Spring is in the air. With<br />

such a mild winter, it seems<br />

ridiculous to long for the<br />

warmer weather, but I do. I’m<br />

anxious to have the trees budding,<br />

flowers blooming and<br />

birds singing.<br />

The last few months our<br />

staff has been exhaustingly<br />

coordinating the 2nd Annual<br />

Head to Toe Women’s Expo.<br />

And what an amazing job<br />

they’ve done. (See Pages 40-41<br />

for more details). We’ll be giving<br />

away lots of door prizes;<br />

there’ll be food samples from<br />

local restaurants, massage<br />

sessions, health screenings, giveaways<br />

and so much more. With<br />

The specialty of<br />

women’s health care has<br />

experienced tremendous<br />

growth ever since the medical<br />

community began recognizing<br />

that women have<br />

different health care needs<br />

than men. This trend is<br />

expected to continue as the<br />

population’s life expectancy<br />

increases and the need for<br />

comprehensive health<br />

care for women beyond the<br />

reproductive years intensifies.<br />

As women’s health<br />

centers spring up around<br />

the country, the subspecialty<br />

of women’s imaging is destined<br />

to grow. Radiologist<br />

will play a critical role as<br />

the primary provider of<br />

women’s imaging services.<br />

The movement for<br />

better women’s health care<br />

was spurred by 1990 report<br />

that highlighted several<br />

inequities in women’s health<br />

care. As the government<br />

began supporting women’s<br />

health care issues, female<br />

health consumers also started<br />

demanding changes to<br />

the way they receive medical<br />

care including imaging.<br />

Public surveys found that<br />

women desire providers<br />

who can offer a full range of<br />

services as well as physicians<br />

who have specialty<br />

training in women’s<br />

health care. In addition,<br />

women stressed the importance<br />

Colonoscopy and sigmoidoscopy<br />

are excellent<br />

screening exams for colorectal<br />

cancer (commonly<br />

referred to as “colon cancer”).<br />

But false information<br />

and a misplaced sense of<br />

modesty have scared some<br />

people away from these lifesaving<br />

tests. Here are some<br />

questions you might have<br />

and some answers for them:<br />

WHAT IS A COLONOSCOPY?<br />

A colonoscopy is an<br />

exam that allows a doctor to<br />

see and closely look at the<br />

inside of the entire colon for<br />

signs of cancer or polyps.<br />

Polyps are small growths<br />

that over time can become<br />

cancer. The doctor uses a<br />

slender, flexible, hollow,<br />

lighted tube about the thickness<br />

of a finger. This<br />

“colonoscope” is gently<br />

eased inside the colon and<br />

has a tiny video camera,<br />

which sends pictures to a TV<br />

screen. Small puffs of air are<br />

put in the colon to keep it<br />

open and allow the doctor to<br />

see clearly.<br />

The exam itself takes 15<br />

to 30 minutes. Patients are<br />

usually given medicine to<br />

help them relax, which often<br />

puts them to sleep during the<br />

procedure. Your doctor<br />

decides how often you need<br />

this test, usually once every<br />

10 years, depending on your<br />

personal risk for colon cancer.<br />

It’s important for people<br />

to talk with their doctor to<br />

understand their personal<br />

risk for getting colon cancer,<br />

the guidelines they should<br />

follow for testing, and<br />

whether they need to start<br />

being tested at age 50 or earlier.<br />

WHAT IS A SIGMOIDOSCOPY?<br />

During a sigmoidoscopy,<br />

a doctor closely inspects the<br />

lower parts of the colon,<br />

called the sigmoid colon and<br />

the descending colon, for<br />

signs of cancer or polyps.<br />

require medicine to make the<br />

patient sleepy, so some<br />

patients find the air pressure<br />

to be uncomfortable. Air is<br />

pumped into the cleaned-out<br />

colon so it will hold its normal<br />

size and doctors can get<br />

the best pictures. While it<br />

may be slightly uncomfortable,<br />

it should not hurt.<br />

WHO WILL DO THE EXAM?<br />

A colonoscopy is almost<br />

always done by a doctor,<br />

usually a gastroenterologist<br />

or a surgeon. Sigmoidoscopies<br />

are often done by<br />

primary care doctors (general<br />

practitioners, family doctors,<br />

and internal medicine<br />

doctors) in their office.<br />

Studies show that a welltrained<br />

clinical nurse specialist,<br />

nurse practitioner, or<br />

physician’s assistant can do<br />

sigmoidoscopies very effectively<br />

too.<br />

WILL I BE IN A PRIVATE ROOM?<br />

Sigmoidoscopy is always<br />

done in a private room; no<br />

other patients are around.<br />

Doctors and nurse specialists<br />

are professional and very<br />

careful to respect the<br />

patient’s privacy.<br />

Colonoscopy is also performed<br />

in a private area; it<br />

may be done in a hospital<br />

outpatient department, a<br />

clinic, an ambulatory surgery<br />

center, or a doctor’s office.<br />

The patient’s privacy is a<br />

top concern.<br />

HOW DO I PREPARE? WILL<br />

I NEED TO MISS WORK?<br />

The preparation for the<br />

colonoscopy requires you to<br />

go the bathroom a lot! You<br />

follow a special diet the day<br />

before the exam and take<br />

very strong laxatives in the<br />

hours before the procedure.<br />

You may also need an enema<br />

to cleanse the colon. The<br />

key to getting good pictures<br />

is to have the colon cleaned<br />

out. Preparation for a sigmoidoscopy<br />

is much the same.<br />

Because colonoscopy is<br />

usually done under sedation,<br />

people usually will miss a day<br />

of work. People should ask<br />

their doctors whether they’ll<br />

need to miss work before a<br />

sigmoidoscopy. For either<br />

