isolation - two major health threats for those with disabilities.

Seven Hills' Shared Living Providers in Massachusetts and Rhode Island welcome adults

with developmental disabilities into their homes to live as part of a family environment. All

providers have been carefully trained and screened in order to deliver only top-quality

care services.

Horizon House

Horizon House provides housing options tailored specifically for individuals with

intellectual disabilities. These options range from community residential living in small

group homes that provide 24-hour in-home support; semi-independent living

arrangements or supported living options for those needing less 24-hour assistance; life

sharing where individuals live together while still receiving necessary levels of assistance

- these arrangements aim to maximize independence while simultaneously encouraging

community integration and participation.

Horizon works closely with local service providers, organizations and businesses to

connect residents of affordable housing with resources in their community - including

medical and behavioral health care, employment supports and peer support services as

well as social activities and volunteer opportunities.

Mission of REACH Inc: REACH Inc provides hope and opportunity to adults with

disabilities who have been forgotten by society, enabling them to lead healthy and fulfilling

lives by targeting root causes such as mental illness, substance abuse and

homelessness.

Horizon's Community Relations/Diversity Committee visited Solid Ground, a non-profit

that strives to end poverty and other issues that impact people from diverse backgrounds.

Members learned about its programs and services, as well as its diverse population who

reside there.

The Foundling

The New York Foundling is one of the city's oldest nonprofit organizations, offering foster

care services, educational opportunities, and assistance for people with developmental

disabilities. Recently it opened Vital Brookdale apartment building in Brownsville to offer

affordable housing to people aging out of foster care or having disabilities; community

habilitation specialists will assist residents in daily tasks - this facility will house 36 adults

according to The New York Foundling.

Since 1869, this organization has been saving children, strengthening families, and

building communities across New York City, Rockland County, Westchester County and

Puerto Rico. Today it serves five boroughs of New York City, Rockland County

Westchester County Puerto Rico with community programs including foster and adoption

services for children as well as nursery care programs and shelter services for unwed

mothers.

On opening day of this institution, Sister Irene placed a white wicker cradle near its door

and placed one baby there; during its inaugural month alone 45 more babies arrived for

adoption through this hospital's boarding department and placement with neighbors;

these notes can now be seen at New-York Historical Society.

Adults with disabilities often require assistance and healthcare from multiple sources,

including their living environment. At New York Foundling, a home sharing program exists

that matches adult "hosts" who have extra space in their home with adults who require

supportive living arrangements for adult clients with disabilities.

Community Interactions

Community Interactions (CI) is a non-profit organization providing person-centric housing

and services for people living with intellectual disabilities, autism spectrum disorder

(ASD), or dual diagnosed behavioral challenges. Founded by Gerald Saulino after

witnessing his brother living conditions within an institution; Saulino wanted him to

experience more fulfilling living conditions outside. Thus CI was established with less

restrictive programs, services, housing options in Pennsylvania and Delaware.

CSI's residential community homes provide staff support for individuals living in

apartments or single-family homes who require help with daily activities, navigating their

community and attending social or recreational events. Support can be offered a few

hours per week or 24-hours a day.

Other community-based arrangements include family living, or "life sharing", in which an

individual with disabilities lives in a supportive home environment with people other than

relatives. CSI operates group homes as another means for adults with disabilities to

maximize their potential and independence.

CSI's SHARE program matches homeowners with spare rooms with home seekers

looking for affordable housing options in exchange for help around the house or reduced

rent. Campaigns use animation and lighthearted scenarios to demonstrate how both

parties benefit from such arrangements; an animated home seeker who enjoys banana

bread and dogs may find an arrangement more fitting if paired with someone who bakes

cookies while tending their lawn gnomes!

HeartShare

HeartShare has long been at the forefront of creating inclusive communities for people

with disabilities. Their organization offers 38 supervised group homes and 42 independent

living apartments throughout Brooklyn, Queens and Staten Island as well as running day

habilitation programs for adults with disabilities and Transition Services that help families

prepare their loved ones for future challenges.

Heartshare is dedicated to empowering its clients, and one way it does this is through

training and employment programs. Darron was one of the first HeartShare-trained

employees at Cleaning with Meaning - an organic green cleaning service in New York

City and Lower Westchester that employs Darron for two years and counts him among

their happy employees.

As public awareness increased of conditions at Willowbrook State School in Staten

Island, HeartShare opened a group home designed to offer more family-like care. They

also established the Eileen and William Lavin Day Treatment Center in Williamsburg,

Brooklyn to meet the needs of people who had progressed beyond traditional residential

care.

HeartShare Wellness meets the rising need for health care coordination among children

and adults with intellectual and developmental disabilities. They provide comprehensive

health evaluation services such as speech and physical therapies, psychological and

psychosocial evaluations and sexuality assessments as well as sexuality tests.

Furthermore, HeartShare offers a Health Home Targeted Case Management Program

which connects patients to internal and external resources for their care.
