Vitamins, minerals, and supplements<br />
◯● Use●iron●sucrose●and●SFGC●only●for●clients●who●are●undergoing●long-term●<br />
hemodialysis●and●are●deficient●in●iron.<br />
◯● Prevention●of●iron-deficiency●anemia●for●clients●who●are●at●an●increased●risk,●such●as●<br />
pregnant●women,●infants,●and●children.<br />
◯● Parenteral●forms●should●only●be●used●in●clients●who●are●unable●to●take●oral●<br />
medications.●IV●route●is●preferred.<br />
SIDE/ADVERSE EFFECTS NURSING INTERVENTIONS/CLIENT EDUC<strong>ATI</strong>ON<br />
Gi distress (nausea, constipation, heartburn) • if intolerable, administer medication with<br />
food, but this greatly reduces absorption.<br />
• may need to reduce dosage.<br />
• monitor the client’s bowel pattern and<br />
intervene as appropriate. advise clients<br />
that these side effects usually resolve with<br />
continued use.<br />
teeth staining (liquid form) • teach clients to dilute liquid iron with<br />
water or juice, drink with a straw, and rinse<br />
mouth after swallowing.<br />

• Give im doses deep im using Z-track<br />
technique.<br />
• avoid this route if possible.<br />
anaphylaxis risk with parenteral administration • monitor clients who are receiving iV iron<br />
preparations.<br />

Fatal iron toxicity in children can occur when<br />
an overdose of iron (2 to 10 g) is ingested<br />
(severe Gi symptoms, shock, acidosis, liver and<br />
heart failure).<br />
Contraindications/precautions<br />
●● Contraindicated●for●clients●with:●<br />
◯● Previous●hypersensitivity●to●iron<br />
• monitor vital signs when administering<br />
parenteral iron.<br />
• use the chelating agent, deferoxamine<br />
(desferal), to treat toxicity.<br />
◯● Hemolytic●anemia,●peptic●ulcer●disease,●and●severe●liver●disease<br />