test you’ll need to stay close<br />

to a bathroom. You might<br />

want to schedule the proce-<br />

dure for a Monday, so you<br />

can be at home the day<br />

before without taking a day<br />

off work.<br />

HOW WILL I FEEL AFTER-<br />

WARD? WILL I NEED SOME-<br />

ONE TO DRIVE ME HOME?<br />

Most people feel OK<br />

after a colonoscopy. They<br />

may feel a bit woozy. They’ll<br />

be watched and given fluids<br />

after the procedure as they<br />

awaken from the anesthesia.<br />

They may have some gas,<br />

which could cause mild discomfort.<br />

Because of the sedation<br />

that is given for the test,<br />

most facilities ask that you<br />

bring someone to take<br />

you home.<br />

After a sigmoidoscopy,<br />

you get up and walk out.<br />

There should be no problem<br />

driving yourself home, as<br />

long as you have not had any<br />

drugs to make you sleepy<br />

during the test.<br />

WHAT IF THEY FIND<br />

SOMETHING?<br />

If a small polyp is found,<br />

your doctor will probably<br />

remove it because it could<br />

ous. If your doctor sees a<br />

large polyp, a tumor, or anything<br />

else abnormal, a biopsy<br />

will be done. For the biopsy,<br />

a small piece of tissue is<br />

taken out through the<br />

colonoscope or sigmoidoscope.<br />

It is sent to a lab to be<br />

checked under a microscope<br />

for cancerous or precancerous<br />

cells.<br />

WHY ARE THESE TESTS SO<br />

IMPORTANT?<br />

Removing polyps prevents<br />

colorectal cancer from<br />

ever starting. And cancers<br />

found in an early stage are<br />

more easily treated. Nine out<br />

of 10 people whose colon<br />

cancer is discovered early<br />

will be alive 5 years later.<br />

And many will live normal<br />

lifespans.<br />

But too often people<br />

don’t get these tests. Then<br />

the cancer can grow and<br />

spread unnoticed, like a<br />

silent invader. In many cases,<br />

by the time people have any<br />

symptoms the cancer is very<br />

advanced and very difficult<br />

to treat.<br />

Environmental allergy<br />

or “hay fever” is very common<br />

and affects about 1 in 5<br />

adults and 4 out of 10<br />

children. Symptoms include<br />

runny or clogged nose,<br />

sneezing, coughing with<br />

postnasal drip, and itchy,<br />

watery eyes. Persons with<br />

allergies report feeling<br />

“slower” and drowsy. Young<br />

children who can’t express<br />

their symptoms may<br />

demonstrate irritability,<br />

behavior problems, or<br />

reduced school performance.<br />

Environmental allergies<br />

also worsen conditions like<br />

asthma, sinus problems,<br />

cough, snoring, ear infection/fluid,<br />

and itchy rashes.<br />

When allergies are undertreated,<br />

persons with these<br />

conditions suffer more<br />

frequent and severe illness,<br />

miss more work/school, and<br />

need more medicines like<br />

antibiotics and steroids.<br />

While the treatment<br />

strategy for allergies<br />

appears simple, it is actually<br />

quite complicated to execute.<br />

The first step is identifying<br />

allergic triggers and<br />

avoiding them. Step two<br />

is adding medicines.<br />

When Treating Allergies, Know All Your Options<br />

allergen immunotherapy or<br />

“allergy shots”.<br />

Unfortunately, most people<br />

do not consider evaluation<br />

with an allergist until<br />

they believe they have<br />

failed steps one and two<br />

and are ready for<br />

immunotherapy. While<br />

there are patients whose<br />

allergies are easily treated,<br />

many others have more<br />

severe conditions caused by<br />

complicated factors not<br />

addressed during routine<br />

care. In fact, one reason<br />

allergies are so hard to treat<br />

is because no two people<br />

suffer in exactly the same<br />

way. Sadly, I often evaluate<br />

patients only after they<br />

suffer months or years<br />

switching between different<br />

medicines without relief.<br />

Many of these patients<br />

have also spent time and<br />

money on ineffective<br />

and expensive allergenavoidance<br />

plans.<br />

In our practice, my staff<br />

and I have the time and<br />

expertise to focus on<br />

neglected factors such as<br />

environmental irritants,<br />

work/school exposures and<br />

abnormal sinus function (to<br />

name a few). We are<br />

experts in providing practical,<br />

cost-effective and<br />

customized allergen avoidance<br />

advice. We are very<br />

familiar with conventional<br />

allergy medicines and have<br />

many unique strategies to<br />

maximize their effectiveness<br />

so that “less medicine<br />

We need to<br />

remember that our parents<br />

Family members become<br />

“survivors”, and they develop<br />

behaviors that help them<br />

to deny, ignore or avoid difficult<br />

problems or emotions.<br />

These family members<br />

become detached and<br />

fear confrontation. They<br />

experience issues with<br />

regard to trust and their<br />

development is inhibited.<br />

The family is focused on<br />

the family member “with<br />

the problem”. Family members<br />

who do not “have the<br />

problem” sacrifice their own<br />

needs in an effort to take<br />

care of the person who is<br />

“sick”. When family members<br />

place the “person with<br />

the problem” before their<br />

own needs, they often lose<br />

contact with their own desires,<br />

needs and sense of self.<br />

1. Fear of losing control.<br />

They need to maintain their<br />

feelings and behavior, and<br />

they try to control the feelings<br />

and behaviors of others.<br />

They are afraid that<br />

their lives will worsen if<br />

they relax their control.<br />

2. Fear of feelings: From<br />

childhood on they have<br />

buried their feelings (especially<br />

anger sadness) –<br />

eventually all intense<br />

