With my philosophy of m<strong>on</strong>itoring the wear of the prosthesis by measuring serum m<str<strong>on</strong>g>etal</str<strong>on</strong>g> i<strong>on</strong>s and being aware of the destructi<strong>on</strong> of soft tissues and b<strong>on</strong>e<br />

when they are exposed to abnormally high levels of wear products (Fig 22), I <strong>on</strong>ce again want to advise surge<strong>on</strong>s to revise malpositi<strong>on</strong>ed, badly d<strong>on</strong>e<br />

resurfacing prostheses quickly in order to avoid further complicati<strong>on</strong>s for the patient.<br />

osteolysis of the b<strong>on</strong>e and finally loosening of the femoral comp<strong>on</strong>ent. With routine m<str<strong>on</strong>g>etal</str<strong>on</strong>g> i<strong>on</strong> measurements and meticulous follow<br />

up, this case would have been recognized as having wear, and would have been revised earlier than 56 m<strong>on</strong>ths.<br />

The malpositi<strong>on</strong>ing and high wear of the <str<strong>on</strong>g>Birmingham</str<strong>on</strong>g> H<str<strong>on</strong>g>ip</str<strong>on</strong>g> <str<strong>on</strong>g>Resurfacing</str<strong>on</strong>g> result in part from design features such as the alpha angle<br />

and coverage and in part from the difficulty the surge<strong>on</strong> faces when placing the comp<strong>on</strong>ents correctly using their instrumentati<strong>on</strong>.<br />

By c<strong>on</strong>trast, with regard to more “wear resistant” design features (better coverage, less risk of edge loading even when implanted<br />

steeply), instruments that allow better c<strong>on</strong>trol over comp<strong>on</strong>ent positi<strong>on</strong> and the new innovati<strong>on</strong> of the “A-class” differential hardness<br />

material, the C<str<strong>on</strong>g><strong>on</strong>serv</str<strong>on</strong>g>e <str<strong>on</strong>g>Plus</str<strong>on</strong>g> has clear and established wear advantages.<br />
