Musculoskeletal (<strong>MSK</strong>) sonography, which is<br />

used in the evaluation and treatment of joint and<br />

soft-tissue diseases, has long been popular in<br />

Europe and other countries overseas. While <strong>MSK</strong><br />

sonography isn’t used as widely in the United<br />

States, it has been growing rapidly here over the<br />

last decade as well.<br />

As the U.S. looks at ways <strong>to</strong> reduce health-care<br />

costs yet maintain quality, and, as concerns<br />

about radiation safety grow, more American<br />

physicians are expected <strong>to</strong> use <strong>MSK</strong> sonography<br />

- not only <strong>to</strong> diagnose but also <strong>to</strong> treat<br />

musculoskeletal injuries.<br />

In response <strong>to</strong> this growing demand worldwide,<br />

the American Registry <strong>for</strong> Diagnostic Medical<br />

<strong>Sonography</strong> (<strong>ARDMS</strong>) is launching an <strong>MSK</strong><br />

sonography credential. <strong>ARDMS</strong> will begin<br />

accepting examination applications <strong>for</strong> its<br />

Registered in Musculoskeletal (R<strong>MSK</strong>)<br />

domestic musculoskeletal sonography practices<br />

and build a community of health-care professionals<br />

working <strong>to</strong>ward the same goal of patient safety<br />

through standardized practice.”<br />

At first, <strong>ARDMS</strong> is expecting <strong>to</strong> see a greater demand<br />

<strong>for</strong> the new exam overseas where <strong>MSK</strong> is the number<br />

one or number two most commonly per<strong>for</strong>med<br />

sonographic study. “Outside of North America, it<br />

fluctuates between OB/GYN and <strong>MSK</strong>, ” says Kevin D.<br />

Evans, PhD, RT(R)(M)(BD), RDMS, RVS, FSDMS, Chair<br />

of the <strong>ARDMS</strong> Board of Direc<strong>to</strong>rs. However, he says,<br />

“as <strong>MSK</strong> sonography becomes more common in the<br />

United States, <strong>ARDMS</strong> expects <strong>to</strong> see the U.S. demand<br />

<strong>for</strong> this new credential <strong>to</strong> increase dramatically.”<br />

R<strong>MSK</strong> answers a growing need<br />

<strong>ARDMS</strong> has been working on developing the R<strong>MSK</strong><br />

credential <strong>for</strong> several years because of the growing<br />

interest it has seen. “One of the reasons we started<br />

looking in<strong>to</strong> developing the R<strong>MSK</strong> credential was<br />

the growing use of <strong>MSK</strong> sonography in the U.S. and<br />

because we were getting so many inquiries from our<br />

Registrants and the medical community at large,”<br />

Evans says. “It’s an exciting time <strong>for</strong> <strong>ARDMS</strong> and<br />

practice of sonography.”<br />

Be<strong>for</strong>e deciding <strong>to</strong> go ahead with developing the<br />

<strong>MSK</strong> exam, <strong>ARDMS</strong> surveyed the marketplace and<br />

found interest <strong>to</strong> be at a high level. “The <strong>ARDMS</strong><br />

mission is <strong>to</strong> promote quality care and patient safety<br />

through the certification and continuing competency<br />

of ultrasound professionals, ” says Evans. “<strong>MSK</strong><br />

sonography is quickly becoming a worldwide imaging<br />

standard. It was clear that a professional credential in<br />

that specialty is both timely and needed. ”<br />

The <strong>MSK</strong> credentialing exam will be open <strong>to</strong><br />

medical professionals who practice musculoskeletal<br />

ultrasound. That includes not only sonographers but<br />

also physical medicine and rehabilitation specialists,<br />

sports medicine practitioners, osteopathic surgeons,<br />

radiologists, chiroprac<strong>to</strong>rs, rheuma<strong>to</strong>logists,<br />

podiatrists and orthopedic surgeons. The same exam<br />

will be given <strong>to</strong> physicians and non-physicians.<br />

The <strong>MSK</strong> exam will be a single exam with the<br />

sonography principles and instrumentation content<br />

all rolled in<strong>to</strong> it. Because the exam is open <strong>to</strong> anyone<br />

who practices <strong>MSK</strong> ultrasound, it’s modeled more<br />

after <strong>ARDMS</strong>’s RPVI (Registered Physician in Vascular<br />

Interpretation) exam than other sonographer exams,<br />

Evans says. There will be one prerequisite applicants<br />

will need <strong>to</strong> meet, Evans says, but it has yet <strong>to</strong> be<br />

announced.<br />

It’s likely those who register <strong>for</strong> the exam will hold an<br />

existing medical degree or certification and will have<br />

per<strong>for</strong>med a certain number of <strong>MSK</strong> studies, he says.<br />