feelings are feared – even<br />

joy and happiness.<br />

3. Fear of conflict: They<br />

fear people in authority and<br />

they fear angry people.<br />

They are always seeking<br />

approval.<br />

4. An overdeveloped<br />

sense of responsibility. They<br />

are hypersensitive to the<br />

needs of others. Their self<br />

esteem comes from how<br />

others view them. They<br />

need to be perfect.<br />

5. Feelings of guilt when<br />

they stand up for themselves<br />

rather than giving in to others.<br />

They sacrifice their own<br />

needs in an effort to<br />

be “responsible” and to<br />

avoid guilt.<br />

6. They have an inability<br />

to relax, let go, and have fun.<br />

They feel they don’t<br />

deserve to have have fun.<br />

They live in a world of<br />

denial, especially when<br />

they feel threatened.<br />

8. They have difficulties<br />

with intimate relationships.<br />

Intimacy leaves these individuals<br />

feeling out of control.<br />

Intimacy requires self<br />

love and being comfortable<br />

with the expression of one’s<br />

own needs. Often relationship<br />

patterns are repeated<br />

are often attracted to other<br />

10. Compulsive behavior.<br />

They may eat or drink<br />

or work compulsively or<br />

behave in other compulsive<br />

comfortable with chaos than<br />

with security, because they are<br />

more familiar with the drama.<br />

12. They tend to confuse<br />

love with pity. Because of this<br />

confusion, they frequently<br />

“love” people they can feel<br />

sorry for and rescue.<br />

13. They lack trust and<br />

have a fear of abandonment.<br />

They will hold on to a<br />

relationship “no matter<br />

what” to avoid the pain of<br />

abandonment.<br />

14. They have a tendency<br />

toward physical complaints<br />

and often suffer higher rates<br />

of stress related illnesses.<br />

5. They are suffering<br />

from a backlog of delayed<br />

grief. Losses experienced<br />

during childhood were<br />

never grieved because these<br />

families would not tolerate<br />

such intense feelings.<br />

Today, without calling up<br />

these past feelings, losses<br />

cannot be felt. As a result,<br />

these individuals may feel<br />

depressed often.<br />

16. They choose reaction<br />

over action. They are<br />

hypervigilant looking for<br />

potential problems they can<br />

feed into.<br />

17. They have developed<br />

harsh, even fierce self criticism.<br />

They have a low sense of<br />

self esteem, regardless of<br />

Spider veins (also known<br />

as spider hemangiomas)<br />

unlike varicose veins (dilated<br />

pre-existing veins) are<br />

acquired lesions caused by<br />

venous hypertension leading<br />

to proliferation of blood<br />

vessels in the skin and subcutaneous<br />

tissues due to the<br />

release of endothelial growth<br />

factors, causing vascular neogenesis<br />

(growth of new<br />

blood vessels). Clusters of<br />

spider veins have one or<br />

more incompetent perforator<br />

veins connected to the<br />

deeper veins causing reflux<br />

overflow of blood that is<br />

responsible for their<br />

dilatation and eventual<br />

incompetence.<br />

“Spider veins of the<br />

legs” is not a cosmetic condition,<br />

as this disease can be<br />

associated with significant<br />

symptoms and venous insufficiency.<br />

The spider veins<br />

can cause significant symptoms<br />

including pain, itching,<br />

burning, swelling, phlebitis,<br />

cellulites, bleeding and ulceration<br />

and restless legs.<br />

Untreated spider veins may<br />

lead to serious medical<br />

complications including<br />

superficial and deep venous<br />

thrombosis, aggravation of<br />

the already existing venous<br />

insufficiency, hemorrhage,<br />

postphlebitic syndrome,<br />

chronic leg ulceration and<br />

pulmonary embolism.<br />

Chronic inflammation<br />

associated with spider veins<br />

may lead to many degenerative<br />

diseases such as cardiovascular<br />

disease, diabetes,<br />

arthritis, Alzheimer’s disease<br />

and cancer in later life. The<br />

recent medical literature<br />

shows that superficial venous<br />

insufficiency (and varicose<br />

and spider veins) leads to<br />

deep venous insufficiency;<br />

and the treatment of superficial<br />

venous lesions leads to<br />

recovery of deep venous<br />

insufficiency and relief of<br />

swollen legs. The spider<br />

veins, just like varicose veins,<br />

are superficial venous lesions<br />

and can be responsible for<br />

both superficial and deep<br />

venous insufficiency.<br />

An effective method of<br />

treating the symptomatic spider<br />

veins is the use of electromagnetic<br />

energy in the form<br />

of intense light and laser.<br />

Since the majority of patients<br />

with symptomatic spider veins<br />

have associated venous insufficiency,<br />

it is recommended<br />

that in addition to the treatment<br />

of spider veins, saphenous<br />

venous insufficiency be<br />

treated utilizing venous closure<br />

with radio frequency or<br />

laser ablation.<br />

Psoriasis is a very common<br />

skin disorder seen in<br />

the physician’s office. It is<br />

estimated that more than<br />

4.5 million adults in the<br />

United States have been<br />

diagnosed with psoriasis,<br />

and approximately 150,000<br />

new cases are diagnosed<br />

each year. Unfortunately,<br />

because there is currently<br />

no cure for psoriasis, it is a<br />

lifelong skin condition.<br />

Psoriasis is an inflammatory<br />

skin condition that<br />

most typically presents with<br />

red plaques and thick, white<br />

scales. Although these skin<br />

lesions usually affect the<br />

elbow and knees, they can<br />

affect any and all areas of<br />

the skin, including the<br />

scalp, hands, feet and even<br />

