Case 2
Heron IOS Clinical Case
Dr. Tarek El Saeedy
Lecturer of oral and maxillofacial prosthodontics,
Partially edentulous patient with few remaining teeth was presented seeking definitive
rehabilitation of both arches. On examination the condition of the remaining teeth and
their prognosis was doomed to be extracted. And replaced with implant based restorations.
These bars although formed of printed resin
we used them as verification bars during
their seating, depending on the our clinical
experience and tactile sense of the passive
seating. Then each arch was scanned
separately by IOS.
Wax rims were added and adjusted
to the proper VDs and even contact
between each arch and the patient was
manipulated into centric relation. It was
secured to the bars by interlocking with
the occlusal projections
IOS was used to record the bite with the
buccal projections kept uncovered to aid
in virtual alignment of both arches in the
proper intermaxillary relation.
Superimposition between the upper
scans.
5 implants were placed in the upper arch
and all on 4 was done for the lower arch
without immediate loading.
MUAs were installed for both arches
implants and torqued for the final torque.
The Heron IOS was used to scan the
mucosa of the upper arch without the
scan bodies.
Superimposition between the lower
scans.
Virtual mounting of the upper and lower scans acc to the predetermined vertical relation.
Scan bodies were screwed in position on
the multiunit abutments.
Again the Heron IOS was used to scan the
scan bodies intraorally.
As for the lower arch scan bodies were
secured to the MUAs
Then screw retained full contour PMMA
was designed guided by the contours of
the wax rims and printed.
It was tried intraorally without
modifications.
Back again to the design process cut
backs were done to a thimble bar design
with separate crown.
Splinting the scan bodies was made
using flowable composite in different
directions to enhance the scanning
process.
IOS scanning was done for this arch.
A screw retained bar with multiple
occlusal and buccal projections was
designed and printed.
Milled Ti- frame and PMMA crowns were
verified intraorally.v
Pink Visiolign was applied to the gingival
part of Ti frame and Zr crown were
cemented to the thimbles. And the
restoration was delivered.