“The prerequisite was designed so that a vast majority<br />

of medical professionals will be eligible <strong>to</strong> take the <strong>MSK</strong><br />

exam and that anyone using <strong>MSK</strong> sonography should<br />

be able <strong>to</strong> take it, ” Evans says.<br />

<strong>Exam</strong> content being developed<br />

The content outline <strong>for</strong> the R<strong>MSK</strong> credential is<br />

available at www.<strong>ARDMS</strong>.org/R<strong>MSK</strong>. According <strong>to</strong> the<br />

outline, the tasks <strong>to</strong> be covered include: ana<strong>to</strong>my and<br />

physiology (28 %) pathology (28 %), patient care (2%),<br />

integration of data (8%), pro<strong>to</strong>cols (15%), physics and<br />

instrumentation (7%), treatment (10%) and “other”<br />

(2%). The ana<strong>to</strong>mic areas <strong>to</strong> be covered will include:<br />

shoulder, elbow, wrist and hand, hip, knee, ankle and<br />

foot, soft tissue and general.<br />

It’s still undecided whether the R<strong>MSK</strong> credential<br />

exam will be all multiple choice questions or include<br />

advanced item types – where the questions involve<br />

simulated ultrasound consoles. Such questions might<br />

list a problem statement and display the controls of an<br />

ultrasound machine. The test-taker would be asked <strong>to</strong><br />

adjust the controls that translate <strong>to</strong> a correct solution<br />

<strong>to</strong> the problem. Or it could be that the test-takers are<br />

asked <strong>to</strong> use their mouse <strong>to</strong> indicate a particular detail<br />

of the image on the screen.<br />

Item writers are working on the questions and, “they’re<br />

being vetted and reviewed, ” Evans says. “Multiple<br />

versions of the examination are being developed.”<br />

Because the vast majority of <strong>MSK</strong> sonography studies<br />

are done outside of the U.S., <strong>ARDMS</strong> has sought input<br />

from experts from around the world in developing the<br />

credential, Cyr notes.<br />

<strong>ARDMS</strong> is developing a practice version of the <strong>MSK</strong><br />

examination and hopes <strong>to</strong> have it ready prior <strong>to</strong> the<br />

<strong>for</strong>mal exam launch in mid-September. “Medical<br />

professionals will be able <strong>to</strong> visit the <strong>ARDMS</strong> website<br />

(www.<strong>ARDMS</strong>.org) and take a practice version of the<br />

<strong>MSK</strong> examination <strong>for</strong> a nominal fee, ” Evans says.<br />

Opportunity <strong>to</strong> take exam limited<br />

Because it is the first time it’s being given, the exam<br />

will have a window of about a month when it can be<br />

taken, Evans says. “Our other exams are administered all<br />

the time, ” Evans says, “but our <strong>MSK</strong> is going <strong>to</strong> be in a


specific time frame. We will announce the time frames<br />

in the spring and summer <strong>for</strong> the next several years of<br />

administration.”<br />

After the first group has taken the exam, <strong>ARDMS</strong> will<br />

determine what score test-takers will need <strong>to</strong> achieve<br />

in order <strong>to</strong> pass, says Ellen Julian, PhD, Direc<strong>to</strong>r of<br />

Testing and Research at <strong>ARDMS</strong>.<br />

Dr. Julian explains why the test is scored that way:<br />

“Typically, the way we build a test, we have questions<br />

we’ve used in the past and know exactly how difficult<br />

they are. Those are the ones on which we score people<br />

and, because they are known quantities, we can<br />

calculate in advance the number correct needed <strong>to</strong><br />

pass.”<br />

However, because this is a new test, “and we’ve never<br />

used any of the questions be<strong>for</strong>e, we don’t know how<br />

hard the questions will be and which ones are working<br />

well. We have <strong>to</strong> get all the data back and analyze the<br />

questions <strong>to</strong> determine what the cut score will be.”<br />

Also, because it’s a new test, examinees will not hear<br />

the results as quickly as other <strong>ARDMS</strong> credentialing<br />

exams. “We’re saying 60 days from the last test date,<br />

” Dr. Julian says, “but we’re hoping <strong>to</strong> have the scores<br />

out in less time.”<br />

The first administration <strong>for</strong> the <strong>MSK</strong> exam will be<br />

mid-September <strong>to</strong> mid-Oc<strong>to</strong>ber 2012. The second<br />