nails. Some patients only<br />

have a few localized spots,<br />

while others may have the<br />

majority of their skin surface<br />

affected. Fortunately,<br />

psoriatic skin changes usually<br />

spare the facial area.<br />

Although the cause of<br />

psoriasis is still not fully<br />

understood, it is believed<br />

that psoriasis is an immune<br />

mediated disease. The<br />

faulty immune system signals<br />

the body to accelerate<br />

the growth of skin cells.<br />

When compared with normal<br />

behaving skin cells,<br />

psoriatic cells mature and<br />

move to the skin surface<br />

more quickly. Then,<br />

instead of being shed like<br />

normal cells, the psoriatic<br />

skin cells start to “pile-up”<br />

and cause visible lesions.<br />

Psoriasis also appears to<br />

have a genetic component.<br />

Approximately one-third of<br />

people who develop psoriasis<br />

have at least one family<br />

member with the condition.<br />

Even though it is possible to<br />

develop psoriasis at any<br />

age, most patients with<br />

psoriasis develop it before<br />

the age of 40. The earlier<br />

the psoriasis appears, the<br />

more likely it is to be widespread<br />

and recurrent. Due<br />

to the physical appearance<br />

of psoriasis, a common misconception<br />

is that it may be<br />

contagious. However,<br />

this is not true. Psoriasis<br />

is neither infectious<br />

nor contagious.<br />

Unfortunately, psoriasis<br />

can greatly impact a<br />

patient’s quality of life:<br />

physically, emotionally, and<br />

socially. Because itching,<br />

soreness, and cracked skin<br />

can be common manifestations<br />

in patients with psoriasis,<br />

it can cause discomfort<br />

and limit the patient’s phys-<br />

ical activities. The psoriatic<br />

patient’s thick white scales<br />

can often flake off, leaving<br />

behind residual flakes of dry<br />

skin. This can sometimes<br />

be difficult for patients in<br />

social environments.<br />

Because psoriasis is a skin<br />

condition, it can be a conspicuous<br />

disease—unlike<br />

most other medical conditions.<br />

Because of the visibility<br />

of the disorder, it can<br />

create a socially isolating or<br />

self-conscious environment<br />

for the patient. This can<br />

oftentimes impact the<br />

patient’s quality of life.<br />

While there is currently<br />

no cure for psoriasis, there<br />

are several different ways of<br />

managing the signs and<br />

symptoms of psoriasis.<br />

Treatment usually includes<br />

a combination of different<br />

medicated creams for localized<br />

skin disease. For more<br />

severe and extensive skin<br />

involvement, treatment can<br />

include oral medications<br />

and/or light-therapy. In<br />

addition to these treatment<br />

options, there is also a<br />

newer class of injectable or<br />

infusible medications available<br />

to patients with extensive<br />

psoriasis. With these<br />

medications emerging on<br />

the horizon, this is an exciting<br />

time for the treatment<br />

of patients with psoriasis.<br />

It’s no secret that women<br />

today take on more than<br />

ever; in addition to stressful<br />

jobs there are family,<br />

home, health, financial, and<br />

personal responsibilities to<br />

deal with. If not managed<br />

appropriately, stress can take<br />

a toll on your health. But<br />

there are steps you can take<br />

to reduce stress in your life.<br />

First, it’s important to<br />

understand that stress is a<br />

physical response that your<br />

body has when you are<br />

exposed to a stressor. The<br />

response begins in your<br />

nervous system, and is followed<br />

by the release of hormones<br />

and adrenaline. The<br />

results are the symptoms you<br />

feel that are typically associated<br />

with stress: headache;<br />

gritting teeth; muscle aches;<br />

heartburn; panic; or rashes.<br />

The signals leading up to this<br />

reaction include: anxiety;<br />

depression; lack of focus; disorganization;<br />

So how can we manage<br />

stress in our lives? The foundations<br />

of stress management<br />

involve some selfawareness<br />

and personal<br />

reflection. First, pay attention<br />

to the signals and symptoms;<br />

when you notice them<br />

in yourself, try to identify<br />

the cause. Is it a large task<br />

assigned at work? Is your<br />

spouse working late hours? Is<br />

money tighter this week?<br />

While you must recognize<br />

that some stressors in<br />

life are unavoidable, begin to<br />

reduce unnecessary stress in<br />

your life. Eliminate selfdefeating<br />

behaviors; in other<br />

words, avoid perfectionism.<br />

Control your environment;<br />

reduce clutter or institute<br />

some simple organizational<br />

systems in your home or<br />

office. Try to pare down your<br />

to-do list; delegate some<br />

household chores to your<br />

kids or spouse, and, if possible,<br />

some professional<br />

responsibilities to coworkers<br />

or support staff.<br />

Accept things you can’t<br />

change. Don’t engage in<br />

futile, frustrating efforts to<br />

control uncontrollable<br />

things. You may never<br />

change your boss’ habit of<br />

assigning you tasks last<br />

minute, or your child’s habit<br />

of running into the house in<br />

muddy sneakers. Try to see<br />

the positive in situations you<br />

can’t change, and learn to<br />

forgive. You can also remove<br />

some stress from your life by<br />

sharing feelings with a friend<br />

or family member.<br />

Once you begin to identify<br />

stressors, pay attention<br />

to how you deal with them.<br />

My heart was racing and<br />

I couldn’t stop shaking. I felt like<br />

I had trouble swallowing. I had<br />

an awful feeling something dreadful<br />

was going to happen. When<br />

