ED. This questionnaire is most often completed by the patient in the exam
room or waiting room prior to the interview. The SHIM is appended, and is
scored as follows:
Score 22-25 no ED
Score 17-21 mild ED
Score 12-16 mild to moderate
Score 8-11 moderate
Score 0-7 severe ED (in my experience consider psychogenic ED, or is seen
after radical prostatectomy or pelvic surgery)
The SHIM gives a severity index, a common vocabulary and has supplanted
vascular testing in many cases. The SHIM, however, is not predictive
outcome of Low libido.
The ADAM questionnaire, authored by Morley [Morley et al (2000)], is
relatively easy to administer and to use. The ADAM questionnaire is
considered positive for low libido if there is a yes response to q
no response to question 7, or a yes response to any other 3 questions. It
appears that this questionnaire is sensitive, but not very specific for
hypogonadism. The major shortcoming, that is not a questionnaire issue, but
rather a conceptual issue, is that of the definition of low libido. Depression/

While there are many questionnaires available to assess depression, we have
used the second version of the Beck Depression Inventory (
BDI-II)

depression [Sheehan et al (1995)] instruments. These are relatively short,
straightforward and completed by the patient. They provide excellent