administration is slated <strong>for</strong> mid-March through<br />

mid-April 2013. “If demand is high enough, the <strong>MSK</strong><br />

examination may move <strong>to</strong> continuous administration<br />

just like our other exams, ” Evans says.<br />

Sonographers who are interested also can sign-up on<br />

the <strong>ARDMS</strong> Website (www.<strong>ARDMS</strong>.org/R<strong>MSK</strong>) so that<br />

they will receive alerts about the R<strong>MSK</strong> exam when<br />

things are happening, Evans says.<br />

SDMS Annual Conference <strong>to</strong> include <strong>MSK</strong><br />

Sonographers who are interested in taking the new<br />

<strong>ARDMS</strong> R<strong>MSK</strong> exam can prepare by attending this<br />

year’s SDMS Annual Conference, Sept. 20-23, at the<br />

Washing<strong>to</strong>n State Convention and Trade Center in<br />

Seattle.<br />

At this year’s conference, musculoskeletal ultrasound<br />

will be part of the abdominal track. Janette Wybo, BS,<br />

RDMS, RDCS, RVT, Program Coordina<strong>to</strong>r at Providence<br />

Hospital School of Diagnostic Medical <strong>Sonography</strong><br />

and a member of the SDMS Conference Management<br />

Committee, says <strong>MSK</strong> sonography is one of the most<br />

requested <strong>to</strong>pics by attendees.<br />

The Thursday tu<strong>to</strong>rial will have two 90-minute <strong>MSK</strong><br />

lectures with hands-on scanning by invited faculty,<br />

Veronica Rodriguez, RDMS, CEO, of Great Lakes<br />

Musculoskeletal Ultrasound Inc. Rodriquez has 15<br />

years of experience dedicated <strong>to</strong> <strong>MSK</strong> sonography<br />

and has conducted lectures and scanning workshops<br />

at national and international musculoskeletal<br />

conferences, Wybo says.<br />

At the first Thursday session, Rodriguez will lecture<br />

on shoulder scanning pro<strong>to</strong>col and sonographic<br />

appearances with the majority of time devoted <strong>to</strong><br />

live scanning. The second session will focus on <strong>MSK</strong><br />

scanning of the ankle, knee, wrist and elbow.<br />

“To make the session most effective <strong>for</strong> everyone,<br />

there will be dual screens with a videographer<br />

projecting scanning transducer placement along with<br />

displayed machine image, ” Wybo says. “<strong>This</strong> worked<br />

out well <strong>for</strong> attendees last year <strong>for</strong> scanning technique<br />

and instruction, ” she notes.<br />

During the main conference track, the SDMS<br />

Conference Management Committee has asked<br />

Sandra J. Allison, MD, Associate Professor of Radiology<br />

from George<strong>to</strong>wn University in Washing<strong>to</strong>n, D.C., <strong>to</strong><br />

return as part of the conference faculty. Dr. Allison<br />

has a passion <strong>for</strong> musculoskeletal imaging and will<br />

be giving two lectures on <strong>MSK</strong> ultrasound and their<br />

applications <strong>for</strong> the upper and lower extremities. She<br />

will focus on how ultrasound has advanced in imaging<br />

and diagnosis along with what circumstances it<br />

should be the modality of choice <strong>for</strong> clinicians.<br />

“Attendees have enjoyed Dr. Allison’s lectures as part<br />

of the SDMS conference in the past and we are excited<br />

she will be joining us in Seattle, ” Wybo says.<br />

The SDMS has been actively working on the concept<br />

of an Ultrasound Practitioner (UP) <strong>for</strong> over a decade.<br />

The foundation <strong>for</strong> the UP concept is the creation<br />

of a mid-level provider of sonography services that<br />

functions as a physician extender. While there is<br />

currently no “official” designation, title, or certification<br />

examination associated with this position, we know<br />

that some sonographers are currently functioning at<br />

this level. It is important that we know and understand<br />

the extent <strong>to</strong> which some sonographers have already<br />

moved in<strong>to</strong> this role, and the responsibilities and<br />

clinical activities that they have assumed in making<br />

this transition. To document this in<strong>for</strong>mation <strong>to</strong><br />

aid in future decisions, the SDMS and <strong>ARDMS</strong> have<br />

developed this survey.<br />

We ask that sonographers who are currently<br />

functioning in this physician extender role (not <strong>to</strong> be<br />

interpreted as a “lead sonographer” or the SDMS APS<br />

membership classification, <strong>to</strong> complete the survey).<br />

For purposes of this survey, this sonography physician<br />

extender shall be referred <strong>to</strong> as an Ultrasound<br />

First <strong>Sonography</strong> Bachelor’s Degree Students<br />

on the Continent of Africa!<br />

SDMS and the SDMS Foundation recently donated<br />

textbooks, educational videos, scrub <strong>to</strong>ps, and<br />

<strong>to</strong>tebags <strong>for</strong> sonography students at the Kwame<br />