the feelings passed, I constantly<br />

worried about my symptoms<br />

returning.<br />

Does this sound like<br />

something you might be experiencing?<br />

Has any one ever<br />

told you that you need to “calm<br />

down” or “reduce the stress in<br />

your life?” This type of advice<br />

rarely helps alleviate feelings of<br />

anxiety. In fact if you could<br />

calm down, you probably<br />

would calm down.<br />

According to the National<br />

Institute of Mental <strong>Health</strong>,<br />

anxiety disorders are the most<br />

common of the mental disorders.<br />

They affect 18 percent of<br />

Americans over the age of 18<br />

every year.<br />

Fortunately, anxiety disorders<br />

are very treatable. A variety<br />

of techniques exist and can<br />

include Cognitive Behavioral<br />

strategies such as challenging<br />

negative thoughts and beliefs<br />

as well as relaxation techniques<br />

such as guided imagery and<br />

deep breathing.<br />

Eye Movement Desensitization<br />

and Reprocessing<br />

(EMDR) is also a potential<br />

treatment to obtain relief from<br />

the symptoms of anxiety.<br />

This technique has been effective<br />

in helping clients process<br />

disturbing or traumatic<br />

experiences, which could be<br />

feeding their current symptoms<br />

of discomfort. EMDR seems<br />

to rebalance the brain by<br />

allowing negative past<br />

experiences to be integrated<br />

into a more positive<br />

emotional state.<br />

Anxiety disorders can be<br />

intensely uncomfortable and<br />

can create needless suffering.<br />

your level of functioning and<br />

regain the ability to relax,<br />

Menopause is a natural<br />

stage in every woman’s life.<br />

However, it is very individualized,<br />

presenting with an<br />

endless range of symptoms and<br />

severity for each woman. Some<br />

women have no desire to do<br />

anything about their symptoms;<br />

others want help<br />

decreasing their severity or<br />

frequency. Just as there are<br />

infinite presentations of<br />

menopause, there are<br />

also infinite ways to help<br />

with symptoms.<br />

There is a wide array of<br />

information on different ways<br />

in which a woman can make a<br />

smooth transition from premenopause<br />

to post-menopause.<br />

There are books, websites,<br />

newsletters, magazines and<br />

support groups. While there<br />

are many opinions regarding<br />

the management of<br />

menopausal symptoms, there<br />

are a few basics that most people<br />

agree help with symptoms.<br />

Below are some suggestions<br />

that may help with a wide<br />

range of some of the most common<br />

symptoms of menopause.<br />

NEVER start a new exercise<br />

regimen or start taking supplements<br />

or herbs without talking<br />

to your health care provider.<br />

While supplements are fairly<br />

easy to come by and may seem<br />

harmless, they can be very<br />

potent and may interact with<br />

medications you are on.<br />

Your health care provider should<br />

have a complete list of all medications,<br />

vitamins, supplements<br />

and herbs you take on a regular<br />

basis to ensure there are no<br />

potential interactions.<br />

Exercise: This is one of the<br />

healthiest and easiest changes<br />

to make. The benefits of exercise<br />

are endless and advocated<br />

by every health professional. It<br />

decreases your risk of certain<br />

cancers, diabetes, heart disease,<br />

osteoporosis and a countless<br />

number of other health problems.<br />

Studies have long shown<br />

that the benefits of exercise,<br />

along with the nice side effect<br />

of weight loss, are numerous<br />

enough to justify its status as<br />

one of the most important<br />

things you can do for yourself.<br />

For menopause specifically,<br />

exercise can help with fuzzy<br />

thinking, decreased libido,<br />

hot flashes, night sweats,<br />

insomnia and mood swings.<br />

Recommendations are for 30<br />

minutes of exercise at least 4<br />

days a week. Exercise can<br />

include walking, running,<br />

weight lifting, swimming, yard<br />

work, intercourse, aerobics,<br />

yoga, pilates and any sportanything<br />

that increases your<br />

heart rate and make your<br />

perspire. Always talk to a<br />

health care professional before<br />

starting a new exercise<br />

regimen, and start slowly; do<br />

not start working out every day<br />

or you will be exhausted and<br />

risk exhaustion.<br />

Diet: This is referring to<br />

your overall diet, not going on<br />

a diet. A healthy, sustainable<br />

diet does not completely<br />

exclude any foods or drinks;<br />

it merely includes certain<br />

things in moderation. The<br />

Recommendations are for<br />

several servings of fruits and<br />

vegetables daily. These can be<br />

obtain by simply adding another<br />

vegetable to a sandwich,<br />

fruit to yogurt or cereal and<br />

having a small salad with dinner.<br />

Every diet should have a<br />

small amount of fat; trying to<br />

cut fat out completely is not<br />

only extremely difficult, but<br />

can also be more detrimental<br />

than good. There are some<br />

important vitamins and minerals<br />

that require fat to<br />

be absorbed.<br />

Eating a well-balanced diet<br />

can help you feel healthy,<br />

happy, and energized. It can<br />

also help with concentration<br />

and mood swings. Adding 1-2<br />

servings of soy proteins a day<br />

can help decrease hot flashes<br />

and night sweats. These can be<br />

found in soy milk, tofu and<br />

soybeans. Flaxseed also has the<br />

same effect and is found in<br />

many foods or can be sprinkled<br />

on top of cereals, oatmeal<br />

and salads.<br />

Acupuncture: Acupuncture<br />

has long been used in Eastern<br />

medicine for a multitude of reasons<br />

