Medical Control to identify instances where the police are calling EMS for a<br />

first responder to “pronounce death.”<br />

EDUCATION, TRAINING & QUALITY IMPROVEMENT<br />

The EMS Division regrets to announce that EMS Officer Peter Aurrichio has resigned from the division to pursue<br />

professional opportunities with the FDNY’s EMS Command. We wish Peter well in his new endeavors, and<br />

recognize his significant contributions to the EMS Division’s educational programs. We look forward to Peter<br />

staying on with us as a per diem contract instructor. Adminsitrative staff continue to work on ensuring<br />

uninterrupted opportunities during staffing shortages.<br />

Staff spent considerable time finalizing revised ALS protocols, in preparation for distribution to NY SEMAC, for<br />

approval at the October <strong>meeting</strong>.<br />

Staff spent considerable time on revising original, refresher, and CME training requirements, educational content<br />

packages, student manuals, academic policies, and presentations to conform to NY State guidelines for transition<br />

to the National Standard curricula.<br />

Administrative staff working with the SCPD Academy to transition Course Sponsorship management to the EMS<br />

Division, based on sanctions recently imposed on them by the NY State DOH.<br />

Received the results from the June 2012 NY State exam; sixty seven (67) EMT-B Original, forty three (43) EMT-<br />

B Refresher and ten (10) EMT-P Refresher students passed.<br />

Held two (2) NYS PSEs for EMT-CC students, 42/44 passed the 16 station exam.<br />

Held one (1) NYS PSE for EMT-B students, 44/44 students passed.<br />

Held one (1) NYS PSE for CFR students, 6/6 passed.<br />

Reviewed and processed seventy two (72) CME Refresher Packets (25 EMT-B, 39 EMT-CC and 8 EMT-P) for<br />

students recertifying through the CME Program.<br />

Staff performed audits on two (2) CIC canditates.<br />

Staff tracking two thousand nine hundred sixty five (2,965) providers enrolled in the CME Program.<br />

The AHA Training Center conducted three hundred thirty one (331) courses, collectively registering one thousand<br />

seven hundred ninety five (1,795) students.<br />

Conducted one (1) REMAC ALS Credentialing class for fourteen (14) providers.<br />

Division staff currently developing the following courses:<br />

two (2) EMT-CC Original courses in Hauppauge and Riverhead;<br />

one (1) EMT-CC Refresher course in Hauppauge;<br />

five (5) EMT-B Original course in Centereach, Copaigue, Miller Place, East Northport and Sag Harbor;<br />

six (6) EMT-B Refresher courses in Huntington, Bay Shore, Sag Harbor, Nesconset, Southold, and<br />

Hauppauge.<br />

Staff conducted the following CME class offerings:<br />

one (1) CPAP class (11 students);<br />

one (1) PCR Writing class (12 students);<br />

one (1) AHA BLS Instructor course (21 students);<br />

two (2) CPR Instructor renewal course (37 students)<br />

eight (8) ALS CME Core Content classes (160 students); and<br />

seven (7) BLS CME Core Content classes (160 students).<br />

Division staff performed Quality Improvement reviews of the following cases:<br />

one hundred twenty (120) cases of endotracheal intubation;<br />

twenty eight (28) cases where Medical Control MD contacted for hospital destination/where<br />

Medical Control MD divert patient from closest hospital to “appropriate” hospital;<br />

one hundred fifty (150) cardiac arrests (25 sustained PROSC - 4 d/c home);<br />

fifty two (52) cases of IO application;<br />

one (1) needle decompression;<br />

fifty nine (59) cases of controlled substance administration;<br />

nine (9) cases of BLS Albuterol administration;<br />

thirteen (13) uses of chemical restraint medication administration;<br />

thirteen (13) cases of BLS Narcan administration;<br />

three (3) cases of BLS Blood glucoose determination<br />

two (2) cases of BLS Epi Pen Authinjector administration; and<br />

thirty seven (37) CPAP applications.<br />

Worked with Service Medical Directors in reviewing thirty two (32) case reviews.<br />

Worked with the Medical Director on multiple remediations, including restriction of ALS privileges for three (3)<br />

providers due to significant protocol violations.

Participated in a telephone conference call with NY State DOH regarding health care facilities evacuations.<br />

Staff attended a <strong>meeting</strong> on Medical Countermeasures Capacity for Improvised Nuclear Device (IND) hosted by<br />

the federal Assistant Secretary for Prepardeness and Response (ASPR) working with the Regional Catastrophic<br />

Planning Team.<br />

Attended training co-sponsored by Columbia University and Stony Brook Medicine for Maintaining Efficiencies<br />

in Public Health.<br />

Attended one (1) <strong>meeting</strong> of the county’s Incident Management Assistance Team (IMAT).<br />

Staff attended the inagural <strong>meeting</strong> of the State DOH’s newly formed Health Care Emergency Preparation<br />

Coalition. This entity replaces the Regional Resource Center (RRC) as the point of coordination for health care<br />

facilities planning.<br />

Provided EMS planning and recon in preparation for the county’s USAR Team deployment to an exercise at the<br />

Center for National Response Memorial Tunnel in West Virginia.<br />

Attened one (1) <strong>meeting</strong> of the county’s USAR Team.<br />

Met with the Riverhead Volunteer Ambulance Corps to assist with planning an MCI exercise.<br />

Attended a tabletop exercise in the Islip Public School District to discuss emergency planning.<br />

Attended one (1) <strong>meeting</strong> with the U.S. Postal Service and other key stakeholders to revise plans for response to<br />

the Biohazard Detection System (BDS).<br />

Progressed with purchasing specifications and Request for Proposal (RFP) process for Medical Ambulance Bus<br />

(MAB) purchase. Bid has been awarded to Sartin Services, vehicle being built to specification.<br />