Nkrumah University of Science and Technology in<br />

Kumasi, Ghana, where Ann Polin, MS, RDMS, RDCS, RVT,<br />

is a faculty lecturer <strong>for</strong> the <strong>Sonography</strong> Department.<br />

Ann recently sent SDMS this picture of the sonography<br />

students with their new scrub <strong>to</strong>ps and <strong>to</strong>tebags.<br />

These students will be the first sonography bachelor’s<br />

degree students on the continent of Africa – quite an<br />

accomplishment <strong>for</strong> all involved!<br />

Ann has returned <strong>to</strong> Ghana approximately three times<br />

a year since 2000, conducting ultrasound workshops<br />

<strong>for</strong> physicians, residents and medical students at<br />

the Komfo Anokye Teaching Hospital, and teaching<br />

classes <strong>to</strong> students in the university’s sonography<br />

bachelor’s degree program.<br />

What inspired you <strong>to</strong> make a career of sonography?<br />

A very good men<strong>to</strong>r, Dr. Wan Chan, introduced me<br />

<strong>to</strong> Lynda Pitts, RDMS, in the early 1980s. She was an<br />

absolute pioneer and phenomenal sonographer <strong>for</strong><br />

her time. She offered private ultrasound lessons and<br />

inspired me <strong>to</strong> continue <strong>for</strong>ward. The first time I saw<br />

an echocardiogram, I was hooked <strong>for</strong> life.<br />

Did you have a memorable role model/teacher?<br />

The list is long of those whom I admire and have<br />

served as very memorable role models. These people<br />

share a common thread...”doing the right thing<br />

when no one is looking.” I continue <strong>to</strong> revere them as<br />

men<strong>to</strong>rs, friends, and all around great people. Who are<br />

they? At the <strong>to</strong>p of the list, <strong>for</strong> me, is Joan Baker who<br />

emulates all of these qualities. Others are Dr. Michael<br />

Myracle, Dr. William Hastrup, the cardiologist team at<br />

Children’s Hospital Central Cali<strong>for</strong>nia, Dr. Krish Rajani<br />

and Dr. Steve Elliott, Marsha Neumeyer, Dale Cyr, Jean<br />

Lea Spitz, Dr. Diane Kawamura, and the entire Board of<br />

Direc<strong>to</strong>rs/Foundation Board of the SDMS and staff that<br />

I have been privileged <strong>to</strong> serve with. As well, I continue<br />

<strong>to</strong> learn from a host of colleagues in our profession<br />

nationwide.<br />

What was the most significant advance in the<br />

technology that you directly experienced?<br />

I believe the move <strong>to</strong> digital imaging and video clips<br />

<strong>This</strong> is a continuing series of interviews of<br />

our distinguished SDMS Fellow members.<br />

was the most significant advance. Be<strong>for</strong>e the digital<br />

era (developing our films) there was an enormous<br />

amount of pressure <strong>to</strong> “get it right” the first time as<br />

there was no way <strong>to</strong> erase a mistake. That, in itself, was<br />

not a bad thing. But with the advent of digital imaging<br />

our exams have improved resolution and diagnostic<br />

capability.<br />

Were there any memorable developments, cases<br />

or events that changed the way you practiced<br />

sonography?<br />

There were a few cases that stand out. One involved<br />

a patient that was misled by a non-sonographer<br />

and business woman at a 3D establishment. The<br />

patient was <strong>to</strong>ld by this non-sonographer that her<br />

baby “looks great” when in fact the fetus had a<br />

life-threatening congenital heart defect. <strong>This</strong> case<br />

prompted me <strong>to</strong> immediately join the Government<br />

Relations Committee <strong>for</strong> the SDMS <strong>to</strong> try and keep<br />

the transducer in the hands of fully qualified and well<br />

educated sonographers.<br />

Another case, which I am sure we can all share, was an<br />

instance that my gut <strong>to</strong>ld me that I needed <strong>to</strong> go the<br />

extra mile. I did, and the end result was the patient<br />

barely escaped an aortic aneurysm rupture. That case<br />

taught me <strong>to</strong> never give up on a case, especially when<br />

the patient needs you. I have had other sonographers<br />

that I work with share the same type of s<strong>to</strong>ries. At the<br />

end of the day, those cases keep us going because we<br />

know we do make a difference in the lives of those<br />

who may not be as <strong>for</strong>tunate as ourselves. And finally,<br />

from a personal side, was the first case I got <strong>to</strong> share<br />

with my daughter, Allison. She was one of my students<br />

at the time...it was the best ever...time s<strong>to</strong>od still. She<br />