ranging from back pain<br />

and headaches to infertility and<br />

treatment of illnesses. Some<br />

women find relief from heavy<br />

bleeding, hot flashes, night<br />

sweats, mood swings, insomnia<br />

and vaginal dryness.<br />

Unfortunately not many insurance<br />

companies will cover<br />

acupuncture, but the benefits<br />

can often outweigh the cost.<br />

Menopause can be a time of<br />

many changes for women,<br />

from major life changes to<br />

physical symptoms. While<br />

some of these are welcome<br />

changes, the effects of<br />

menopause can be significant<br />

enough to disrupt everyday<br />

life. Remember, menopause is<br />

not something that needs to be<br />

fixed; it is a natural part of a<br />

woman’s life. How to cope<br />

with the symptoms is a highly<br />

individualized decision that<br />

can include altering many areas<br />

of your life. By making some<br />

small changes, this transition<br />

can be a little smoother and<br />

Who Are They And What Do They Offer A Family<br />

What has become a new trend<br />

in funeral service? The choice<br />

of using a Certified Funeral<br />

Celebrant, “officiant”, at a<br />

funeral, is the answer.<br />

What is a “Celebrant”?<br />

A “Certified Celebrant” is<br />

trained to assist in the planning<br />

of a funeral service that is<br />

basically secular in nature and<br />

to present a personalized life<br />

story of the loved one who has<br />

died. Families who wish to<br />

have a more traditional celebration<br />

of their loved one<br />

who has had a particular religious<br />

affiliation, would find a<br />

funeral director reaching out<br />

to the denomination of their<br />

choice. The use of a<br />

Celebrant offers an alternate<br />

choice to those families who<br />

do not wish to have a ceremony<br />

with any religious content.<br />

However, the Celebrant, at<br />

the request of the family, may<br />

be asked to include some<br />

prayers or readings of a<br />

spiritual nature.<br />

A Certified Celebrant meets<br />

with the family of the<br />

deceased in order to understand<br />

who they were, what<br />

interests they had, their<br />

work experience, community<br />

involvement and what impact<br />

their lives had on the people<br />

they loved. They are trained<br />

to help a family create a<br />

personal living tribute to their<br />

loved one by sharing their<br />

stories about them. A bond of<br />

trust needs to exist, however,<br />

between the family and<br />

Celebrant because many times<br />

sensitive information or family<br />

“secrets” may be shared.<br />

Interviewing family members<br />

is important in order to gain a<br />

real sense about the life<br />

of their loved one. The ceremony<br />

created for the deceased<br />

is ever so important not only<br />

because it honors them but<br />

is also deeply moving and<br />

meaningful for those who<br />

are grieving.<br />

Ceremony and ritual is very<br />

important to families as they<br />

begin their journey of grief.<br />

Every life should be honored<br />

with significant stories that are<br />

told about them and shared<br />

with those participating in the<br />

ritual. Family involvement in<br />

presenting this information is<br />

a way for them to strengthen<br />

their bond and no matter what<br />

their belief system is, can help<br />

in their transition into living<br />

without their loved one.<br />

ceremonies, poetry readings,<br />

incorporating music<br />

whether it be secular or religious,<br />

is strongly encouraged,<br />

and distributing meaningful<br />

mementos, etc. have become<br />

part of what he will incorporate<br />

into his tribute, if the<br />

family wishes.<br />

By tapping into the attentive,<br />

sensitive and officiating abilities<br />

of a Celebrant, families, along<br />

with a Celebrant, can create an<br />

extremely personal and meaningful<br />

There are various<br />

statutes and Court Rules<br />

that address custody and<br />

related issues in New<br />

Jersey. It is important for<br />

divorcing parents to<br />

understand that the state<br />

Legislature has found that<br />

it is in the best interests of<br />

children to have frequent<br />

and continuing contact<br />

with both parents after<br />

separation and divorce and<br />

that it is in the public<br />

interest to encourage both<br />

parents to share the rights<br />

and responsibilities of<br />

rearing their children.<br />

The law in New Jersey also<br />

provides that the rights of<br />

both parents are equal.<br />

The Court may accept<br />

any custody arrangement<br />

which is agreed to by<br />

both parents provided<br />

that it<br />

is not contrary<br />

to the children’s best<br />

interests.<br />

If the parents are<br />

unable to agree upon custody,<br />

the case must be<br />

referred to mediation. If<br />

mediation is unsuccessful,<br />

the Court may then order<br />

an investigation or<br />

psychological best interests<br />

evaluation before the trial.<br />

In making a custody<br />

determination, the court<br />

must consider factors<br />

enumerated in the statute<br />

including, but not limited<br />

to, the parents’ ability to<br />

communicate and cooperate;<br />

any history of unjustified<br />

interference with<br />

the non-custodial parent’s<br />

parenting time; the preference<br />

of the child when of<br />

sufficient age and capacity<br />

to reason so as to form an<br />

intelligent decision; the<br />

needs of the child; the<br />

stability of the home; the<br />

fitness of the parents; and<br />

the parents’ employment<br />

responsibilities.<br />

physical custody, whether<br />

joint or sole; provisions for<br />

the enjoyable part, but making<br />

sure you’ve got a sturdy<br />

When it comes to marketing,<br />

the foundation of<br />

your efforts should take the<br />

form of a marketing strategy.<br />

Think of a marketing strategy<br />

as your marketing raison<br />

d’jtre--the position you want<br />

to hold in the mind of a very<br />

specific customer. How can<br />

you develop an effective<br />

marketing strategy? First,<br />

you must spend a little time<br />

and energy determining two<br />

crucial factors: who makes an<br />

ideal client for your business,<br />

and how your business is different<br />

from others in your<br />

industry.<br />

If you’re just getting<br />

started, you might not know<br />

exactly whom you should<br />

target or what makes an ideal<br />

client – that knowledge<br />

comes through trial and<br />

error. In the beginning,<br />

though, you need to give<br />

some thought to identifying<br />

a narrowly defined market<br />

that could respond to your<br />

products and services. I<br />

intentionally use the term<br />

“narrowly defined” to get you<br />

thinking about specific market<br />

segments or niches where<br />

industry they can choose<br />

from, so you need to do as<br />

much as you can to stand<br />

out. One of the best ways to<br />

do this is to concentrate on<br />

For example, let’s say<br />

you’re a financial planner.<br />

There are several well-established<br />

firms chasing the same<br />

clients you’re after. But if you<br />

target individuals who have<br />

recently sold their businesses,<br />

you may be able to develop<br />

a marketing strategy that<br />

Some of the basic points<br />

I just mentioned may sound<br />

like common sense, but you<br />

would be surprised at how<br />

many of these “common<br />

errors” are lurking in your<br />

material. Rarely does a customer<br />

come to us with material<br />

that doesn’t have words<br />

misspelled, pixilated graphics<br />

(those boxy squares in<br />

photographs because the<br />

wrong size image was used),<br />

missing information or too<br />

much text. We have seen<br />

poor renderings of company<br />

logos, incorrect words used<br />

which changed the meaning<br />

of the message, grainy photographs<br />

of indistinguishable<br />

products and much worse.<br />

Some of these were do-ityourself<br />

jobs, but many have<br />

come from leading advertising<br />

agencies that should<br />

have caught the errors.<br />

We’ve all heard that you<br />

get one chance to make a<br />

first impression and it’s true.<br />

If you think that the errors<br />

and problems are just mentioned<br />

are minor and do not<br />

affect your bottom line,<br />

think again. We have heard<br />

from countless clients who<br />

learned otherwise the hard<br />

way – and from potential<br />

customers who did not use a<br />

company because of the<br />

material they were handed.<br />

Getting the Most for Those Who Need It Most<br />

Amidst rising healthcare<br />

costs and rigid public-aid<br />

requirements, special-needs<br />

trusts can enrich the lives of<br />

your disabled loved ones.<br />

Here’s the lowdown on<br />

what you need to know.<br />

The facts and figures are<br />

staggering: nearly 54-million<br />

people in America cope<br />

with special needs and the<br />

rising expenses related to<br />

some form of disability—<br />

whether cognitive, developmental<br />

or physical—<br />

according to the National<br />

Organization on Disability.<br />

The rise in cases of certain<br />

disabilities may be on the<br />

verge of creating a national<br />

health crisis. Autism, for<br />

one, drains the economy of<br />

$35 billion a year according<br />

to a recent Harvard School<br />

of Public <strong>Health</strong> study. The<br />

Harvard research also indicates<br />

that the lifelong bill<br />

for one autistic individual,<br />

from birth until death,<br />

could reach as much as $3.2<br />

million. What’s more, individuals<br />

with special needs<br />

relying on government<br />

assistance alone—such as<br />

Medicaid and Supplemental<br />

Security Income—often<br />

live near the poverty level<br />

in the absence of additional<br />

income from their family,<br />

community or other<br />

loved ones.<br />

Despite these alarming<br />

numbers, once a disability is<br />

diagnosed, early planning<br />

helps families add to and<br />

protect the assets of a special-needs<br />

individual, preventing<br />

the loss of critical<br />

benefits. One effective way<br />

to protect a loved one is<br />

through a special-needs<br />

trust. “It’s the most popular<br />

planning tool,” according to<br />

Vincent J. Russo, cofounder<br />

of the Academy of Special<br />

Needs Planners and a special-needs<br />

estate attorney.<br />

To Preserve and Protect<br />

Special-needs trusts<br />

were created to manage<br />

resources while protecting<br />

an individual’s eligibility for<br />

vital public assistance.<br />

Establishing an SNT may<br />

also prevent court intervention<br />

by eliminating the<br />

need to appoint a guardian<br />

to manage the disabled<br />

individual’s assets. SNTs are<br />

designed to pay for items—<br />

such as education, alternative<br />

therapies, counseling<br />

and vacations—which go<br />

beyond the simple life<br />

expenses covered by government<br />

benefits.<br />

There are two basic<br />

types of SNTs: the thirdparty<br />

special-needs trust<br />

and the first-party specialneeds<br />

trust. Third-party<br />

SNTs commonly are created<br />

by a parent or other family<br />

member for a child with<br />

special needs—though the<br />

child may be an adult when<br />

the trust is created or funded.<br />

These SNTs require no<br />

government paybacks and<br />

allow the beneficiary to<br />

receive gifts, lawsuit settlements<br />

or other funds<br />

without losing their eligibility<br />

for certain government<br />