has now graduated, passed her board exams and is<br />

employed full time as a sonographer in a Perinatal<br />

Center. Good stuff!<br />

What changes do you predict in the immediate<br />

future?<br />

We are at a pivotal time. It is a time <strong>for</strong> all<br />

sonographers, across all modalities, <strong>to</strong> unite. We need<br />

<strong>to</strong> shore up both sides of our profession including


manda<strong>to</strong>ry certification and/or credentialing of<br />

sonographers as well as <strong>to</strong> pave a way <strong>for</strong> a mid-level<br />

practitioner <strong>to</strong> ensure our place in global healthcare<br />

market. I feel that Point of Care sonography will<br />

continue <strong>to</strong> expand in untraditional venues.<br />

What advice would you give <strong>to</strong> students/ future<br />

sonographers?<br />

You make your own destiny. Get involved and let your<br />

voice be heard. I am the first <strong>to</strong> admit that all of you<br />

kids are WAY smarter than I was when I first started.<br />

You have the opportunity <strong>to</strong> mold our future. You<br />

have a lot <strong>to</strong> offer. Try and find a men<strong>to</strong>r <strong>to</strong> help you<br />

along the way. Tap in<strong>to</strong> their vast knowledge and use<br />

Maria Johnson is a first<br />

year student in the General<br />

<strong>Sonography</strong> program at El<br />

Centro College in Dallas,<br />

Texas. She is a mother of<br />

three children of which<br />

all are in college and<br />

developing their own<br />

professional careers. Maria<br />

was employed with AT&T<br />

<strong>for</strong> ten years where she<br />

worked as an account representative and manager.<br />

After her third child was born she was <strong>for</strong>tunate <strong>to</strong> be<br />

able <strong>to</strong> stay at home with her children. During that<br />

time she <strong>to</strong>ok the opportunity <strong>to</strong> become active in the<br />

community by volunteering in her children’s schools<br />

PTA and Boosters Clubs.<br />

We asked Maria why she decided <strong>to</strong> re-enter the<br />

work<strong>for</strong>ce and why she chose diagnostic medical<br />

sonography, she replied “When my kids started<br />

your innate skills <strong>to</strong> refine that in<strong>to</strong> a newer and fresh<br />

version that only your generation can do. And finally,<br />

remember that there is no room <strong>for</strong> mediocrity in<br />

medicine. Try your best...it will always serve you well.<br />

We are proud of you!<br />

What’s your favorite part of your job?<br />

I love clinical research and the time in the NICU<br />

scanning our premature infants. I love listening <strong>to</strong><br />

our staff and graduates teach each other. To see them<br />

“pass it <strong>for</strong>ward” not only with each other but also<br />

with the students makes me very proud of all<br />

of them.<br />

looking in<strong>to</strong> colleges I began <strong>to</strong> think about what<br />

my next steps would be in life. I knew I wanted<br />

<strong>to</strong> go back <strong>to</strong> work but I also felt that I wanted a<br />

career where I could make a difference. I had always<br />

regretted not getting in<strong>to</strong> healthcare because I’ve<br />

been around nurses all my life, my mother and aunt<br />

were both nurses.” So she researched many different<br />

areas in healthcare and found that she was drawn <strong>to</strong><br />

sonography due <strong>to</strong> the rapidly advancing technology<br />

as well as its importance in the diagnostic process.<br />

Maria said when she joined the SDMS; she was mostly<br />

excited about the free subscription <strong>to</strong> the Journal<br />

of Diagnostic Medical <strong>Sonography</strong> (JDMS). She said<br />

the JDMS is an important teaching <strong>to</strong>ol both <strong>for</strong> her<br />

current classes and continuing education as a future<br />

sonographer. Maria said, “Overall, my favorite benefits<br />

of SDMS, is the vast in<strong>for</strong>mation available <strong>to</strong> me<br />

on the website and in the JDMS. Between the two<br />

sources I have a plethora of valuable in<strong>for</strong>mation from<br />

exam scheduling in<strong>for</strong>mation <strong>to</strong> new technological<br />

advances and interesting pathology cases. <strong>This</strong><br />

in<strong>for</strong>mation has been invaluable <strong>to</strong> me as a new<br />

sonography student and makes me very excited <strong>to</strong> be<br />

getting started in this amazing field.”<br />