programs.<br />

When the disabled individual<br />

establishes the trust<br />

for himself, it is referred to<br />

as a first-party or self-settled<br />

SNT. These SNTs permit<br />

the disabled person to<br />

protect his or her assets<br />

from creditors and can help<br />

to reduce total assets for tax<br />

or Medicaid planning<br />

purposes. Three types of<br />

first-party trusts exist. Payback<br />

trusts are available for<br />

disabled individuals under<br />

age 65 and require payback<br />

for government assistance<br />

upon the death of the disabled<br />

individual. Secondly,<br />

qualified-income trusts are<br />

funded solely by an individual’s<br />

income—such as a<br />

pension or social security.<br />

They have no age or disability<br />

restrictions, but require<br />

government paybacks.<br />

Lastly, not-for-profit pooled<br />

trusts have no age or government<br />

payback requirements<br />

if the amounts remaining at<br />

the disabled beneficiary’s<br />

death remain in the trust for<br />

the benefit of other disabled<br />

beneficiaries.<br />

One very important fact<br />

to remember: it is critical to<br />

plan for the SNT before the<br />

disabled individual receives<br />

assets in his or her own<br />

name. Many parents incorrectly<br />

assume that their<br />

children qualify for government<br />

benefits simply<br />

because they are disabled.<br />

The eligibility for government<br />

benefits is based on<br />

their parents’ income and<br />

assets. However, when<br />

these children reach a certain<br />

age (usually 18), eligibility<br />

is based primarily on<br />

their own income and assets<br />

rather than those of their<br />

parents. If the disabled person’s<br />

assets (excluding certain<br />

items, like a home or<br />

car) exceed $2000, the government<br />

will discontinue<br />

benefit payments until any<br />

extra money has been<br />

accounted for.<br />

Where to Turn<br />

When it comes to funding<br />

your SNT, first assess<br />

your savings goals for lifetime<br />

care expenses. The<br />

Academy of Special Needs<br />

Planners offers useful calculators<br />

and checklists on its<br />

Web site, www.specialneedsanswers.com.<br />

You<br />

may also want to hire a lifecare<br />

planner to develop a<br />

comprehensive assessment<br />

of current and future care<br />

needs and make recommendations.<br />

Life insurance is<br />

often the best source of<br />

SNT funding. It requires a<br />

smaller initial outlay and<br />

the proceeds are received as<br />

tax-free income and often<br />

as a tax-free estate. When<br />

there are two parents, survivorship<br />

life insurance is<br />

the most popular funding<br />

choice because it is usually<br />

the least expensive. SNTs<br />

also can be funded with<br />

proceeds obtained by the<br />

beneficiary through court<br />

proceedings, inheritances,<br />

life insurance claims, settlements<br />

or gifts.<br />

When it comes to special-needs<br />

care, government<br />

assistance only goes so far.<br />

though it is a bit early to get<br />

your hands in the dirt, there<br />

is plenty you can do to<br />

gear up for the gardening<br />

season.<br />

March is a great month<br />

for trimming and tidying.<br />

Now is when you should<br />

cut back your ornamental<br />

grasses. Hand pruners do a<br />

good job on smaller clumps,<br />

but hedge trimmers are<br />

handy for larger clumps.<br />

Hold or tie the old growth<br />

with twine and cut the grass<br />

and look for new, green<br />

Sabbath, and can also be enjoyed at any time of the year.<br />

3 Eggs<br />

1/3 Cup Water<br />

(room temperature)<br />

1 1/2 tbsp butter<br />

(room temp)<br />

3 tbsp sugar<br />

1 1/2 tsp salt<br />

3 cups all-purpose<br />

flour<br />

1 tbsp dry yeast<br />

1 tbsp cornmeal or<br />

semolina flour<br />

1 egg yolk<br />

(beaten)<br />

Put wet ingredients and eggs in the bread pan first except for the extra egg yolk; cut up the butter into<br />

small pieces and drop in. Add dry ingredients except for the cornmeal or semolina, making sure the<br />

salt is added anywhere but on top of or near the yeast. (If you’re making this by hand, add the dry<br />

yeast to the water first, stir, and let it sit for a few minutes until you see a few bubbles.) Set the bread<br />

machine to “dough” cycle, and when it is ready, punch down. If you don’t have a bread machine, mix<br />

and knead the ingredients well and set the dough aside in a covered container to rise until it is doubled<br />

in size (usually about an hour). Preheat the oven to 350 degrees.ΔCut the dough into 3 equal parts,<br />

and roll into “ropes.” Braid the ropes together, pinch the ends and turn them under. Sprinkle the<br />

cornmeal or semolina flour on your baking pan or stone (prevents the bread from sticking). Brush the<br />

braided dough with egg yolk. Bake until nicely browned, 30 to 35 minutes.<br />

Banana Pecan Bread<br />

A favorite recipe that is also a simple one, and a<br />

great way to avoid throwing away old bananas.<br />

You don’t need to add water or milk to this recipe;<br />

there is plenty of moisture in the bananas.<br />

1 cup sugar<br />

1/2 cup shortening<br />

2 eggs<br />

2 cups self-rising flour<br />

1/2 cup finely chopped pecans<br />

3 overripe bananas, mashed<br />

(the blacker, the better)<br />

dash of nutmeg<br />

1/4 tsp cinnamon<br />

1/2 tsp salt<br />

Preheat the oven to 325 degrees. Cream the sugar<br />

and shortening, then add the mashed bananas.<br />

Mix. Add the flour and the rest of the dry<br />

ingredients. Don’t overmix! Pour the batter into a<br />

greased loaf pan, and bake at 325 degrees for an<br />

hour and 15 minutes. The banana bread is done<br />

when you insert a toothpick or fork and it comes out<br />

clean. Makes 1 Banana Bread loaf.<br />
